Atrial septal defect closure as second-line therapy in refractory heart failure: a successful case report in a 6-year-old child with phosphoglucomutase 1 deficiency (PGM1-CDG).

BACKGROUND: Atrial septal defect (ASD) is a common congenital heart defect that remains mainly asymptomatic in childhood. However, when left ventricular (LV) compliance is impaired, as in myocarditis or cardiomyopathy, ASD may lead to severe heart failure due to increased left-to-right shunting and a reduced systemic cardiac output. CASE SUMMARY: We report the case of a 6-year-old boy with CDG-PGM1 and a large ASD who developed acute refractory heart failure, likely triggered by Parvovirus B19 myocarditis. Despite optimal medical therapy, including immunoglobulins, inotropes, and heart failur

[Spontaneous Closure of Atrial Septal Defect Caused Severe Heart Failure in a Child with Pulmonary Atresia and Intact Ventricular Septum Having an Extracardiac Total Cavopulmonary Connection;Report of a Case].

Unobstructed atrial communication is a prerequisite for favorable pulmonary circulation in patients with single-ventricle physiology and atrial-communication-dependent pulmonary venous drainage. It might also be a prerequisite for favorable coronary circulation in patients with single-ventricle physiology and atrial-communication-dependent coronary venous drainage. Obstruction of atrial communication, on which coronary venous drainage depends, has not yet been documented. We report a patient with pulmonary atresia and an intact ventricular septum having an extracardiac total cavopulmonary conn

Usefulness of early diastolic mitral annular velocity to predict plasma levels of brain natriuretic peptide and transient heart failure development after device closure of atrial septal defect.

Device closure of atrial septal defect (ASD) is sometimes followed by elevation of plasma brain natriuretic peptide (BNP), a marker of heart failure, and progression to heart failure. This study tested the hypothesis that the underlying diastolic dysfunction, assessed on tissue Doppler images (TDI) before device closure, can predict BNP level after ASD closure. The study subjects were 39 consecutive patients (age 27.5 +/- 16.3 years, range 5 to 63) who underwent device closure for ASD. Echocardiographic evaluation using TDI and 2-dimensional and pulse wave Doppler were performed, together with

Application of a modified clinical classification for pulmonary arterial hypertension associated with congenital heart disease in children: emphasis on atrial septal defects and transposition of the great arteries. An analysis from the TOPP registry.

AIMS: A proportion of patients with pulmonary arterial hypertension associated with congenital heart disease (PAH-CHD) do not fit in the current classification. We aimed to analyse the applicability of an adapted clinical classification of PAH-CHD to pediatric patients using the TOPP-1 registry (Tracking Outcomes and Practice in Pediatric Pulmonary Hypertension) and focus on atrial septal defects (ASD) and transposition of the great arteries (TGA). METHODS AND RESULTS: Hemodynamic and clinical data of all patients with PAH-CHD in the TOPP cohort were reviewed. Patients were classified accordi

Percutaneous closure of atrial septal defect: a consensus document of the joint group of experts from the Association of Cardiovascular Interventions and the Grown-Up Congenital Heart Disease Section of the Polish Cardiac Society.

Atrial septal defect is the most common congenital heart lesion in adults. Although atrial septal defect closure is recommended in those with right heart enlargement or paradoxical embolism, data informing such indications in adults are quite limited. This population has many unique characteristics and needs. In recent years, significant progress has been made with regard to diagnostic modalities that facilitate the diagnostic workup of these patients. However, the decision‑making process, especially in selected adult patients population (advanced age, various concomitant diseases, poor LV fun

Left atrial isomerism combined with right-sided heart, common atrium, incomplete atrioventricular septal defect and partial anomalous systemic and pulmonary venous drainage. Report of a case with successful surgical correction.

Precise preoperative diagnostic clarification was accomplished in an infant with left atrial isomerism. The inner/outer arrangement of the pulmonary/systemic veins in relation to the posterior wall of the common atrium necessitated a specific baffle in order to prevent pulmonary or systemic venous obstruction. The development of a sick sinus syndrome six months after surgery is in keeping with the natural history of the disease.

