Among 173 patients undergoing reconstructive gastric operations, including 115 patients who received Roux-en-Y gastrojejunostomy, 25 with jejunal interpositions, 11 with conversion of Billroth II to Billroth I, 8 with closure of gastroenterostomy, 5 with pyloric reconstruction and 9 with other operations, only 2 immediate postoperative mortalities occurred. A perfect or good result was obtained in 108 of 156 patients (69 per cent) at long term follow-up, while a fair result was achieved in 35 patients (22 per cent) and a poor result in 13 patients (9 per cent). We concluded that reconstructive…

Since 1961, the authors have treated 369 patients for a duodenal ulcer. Emergency operation was performed in 69 cases (mortality: 7.2 percent) and elective operation in 300 (mortality: 0.86 percent. Procedures associated truncal vagotomy to hemigastrectomy (257 cases), to pyloroplasty (85 cases) or to gastroenterostomy (27 cases). Three hundred and thirty-nine patients were followed for an average of 8 years. The authors preferred truncal vagotomy associated to hemigastrectomy. This yielded excellent long term results (Visick I) in 89 percent of cases (221 out of 246 reviewed cases), with a ve…

Two clinical variants of jejunogastric intussusception are described. In one, self-reduction is the rule, whilst in the other strangulation occurs and emergency surgery is mandatory. A case illustrating each variant is presented and its diagnosis and treatment are discussed.

An analysis of the causes of death of 85 patients with cicatricial esophageal obstruction after chemical burns is presented. Suppurative complications were most frequently among them: mediastinitis, peritonitis, and aspirational pneumonias. They developed in exacerbations of chronic esophagitis, trauma of the esophageal wall by instrumental interventions and because of necroses of the transplants or incompetence of sutures of the created gastroenterostomies in esophagoplastics or esophagus extirpation. Early application of mediastinal lavage with aspiration of its contents led to delimitation

Percutaneous gastrostomies or gastroenterostomies serve for temporary or permanent enteric feeding in patients with obstruction or functional derangement of the esophagus or hypopharynx. In addition, this radiological procedure may be indicated for small bowel decompression. The authors present their experience in 71 patients. Insufflation of air through a nasogastric tube or catheter is the preferred method for gastric distension. The inferior margin of the left lobe of the liver and the transverse colon are localized sonographically and fluoroscopically prior to puncture. Either Seldinger or

The experience with 25 patients who required operation for Crohn's disease involving the duodenum is reviewed. Two distinct patterns of duod … dity associated with vagotomy in our series and the high incidence of marginal ulcers reported with gastroenterostomy in other clinical settings lead us to recommend gastroenterostomy with vagotomy as the procedure of choice for duodenal Crohn's disease. Proceeding with vagotomy in persons who have had previous ileocecal or extensive small bowel resection should no …

Some considerations on the use of staplers, use currently classified as "elective" and "of convenience", are shown. The Author wishes a change in this classification especially for what elective subtotal gastrectomy is concerned. In fact, the use of staplers in subtotal gastrectomy for tumors of the pyloric antrum and for complicated ulcer assures considerable advantages in terms of quickness, safety and sterility. Finally, the Author describes in detail a Billroth II subtotal gastrectomy performed with staplers.

Jejunogastric intussusception is a rare complication after gastric surgery. The authors report a case of acute jejunogastric intussusception diagnosed in a 57-year-old woman, 22 years after vagotomy and gastroenterostomy for duodenal ulcer. There are three types of jejunogastric intussusception: 1) the acute type, presenting as a surgical emergency, and characterized by a sudden onset of cramp-like epigastric pain, followed by nausea and vomiting, with a palpable epigastric mass. 2) the chronic recurrent type, which may progress to the acute type of may result in severe disability, and may req…

The results of treatment of 317 patients with concurrent burn of the esophagus and stomach are generalized. Various restorative operations were carried out in 214 of them. The pyloroplasty-type plasty with local tissues was performed in short strictures of the pylorus, cardioesophageal junction, duodenum, and the initial portion of the small intestine. Resection of the stomach after Billroth I was conducted in cicatricial affection of the distal part of the stomach. The Billroth II operation or gastroenterostomy is indicated in subtotal burn and combined cicatricial stenosis of the stomach and…

BACKGROUND: Afferent loop syndrome (ALS) is an uncommon but clinically relevant complication after pancreaticobiliary or upper gastrointestinal reconstruction, most often in patients with recurrent malignant disease. Endoscopic ultrasound-guided creation of a transmural bypass, either as gastroenterostomy or enteroenterostomy, has emerged as a minimally invasive alternative to surgery or percutaneous drainage. We aimed to systematically review the available evidence and provide a pooled descriptive analysis of the efficacy and safety of EUS-guided treatment for ALS.
METHODS: A systematic revi…

The major cause of peritonitis in bariatric surgery is leakage of GI contents, which can have a catastrophic outcome for the bariatric patient. To resolve this serious problem, the surgeon must act quickly. This paper describes a 27-year-old female after gastric bypass with disruption of the gastroenterostomy and severe contamination and peritonitis. Closure of the anastomotic leak, drainage, and gastrostomy in the bypassed stomach were performed, but the abdomen could not be closed, due to dilated bowel and the intra-abdominal edema with the sepsis. Temporary laparostomy closure was performed…
