Gastrointestinal transit and prolonged ambulatory colonic motility in health and faecal incontinence.

BACKGROUND: Colonic motor function has not been studied in the ambulatory setting over a prolonged period in the unprepared state. Furthermore, the disturbance of this function in patients with faecal incontinence is unknown. AIM: To study colonic function over two to three days in the ambulatory, unprepared state in health and in patients with idiopathic faecal incontinence. METHODS: Six healthy women and six women with faecal incontinence and a structurally intact anal sphincter ingested a dual radioisotope meal, and had a six sensor, solid state manometric probe colonoscopically inserted

[Gastrointestinal motility disorders in late stages of adhesive intestinal obstruction in children].

The results of examination and treatment of 211 patients with late adhesive ileus (LTAI) are discussed. Highly sensitive noninvasive method of phonoenterography was used for LTAI diagnosis and control of the efficacy of non-operative treatment. The gastrointestinal motor-evacuation disorders were studied in a group of patients with chronic recurring LTAI. For revealing hypertension in the upper parts of the gastrointestinal tract, a special catheter is suggested by means of which the pressure in the gastric and duodenal cavities can be measured simultaneously. The suggested examination methods

Congenital failure of automatic control of ventilation, gastrointestinal motility and heart rate.

A new congenital syndrome characterized by the simultaneous failure of control of ventilation (Ondine's curse) and intestinal motility (Hirschsprung's disease) is reported in three infants, all of whom died in the first few months of life; two were siblings. Detailed studies in one also revealed markedly decreased esophageal motility and abnormal control of heart rate. In one infant, minute ventilation was lower in quiet than in REM sleep and lower in both states of sleep than in wakefulness. Although the mean inspiratory flow was decreased in quiet sleep, the hypoventilation resulted primaril

Gastrointestinal Symptom Events and Motility Alterations Associated With GLP-1 Receptor Agonists.

BACKGROUND: Gastrointestinal symptom events (GISE) are common with glucagon-like peptide-1 receptor agonists (GLP-1RA). We aimed to determine the prevalence of GISE and clinical factors associated with GISE. METHODS: An integrated platform database was used to collect real-world data. Using a retrospective cohort study design, Kaplan-Meier analysis was used to estimate GISE-free survival over 60 months. To identify predictors of GISE, the cohort was randomly split 80/20 into training and testing sets. A LASSO-penalized Cox model was used to identify relevant associations, after which main-eff

Evolving prokinetic therapy: New targets and therapeutic opportunities in gastrointestinal motility disorders.

Gastroparesis, functional dyspepsia, chronic constipation, esophageal motility disorders, and colonic dysmotility are among the gastrointestinal (GI) motility disorders that constitute a significant global health burden. These disorders lead to reduced quality of life, higher healthcare utilization, and substantial socioeconomic costs. Therapeutic options are still scarce despite their prevalence. Prokinetic medications currently on the market offer only modest symptomatic relief, frequently reaching a plateau in efficacy, and their long-term use is limited by safety concerns, especially those

Polyphenols as Modulators of Gastrointestinal Motility: Mechanistic Insights from Multi-Model Studies.

Dietary polyphenols are recognized as crucial modulators of gastrointestinal motility, holding therapeutic promise for conditions like irritable bowel syndrome, postoperative ileus, and functional dyspepsia. However, their reported effects are heterogeneous, ranging from spasmolytic to prokinetic. This review aims to clarify these inconsistencies by synthesizing experimental evidence on structure-activity relationships and underlying mechanisms. Relevant publications were identified in PubMed and Google Scholar using terms related to polyphenols and gastrointestinal motility. References were s…

Oxytocin regulates gastrointestinal motility, inflammation, macromolecular permeability, and mucosal maintenance in mice.

Enteric neurons express oxytocin (OT); moreover, enteric neurons and enterocytes express developmentally regulated OT receptors (OTRs). Although OT (with secretin) opposes intestinal inflammation, physiological roles played by enteric OT/OTR signaling have not previously been determined. We tested hypotheses that OT/OTR signaling contributes to enteric nervous system (ENS)-related gastrointestinal (GI) physiology. GI functions and OT effects were compared in OTR-knockout (OTRKO) and wild-type (WT) mice. Stool mass and water content were greater in OTRKO mice than in WT. GI transit time in OTRK

Systemic oxytocin and vasopressin excite gastrointestinal motility through oxytocin receptor in rabbits.

The aim of the present study was to investigate the effect of systemic oxytocin (OT) and vasopressin (VP) on the motility of stomach and duodenum. Two plastic balloons made of condom were inserted into stomach and duodenum to monitor the change of mean pressure. Intravenous injection of OT (0.1-0.8 microg kg(-1)) or VP (0.02-0.08 IU kg(-1)) dose-dependently increased the stomach and duodenum pressure. Pretreatment of atosiban (1 microg kg(-1)), the specific OT receptor (OTR) antagonist, attenuated the excitatory effect of OT or VP on the pressure of stomach and duodenum. Pretreatment of V1880

[Gastrointestinal surgery and gastroenterology. VII. Proximal motility disorders in the digestive tract].

Dysphagia and chest pain are the two commonest symptoms of abnormalities of oesophageal motility. Dysphagia is to be distinguished into high or oropharyngeal and low or oesophageal dysphagia. Oropharyngeal dysphagia pertains to dysfunction of the pars cricopharyngea of the M. constrictor pharyngis inferior (M. cricopharyngeus), which is frequently associated with a Zenker diverticulum. Treatment consists of endoscopical or surgical myotomy and diverticulectomy. In achalasia there is incomplete relaxation of the lower esophageal sphincter with aperistalsis. The main treatment modalities are end

Improved motility in the gastrointestinal tract of a postoperative ileus rat model with ilaprazole.

Postoperative ileus (POI), a symptom that occurs after abdominal surgery, reduces gastrointestinal motility. Although its mechanism is unclear, POI symptoms are known to be caused by inflammation 6 to 72 h after surgery. As proton pump inhibitors exhibit protective effect against acute inflammation, the purpose of this study was to determine the effect of ilaprazole on a POI rat model. POI was induced in rats by abdominal surgery. Rats were divided into six groups: control: normal rat + 0.5% CMC-Na, vehicle: POI rat + 0.5% CMC-Na, mosapride: POI rat + mosapride 2 mg/kg, ilaprazole 1 mg/kg: POI…