Addressing the needs of treating congenital heart disease: The influence of deficient retro-aortic rim on technical success and early adverse events following device closure of secundum atrial septal defects: An analysis of the IMPACT registry®.

For secundum ASD, a deficient retro-aortic rim is common (40% of cases) However, the deficient retro-aortic rim had no effect on ASD device placement success or immediate outcome The IMPACT Registry does not track long-term outcome, so there is no information provided about long-term complications, such as erosion.

Secundum atrial septal defect (ASD) is the most common adult congenital heart defect and can present with wide variation in clinical findings. With the intention of preventing morbidity and mortality associated with late presentation of ASD, consensus guidelines have recommended surgical or percutaneous ASD closure in adults with right heart enlargement, with or without symptoms. The aim of the present analysis was to determine if the protective effect of secundum ASD closure in adults could be qualified by pooling data from published studies. Surgical repair of a large residual atrial septal defect after transcatheter closure.

Transcatheter closure of ostium secundum atrial septal defects is a less invasive method of repairing atrial septal defects in comparison with an open heart operation. In selected patients the transvenous closure of atrial septal defects may be an effective alternative. Defects with circumferential septal rims, secundum atrial defects, and patent foramen are the best candidates for this method. We report a case in which a large residual atrial septal defect after transvenous closure with a Sideris occluder device required surgical closure.

Robotic, totally endoscopic atrial septal defect repair.

Atrial septal defect accounts for 10-15% of congenital heart disease cases. Small-diameter atrial septal defects diagnosed during infancy or early adulthood are prone to spontaneous closure, whereas uncorrected, persistent moderate or large atrial septal defects can induce left-to-right shunting, which causes volume overload, heart failure, atrial arrhythmia, and/or pulmonary hypertension starting between the third and fourth decades of life. We describe in detail our technique for totally endoscopic, robotic-assisted atrial septal defect repair.

Cardiac cirrhosis: a rare manifestation of an uncorrected primum atrial septal defect.

Atrial septal defects constitute the second most common cardiac congenital abnormality. These defects when undiagnosed during childhood, present in adulthood with symptoms of right heart failure. We describe a case of uncorrected ostium primum atrial septal defect that presented as liver failure in a 43-year-old woman. The patient was found to have liver cirrhosis and a complete work-up of hepatic causes for cirrhosis was negative. Further examination revealed an atrial primum septal defect. Liver cirrhosis in this patient was thought to be secondary to chronic congestion from the right heart

[Transvenous atrial septal defect occlusion in an animal experiment].

Preliminary results concerning the occlusion of experimental atrial-septal defects (ASDII) of the secundum type are reported. An occlusion element was inserted transvenously and its passage across the atrial-septal defect was controlled by open-heart surgery in dogs. Subsequently, the defect was occluded using a single-piece element. In all 5 surgical experiments the occlusion was possible without dislocation or embolism of the element during an atrial stimulation of the heart for more than 1 hour. In additional experiments without open-heart surgery, atrial-septal defects were produced in 7 d

Safety and effectiveness of transcatheter closure of atrial septal defects: Initial results in Fukushima Prefecture.

BACKGROUND: Atrial septal defect is the third most common type of congenital heart disease. Surgical closure was the standard treatment for atrial septal defects before transcatheter closure was approved as minimally invasive treatment in Japan in 2006. In our hospital, this procedure has been performed since 2015. OBJECTIVE: To evaluate the safety and effectiveness of transcatheter closure of atrial septal defects in our hospital. METHODS: Thirty patients (mean age 57.0 ± 19.7 years, 11 males), who underwent transcatheter closure of atrial septal defect between September 2015 and February 2

Percutaneous management of coronary sinus atrial septal defect: two cases representing the spectrum for device closure and a review of the literature.

Coronary sinus atrial septal defects are the rarest defects of the atrial septum comprising <1% of the five different types of atrial septal defects. Despite the widespread adoption of percutaneous device closure of secundum atrial septal defects, the published experience with percutaneous device closure of coronary sinus atrial septal defects is limited to only a few isolated case reports because of uncertainty regarding safety and efficacy. Open-heart surgical repair remains the treatment of choice for coronary sinus atrial septal defects, although this may not be the only treatment option i
