Human gnathostomiasis is most frequently caused by the nematode Gnathostoma spinigerum. This disease is endemic to Southeast Asia, particularly Thailand and Japan. The clinical presentation is most commonly characterized by localized, intermittent, migratory swellings of the skin and subcutaneous tissues, often in association with localized pain, pruritus, and erythema. Since this worm can migrate to deeper tissues, any organ system may become involved. Characteristically, patients with gnathostomiasis have a moderate to severe elevation of the peripheral eosinophil count, with values not unco…

A 42-year-old Thai man from central Thailand came to Ramathibodi Hospital with the complaint of abdominal pain. Physical examination revealed localized tenderness in the right lower quadrant. The clinical impression was acute appendicitis and an operation was performed, revealing a large mass in the cecum. A right hemicolectomy was done for what was thought to be a carcinoma of the colon. The resected colon showed subserosal thickening on the antimesenteric side of the ascending colon with a constricted area at the middle portion. An immature adult Gnathostoma spinigerum was identified in the

Gnathostomiasis is increasingly reported among travelers returning from endemic areas. Between 2000 and 2004, thirteen patients were diagnosed with imported gnathostomiasis and followed for at least 6 months after treatment. Nine patients presented with cutaneous signs, two with gastrointestinal signs, and two with neurological signs. The median age was 38 years and the female/male sex ratio was 1.6. The patients had visited South East Asia or Central America. The median interval between symptom onset and treatment (with albendazole in 12 cases and ivermectin in one case) was 3.5 months. Post-…

Human gnathostomiasis, which is endemic in Mexico, is a worldwide health concern. It is mainly caused by the consumption of raw or insufficiently cooked fish containing the advanced third-stage larvae (AL3A) of Gnathostoma species. The diagnosis of gnathostomiasis is based on epidemiological surveys and immunological diagnostic tests. When a larva is recovered, the species can be identified by molecular techniques. Polymerase chain reaction (PCR) amplification of the second internal transcription spacer (ITS-2) is useful to identify nematode species, including Gnathostoma species. This study a…

In this article, we described a rare atypical clinical case of tropical helminthiasis - a visceral gnathostomiasis with simultaneous damage of the upper and lower parts of the gastrointestinal tract. The visceral form is the rarest variant and among the literary data, only single cases of this form are described in the world. Therefore the management of such patients requires an individual and balanced approach since it is almost impossible to predict the consequences of any medical interventions. This determines the clinical uniqueness of this case and requires further research of clinical gu…

Gnathostoma spinigerum and Angiostrongylus cantonensis are human parasites that can cause neurological symptoms. The human diseases produced by these parasites can usually be differentiated by clinical symptoms. The aim of this study was to report neuroimaging abnormalities detected with computed tomography (CT) and MR in patients with gnathostomiasis and angiostrongyliasis. We enrolled 15 and 12 patients with serologically proven gnathostomiasis and angiostrongyliasis, respectively, who had brain or spinal imaging done. The neuro-gnathostomiasis group had significantly more patients with intr…

A 48-year-old French diplomat presented with a sensory-motor paraparesis of rapid onset, leading to paraplegia. Successive magnetic resonance image scans showed lesions of the thoracic spinal cord that were at different levels from one examination to the next. Specific anti-gnathostome antibodies were detected by means of enzyme-linked immunosorbent assay and Western blot test in both plasma and cerebrospinal fluid. Albendazole treatment prevented disease progression, but only partial regression of the neurologic symptoms was obtained.

We describe a 21-year-old patient who experienced a relapse of cutaneous gnathostomiasis after receiving initial treatment with albendazole and who had a successful outcome after receiving a short course of ivermectin for the relapse. This is the first reported case of gnathostomiasis acquired by a human in Peru.

The authors report a case of cutaneous Larva migrans attributed to a gnathostomiasis in a Laotian man living in France since 3 years. The diagnosis was suggested on the basis of epidemiological and clinical context (he often ate raw fresh fish in Thailand) and blood eosinophilia. It was confirmed by ELISA method which showed a very high antibody titer to Gnathostoma spinigerum. The patient was treated with cortisone and diethylcarbamazine and he has remained asymptomatic since.

