Tendinitis and Tendinopathies About the Elbow and Forearm.

Lateral epicondylitis, de Quervain tenosynovitis, and trigger finger are among the most common tendinopathies of the upper extremity. Lateral epicondylitis is a common condition that can be very frustrating to patients. Many patients will improve without any acute interventions. Physical therapy has been shown to be helpful as initial treatment, and surgery can be helpful for persistent symptoms. de Quervain tenosynovitis similarly responds well to nonsurgical modalities, including brace treatment and injections, and surgery can be effective for recalcitrant cases. Finally, trigger finger is a

Treatment of Distal Forearm Fractures in Children.

BACKGROUND AND AIMS: Distal forearm is the most common fracture location in the growing skeleton. The aim of this article is to describe the current practice of these fractures. MATERIALS AND METHODS: Case series accompanied by experts' opinion. RESULTS AND CONCLUSION: Most of these injuries are benign incomplete distal metaphyseal torus fractures best treated with a dorsal below elbow splint for 2-3 weeks with no follow-up. Completely displaced metaphyseal fractures in prepubertal children can be either immobilized in bayonet position after axial alignment or fixed with K-wires after reduct…

Chronic exertional compartment syndrome of the forearm.

Chronic exertional compartment syndrome (CECS) is an overuse injury characterized by increased intracompartmental pressure during exercise. CECS has been described in the foot, thigh, and trunk, but 95% of cases occur in the lower leg. Interestingly, CECS may also affect the upper extremities and has been best described in the forearms. Unfortunately, due to the rarity of this condition, there is no consensus regarding its diagnosis and treatment. This review seeks to discuss the prevalence, etiology, diagnosis, and treatment of CECS of the forearms, which has been described in the literature.

Perioperative management of a bilateral forearm allograft.

Composite tissue allotransplantation (CTA) is a complex procedure requiring a multidisciplinary collaboration between surgeons, anesthetists, and transplantation specialists. We will describe the perioperative management of a bilateral forearm allograft performed at our facility. A 40-year-old man who lost both forearms was registered on the transplant waiting list; a suitable graft was available 11months later. Anesthesia required planning for vascular access, hemodynamic monitoring, fluid therapy management and prevention of deep vein thrombosis. Blood loss was not quantifiable, which made c…

Sonographic identification of peripheral nerves in the forearm.

BACKGROUND: With the growing utilization of ultrasonography in emergency medicine combined with the concern over adequate pain management in … gain proficiency in accurately locating and identifying the median, radial, and ulnar nerves of the forearm via ultrasound. METHODS: The proficiency outcome was defined as the number of attempts before a resident is able to correctly locate and identify the nerves on ten consecutive examinations. Didactic education was provided via a 1 h lecture on forea …

The Factors Influencing the Refracture of Pediatric Forearms.

PURPOSE: Pediatric forearm fractures are common and usually heal uneventfully. The purpose of this study was to review the refracture rate and to identify trends and risk factors that may lead to a refracture. METHODS: Using current procedure terminology code and subsequent chart review we retrospectively identified 2590 patients who sustained forearm fractures over the past 10 years (2000 to 2010) and were treated at a single, large pediatric orthopaedic practice. RESULTS: We identified 37 patients who met our search criterion which yielded a refracture rate of 1.4%. Average length of immob…

Treatment of diaphyseal forearm fractures in children.

Both bone forearm fractures are common orthopedic injuries. Optimal treatment is dictated not only by fracture characteristics but also patient age. In the pediatric population, acceptable alignment can tolerate greater fracture displacement due to the bone's ability to remodel with remaining growth. Generally, these fractures can be successfully managed with closed reduction and casting, however operative fixation may also be required. The optimal method of fixation has not been clearly established. Currently, the most common operative interventions are open reduction with plate fixation vers…

Linear milia en plaque on the forearm.

A 64-year-old man presented with asymptomatic eruption on his right forearm and the dorsum of the hand present for 2 weeks. There was no history of trauma, prolonged sun exposure, or application of or contact with any substance prior to the development of lesions. He was a known hypertensive and diabetic and was taking treatment for these conditions. The rest of his history was noncontributory. On examination, multiple grouped tiny white papules were found on both normal skin and on the erythematous plaque. These papules were of almost uniform size (2-4 mm) and were notable for absence of umbi…

Paediatric forearm fractures in the west of Scotland.

Forearm fractures are common paediatric injuries. This study aimed to describe the epidemiology and treatment of paediatric forearm fractures in the urban population of Glasgow. We reviewed all forearm fractures treated by the orthopaedic service in Yorkhill Children's Hospital in 2008 up to the age of 13 years. Data were gathered from case-notes and radiographs using the prospective orthopaedic database to identify patients with forearm fractures. The age, sex, side and type of fracture, the timing and mechanism of the injury and treatment were documented for the 439 fractures. Census data we…

Technique of forearm osteotomy for pediatric problems.

Correction of a rigid forearm deformity in children is often desired in congenital radioulnar synostosis, brachial plexus palsy, cerebral palsy, or posttraumatic torsional deformity. Osteotomies at the diaphyseal level present difficulties with maintenance of reduction, whether or not internal or pin fixation is used. The stabilizing and healing potential of the periosteum in these cases can be used to advantage in the correction of these deformities.

Compartment syndrome of the forearm: a systematic review.

In this systematic review, we examined the available evidence regarding compartment syndrome of the forearm. Applying our inclusion criteria, we found 12 articles for a total of 84 cases using the MEDLINE (Ovid) database. All were retrospective studies (level IV evidence). In this study, papers were analyzed for causes, diagnosis, treatment, methods of wound closure, functional outcome, and complications. The most common cause of compartment syndrome of the forearm in children was a supracondylar fracture, while in adults the most common cause was a fracture of the distal radius. The diagnosti…

A split and conjoined median nerve in the forearm.

We observed a variation of the median nerve that split into two divisions in the proximal forearm and conjoined into a single cord in the distal forearm before entering the carpal tunnel. The medial division was larger than the lateral division. An aberrant muscular slip from the flexor digitorum superficialis ran over the medial division of the split median nerve. The muscular slip over the medial division might have caused an entrapment syndrome of this nerve due to several muscular and cutaneous branches originating from the medial division. This variation of the median nerve has not been r…
