Transcarpal carpometacarpal dislocations, excluding the thumb.

During a 9-year period, we treated 13 patients with major fractures of bones of the distal carpal row creating instability of the corresponding carpometacarpal joints. Diagnosis of the injury was made early in only eight patients whose treatment consisted of open reduction, pin or screw fixation, and immobilization from 6 to 8 weeks. They showed no complications and had an excellent functional recovery. Diagnosis in five (38%) patients was delayed 1 week to 8 months after the accident. At initial presentation, symptoms included pain, loss of grip strength, and mild limitation of finger motion.

Subcutaneous rupture of the flexor tendon of the little finger associated with anomalous flexor digitorum superficialis.

A 59-year-old man suffered from subcutaneous rupture of the flexor tendon of the little finger associated with fracture of the hook of hamate. He could not flex his little finger completely at the distal interphalangeal joint, but incomplete flexion of the proximal interphalangeal joint was possible. Surgical exploration revealed anomaly of the flexor digitorum superficialis of the little finger, as it originated from the palmar aspect of the carpal ligament, and a small portion of the muscle belly was traversed toward the A1 pulley over the profundus tendon and then it ran into the A1 pulley

Closed Rupture of the Flexor Tendon Secondary to Sclerosis of the Hook of the Hamate: A Report of Two Cases.

Closed flexor tendon ruptures in the little finger can be caused by fracture or nonunion of the hook of the hamate, but no case of the disorder secondary to the sclerosis and thinning of the hamate hook has been reported. We report two rare cases with this complication due to rough surface of the hamate hook. Carpal tunnel view radiographs and computed tomography showed the sclerosis and thinning of the hook.

Value of bone scintigraphy in patients with carpal trauma.

OBJECTIVE: We planned this study to evaluate the role of bone scintigraphy in patients with suspected carpal fracture and normal or suspicious radiographs following carpal injury. METHODS: Three-phase bone scintigraphy using Tc-99m-MDP was performed on 32 patients with negative radiographs but clinically suspected fracture at two weeks after the trauma. Focally increased radiopharmaceutical uptake was interpreted as a fracture. The final diagnosis was established with clinical follow-up. RESULTS: Twelve (38%) patients had a normal scan excluding fracture. Twelve patients had a single fractur

The vascularity of the wrist. Identification of arterial patterns at risk.

The intraosseous and extraosseous vascularity of carpal bones was studied in 75 cadaver limbs. Vascular patterns were correlated with the clinical incidence of avascular necrosis, and at-risk patterns of vascularity were identified. The carpal bones were found to fall into three groups. Group 1, which consisted of the scaphoid, the capitate, and eight percent of the lunates, had either vessels entering only one surface or large areas of bone that were dependent on a single vessel. This group was the most vulnerable to posttraumatic avascular necrosis. The bones of Group 2, the hamate and the t

Isolated Osseous Excision in the Adult Carpus: A Narrative Review.

Various pathologies of the adult carpus result in clinical scenarios where excision can be considered and even recommended. In the appropriate patient population, isolated carpal excision can alleviate pain and improve mobility. Excisions of the pisiform, trapezium, and trapezoid have abundant literature evidence to support positive long-term functional outcomes. In contrast, isolated excision of the capitate, hamate, and triquetrum has limited support in the literature secondary to compromise of carpal mechanics and lead to recurrent pain. Additionally, isolated scaphoid and lunate excision a

Bone fusion in the foetal alcohol syndrome.

Clinical examinations and radiographic skeletal surveys have been carried out in 15 patients with foetal alcohol syndrome. Fusion of the capitate and hamate bones in the carpus was bilateral in one patient and unilateral in two. All three had accessory ossification centres at the proximal ends of both second metacarpals. Two of these patients also had radio-ulnar synostosis. Digital shortening, which was demonstrated by pattern profile analysis, was very variable in degree and anatomical distribution. Other skeletal changes of uncertain significance were a "beaten copper" appearance of the cal…

The anatomy of the ligaments of the wrist and distal radioulnar joints.

