Applying machine learning to gait analysis data for disease identification.

A machine-learning framework to identify the specific disease afflicting certain patients diagnosed with Neurological and Neuromuscular Diseases (NND) or Juvenile Idiopathic Arthritis (JIA) using only gait analysis data is presented. Classifying such data into disease types consumes valuable clinical time that may be better spent. Effective classification also facilitates its future retrieval. To prove the feasibility of the approach, we applied it to the simpler case of identifying the disease class of patients with a view to extending the method to specific diseases in future work. Standard …

Gait analysis post anterior cruciate ligament reconstruction: knee osteoarthritis perspective.

Individuals with anterior cruciate ligament (ACL) reconstruction are at increased risk to develop knee osteoarthritis (OA). Gait analysis describing kinetics of the lower extremity during walking and stair use (stair ascent and stair descent) can provide insight to everyday dynamic knee joint loading. In this study, we compared lower extremity gait patterns of those with ACL reconstruction (>1 year) to a control group. Fifteen ACL reconstructed individuals and 17 healthy controls participated in this study. Knee extensor and flexor strength were assessed. Using inverse dynamics, lower extremit…

Reliability of three-dimensional gait analysis in cervical spondylotic myelopathy.

BACKGROUND: Gait impairment is one of the primary symptoms of cervical spondylotic myelopathy (CSM). Detailed assessment is possible using three-dimensional gait analysis (3DGA), however the reliability of 3DGA for this population has not been established. The aim of this study was to evaluate the test-retest reliability of temporal-spatial, kinematic and kinetic parameters in a CSM population. METHODS: Twelve patients with CSM (mean age 54 years) were consecutively recruited from a neurosurgery clinic. 3DGA was conducted on 2 separate days, less than 1 week apart, using the VICON(®) 250 Moti…

Kinematic differences in lower limb gait analysis of scoliotic subjects.

Although various factors have been attributed to the etiology of idiopathic scoliosis, studies have indicated that the kinematic differences in the spine, pelvis and lower limb may contribute to the causation and progression of idiopathic scoliosis. The aim of this investigation was to identify asymmetries in lower limb kinematics and pelvic and back movements during level walking in scoliotic subjects that can be related to the spinal deformity. The study has employed a movement analysis system to estimate various joint angles in the lower extremities and other kinematic parameters in the pel

Tibialis anterior transfer for calcaneal deformity: a postoperative gait analysis.

We retrospectively evaluated seven children who had low-lumbar-level spina bifida and who had undergone bilateral transfer of tibialis anterior to the calcaneus. The mean age at the time of operation was 8 years (range, 3-12), and the patients were monitored for an average of 40 months (range, 24-60). All children underwent a postoperative gait analysis to assess the function of the transfer and the need for continued postoperative bracing. Transfer of the tibialis anterior muscle to the calcaneus arrested progression of the calcaneal deformity; however, the transfer could not prevent excessiv…

Analysis of morphology and gait function after intraarticular calcaneal fracture.

Open reduction and internal fixation is suggested by an increasing number of investigators as preferable treatment of displaced intraarticul … eal fractures using standard radiographic and computed tomographic scores, clinical evaluation, and gait analysis (dynamic pedography). Mean follow-up time after reconstruction was 23 months (range 18-50). Although clinical evaluation and assessment of gait function corresponded well with each other, radiographic scores showed a poor to moderate correlation wit …

Gait analysis. Alterations in support phase forces using supportive devices.

This study documents changes in momentary distribution of forces under the foot, comparing barefoot gait to that with heel cups, medial arch supports, and low-dye taping. Cholesterol crystal force plate analysis and a computerized Kistler force platform were used in the study. Low-dye taping or a heel cup significantly diminished the duration of forces under the midfoot, medializing the instant center of forces. A medial arch support shifted the instant center of forces laterally, though it did not diminish the duration of forces under the arch. The relationship between these alterations of fo

Parallel Factor Analysis of gait waveform data: A multimode extension of Principal Component Analysis.

Gait data are typically collected in multivariate form, so some multivariate analysis is often used to understand interrelationships between observed data. Principal Component Analysis (PCA), a data reduction technique for correlated multivariate data, has been widely applied by gait analysts to investigate patterns of association in gait waveform data (e.g., interrelationships between joint angle waveforms from different subjects and/or joints). Despite its widespread use in gait analysis, PCA is for two-mode data, whereas gait data are often collected in higher-mode form. In this paper, we p…

Efficacy of repeated external cueing training on freezing of gait and gait performance in Parkinson's disease: a systematic review and meta-analysis.

BACKGROUND: Freezing of gait (FOG) is a disabling gait disturbance in Parkinson's disease (PD) that responds poorly to pharmacological treatment. External cueing has been proposed as a non-pharmacological strategy to improve gait and mobility; however, the effectiveness of repeated external cueing training on FOG-related outcomes remains uncertain, particularly across different cueing modalities. METHODS: We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs), with a specific focus on repeated external cueing interventions (≥2 weeks). The influence of spasticity in the lower limb muscles on gait pattern in children with sacral to mid-lumbar myelomeningocele: a gait analysis study.

Gait analysis and recording of standing position were performed in 38 ambulatory children with myelomeningocele. Thirty-four were independent ambulators and four required a walking aid. All subjects were assigned one of four muscle function groups based on muscle strength. They were also divided into subgroups based on the distinction between flaccid and spastic paresis in the lower limb joints. A comparison was made between the gait pattern of the children with spasticity and that of the children with flaccid paresis in each muscle function group. Spasticity in only the ankle joint muscles in…

This is a narrative review of the functional evaluation of clubfoot treatment with gait analysis.

Clinicians worldwide have embraced Ponseti's nonoperative approach in the treatment of clubfoot, primarily due to ubiquitous reports of successful outcomes. A crucial component in this measured success, has come from researchers assessing long-term physical function following nonoperative treatment. Gait analysis has been instrumental in objectively evaluating lower extremity kinematics and kinetics while plantar pressures demonstrate the load bearing patterns experienced in the foot. As technology improves, our ability to evaluate function can take place both in the laboratory setting, and in…