Five confirmed human cases of gnathostomiasis nipponica exhibiting creeping eruption and itching were found sporadically from the autumn of 1991 to the winter of 1992 in the northern region of the mainland of Japan. In all cases, a causative gnathostome with 3 transverse rows of hooklets on the head bulb was detected in biopsied skin. The morphological characteristics agreed with the advanced third-stage larvae of Gnathostoma nipponicum. Within a few weeks before development of symptoms, all patients had histories of eating raw freshwater fishes, kokanee (Salmo nerka nerka), carp (Cyprinus car…

BACKGROUND: Spinal gnathostomiasis results in serious multiorgan impairments. Magnetic resonance imaging (MRI) and serology help in diagnosis, and assessing the severity and extent of the disease. However, the physiology of neural pathway could not be demonstrated. Electrodiagnosis may have a role in demonstrating the functions of the central and peripheral neural pathways and prognostic assessment of the disease.
MATERIAL AND METHOD: An 18-year-old man presented with radicular pain and rapid progressive weakness of lower extremities, leading to paraplegia in three days. A clinical evaluation…

BACKGROUND: Cutaneous gnathostomiasis is an emerging food-borne parasitic zoonosis. Histopathological demonstration of the larva on random biopsy specimen of erythematous plaques is infrequent because of its migrating nature.
OBJECTIVE: We sought to determine whether medical treatment with albendazole or ivermectin increases the diagnostic yield of skin biopsy specimen.
METHODS: A retrospective chart review was conducted in a private dermatology practice in Lima, Peru. Cases with a clinical diagnosis of nodular migratory panniculitis and pathological diagnosis of eosinophilic panniculitis or…

Cerebral gnathostomiasis is a severe form of human infection caused by Gnathostoma spinigerum. We report sequential brain imaging in serologically proven cerebral gnathostomiasis. The clinical and radiographic correlation showed the probable route taken by the parasite during its migration through the central nervous system. We offer additional insight into pathogenesis of cerebral gnathostomiasis and its rare complication, subdural hemorrhage, a finding not previously reported in the literature.

Previous studies have revealed that ivermectin treatment for gnathostomiasis can reduce parasitic loads in animals and make recurrent subcutaneous swelling subside in 76% of patients. Our study aimed to evaluate the efficacy of ivermectin for cutaneous gnathostomiasis treatment in a placebo-controlled trial. This study was a prospective randomized placebo-controlled study performed at The Bangkok Hospital for Tropical Diseases, Mahidol University, Thailand. Thirty patients with a serologically confirmed diagnosis of cutaneous gnathostomiasis were enrolled. Seventeen patients in the ivermectin …

Methods for the detection of antigens, antibodies and immune complexes in the cerebrospinal fluid (CSF) of patients with neurological manifestations suggestive of cerebral gnathostomiasis were developed, in the hope that they may be useful in confirming the diagnosis of Gnathostoma spinigerum infection. Gnathostoma antigens were determined by a sandwich enzyme linked immunosorbent assay (ELISA) using antibodies from rabbits immunized with the excretory/secretory (ES) antigens obtained from the in vitro supernatant fluid in which the third-stage G. spinigerum larvae were maintained. With a biot…

Cutaneous gnathostomiasis (CG) is a disease caused by ingestion of third-stage Gnathostoma larva in raw snakes, freshwater fish or frogs. The common causative organisms of CG in Japan include G. nipponicum, G. spinigerum, G. doloresi, G. binucleatum and G. hispidum. We report two cases of CG after eating many raw Japanese icefishes (Salangichthys microdon). In both cases, linear itchy eruptions on the trunk developed after eating many S. microdon. We performed genetic analysis in the first case, which revealed G. nipponicum. Of note, this is the first case of CG diagnosed based on genetic anal…