The ligaments of the wrist are responsible for guiding and constraining the complex motion of the carpal bones relative to the forearm bones, the metacarpals, and contiguous carpal bones. The majority of wrist ligaments are found within the joint capsule as organized thickenings composed of parallel collagen fascicles, small caliber nerves and blood vessels, and lined on their deep surfaces by synoviocytes. The palmar radiocarpal ligament complex is composed of the radioscaphocapitate, long radiolunate, radioscapholunate and short radiolunate ligaments. The ulnocarpal ligaments include the uln

Bone-tissue-bone repairs for scapholunate dissociation.

Scapholunate dissociation is a commonly seen and treated form of carpal instability. Several surgical options have been used for the repair of scapholunate instability over the past 50 years. These have included benign neglect, reduction and percutaneous pinning, primary repair, partial fusions, tendon weaves, and combinations of these. Recent advancements in scapholunate repair and anatomy have been aimed at a more physiologic repair. Composite replacement of the entire scapholunate interval, similar to other tendon repairs seen in orthopedic surgery, has become popular. Bone-tissue-bone (BTB

Classification of Hamate Fractures: An Analysis of 247 Patients with Hamate Fractures.

Background: We devised a new classification of hamate fractures named the TOUCH classification. Each letter of this acronym depicts a fracture type - Type I (Transverse fracture), Type II (Open and/or complex fracture), Type III (Ulnar/medial tuberosity fracture), Type IV (Coronal fracture) and Type V (Hook fracture). Each fracture type was further divided into two or three subtypes (a, b, and/or c) based on degree of severity. The aim of this study is to classify the hamate fractures treated at our centre using this classification. Methods: A retrospective review of all patients with hamate f

Hemi-hamate arthroplasty for dorsal fracture-dislocation of proximal interphalangeal joint.

Dorsal fracture-dislocations of the proximal interphalangeal (PIP) joint, often accompanied by palmar lip fractures, are commonly seen in hand trauma. Management of these fractures requires restoring joint congruity and stability while allowing for early mobilisation. This case report discusses the successful use of a hemi-hamate autograft for reconstruction of the base of the middle phalanx in a young male patient with a severe dorsal PIP joint fracture-dislocation. The results demonstrate a promising technique for joint restoration with minimal functional impairment and excellent postoperati

The three-dimensional shape symmetry of the lunate and its implications.

We studied the three-dimensional (3-D) shape variations and symmetry of the lunate to evaluate whether a contralateral shape-based approach to design patient-specific implants for treatment of Kienböck's disease is accurate. A 3-D statistical shape model of the lunate was built using the computed tomography scans of 54 lunate pairs and shape symmetry was evaluated based on an intraclass correlation analysis. The lunate shape was not bilaterally symmetrical in (1) the angle scaphoid surface - radius-ulna surface, (2) the dorsal side and the length of the side adjacent to the triquetrum, (3) the

Excision of the Distal Pole of the Scaphoid and the Midcarpal Joint.

Background  Excision of the distal pole of the scaphoid is used to treat arthritis of the scaphotrapezial trapezoid (STT), radioscaphoid joint, and arthritis following scaphoid nonunion. Some patients develop midcarpal instability limiting utilization of this technique. Why some wrists develop postoperative instability while others do not, remains unclear. Purpose  To identify the wrists prone to developing midcarpal joint instability we evaluated the effect of midcarpal joint structure on force transfer through the wrist, we hypothesized that the force transfer will be further altered when a

A longitudinal study of carpal bone development in Perthes' disease: its significance for both radiologic standstill and bilateral disease.

This paper reports a longitudinal study of carpal bone development in 125 children (98 boys, 27 girls) attending a clinic for Perthes' disease. The age at appearance of ossification in each carpal bone was estimated by studying consecutive radiographs obtained at six-months intervals. The results were compared with the findings for normal children published by Stuart et al. Three radiologic groups of carpal bone maturation are defined: not delayed (capitate and hamate); markedly delayed (triquetral and lunate); and slightly delayed (scaphoid, trapezium, and trapezoid). Overall, the bone age de
