The use of circular staplers for the treatment of haemorrhoids is a new technique that makes for better correction of the physiopathology of the condition, affords greater patient comfort and reduces health-care expenditure. The aim of the therapy of piles is to cure the complaints of the patient by reducing the enlarged haemorrhoidal plexus according to the stage (1 degree to 3 degrees) to a nearly physiological size and in case of a prolapse to replace the sensitive anoderma. The basic therapy consists of regulating the bowel function and avoiding straining. A high fibre diet or bulk laxatives may be necessary. In a prospective study, 512 patients undergoing hemorrhoidal band ligation over a seven-year period were followed up to focus on complications. Thirteen patients (2.5 percent) were hospitalized: six with delayed massive rectal bleeding, three with urinary retention, pain, and fever (one developed perianal abscess), and three others with severe pain due to prolapsed thrombotic hemorrhoids (one developed difficulty in urination). The authors retrospectively studied 43 patients suspected of having internal hemorrhoids at double-contrast barium enema examination. In light of recent reports describing severe and even fatal complications, the authors would like to report good results in two patients with massive edema and one with localized necrosis following rubber band ligation of internal hemorrhoids, which were recognized early and treated aggressively. The potential for severe complications emphasizes the need for determining other causes of rectal symptoms before ligation of hemorrhoids is undertaken.

Postpartum anorectal disease often complicates an otherwise normal and pleasant delivery. The obstetrician must relieve severe symptoms in patients who otherwise could leave the hospital. One hundred nineteen patients with severe hemorrhoidal symptoms or painful anal fissures were treated by anal dilatation. This therapy relieved the symptoms in all patients without extended hospitalization and did not expose nursing mothers to anesthesia or narcotic medication. The long-term effects were excellent; there was only 1 recurrence of hemorrhoidal symptoms after 3 years of follow-up.

Anal melanoma is a rare and aggressive neoplasm of the anal canal seen in the elderly population in the six or seventh decade of their lives. Presentation is usually nonspecific and diagnosis is often delayed or missed initially. The management is surgical and prognosis is poor. Here we present a case of anal melanoma in an elderly patient masquerading as hemorrhoid.

A 46-year-old man with cirrhosis and portal hypertension complained of lower pelvic pain. CT of the rectum raised a strong suspicion of a rectal tumor.
