Adventure sports and sexual freedom hip replacement: the tripolar hip.

Certain athletic activities and lifestyles require a completely stable and very mobile hip. Total hip replacement with a natural femoral head size and two mobile-bearing surfaces (i.e., a "tripolar" prosthesis) is the most stable prosthesis. Elegant design and wear-resistant bearing surfaces are the keys to long-term implant survivorship. The hypothesis is that a ceramic-coated tripolar prosthesis using highly cross-linked polyethylene can provide full function and complete stability with low wear. This study sought to determine: (1) patient-reported outcomes, (2) functional outcomes, (3) impl…

Hip Dislocation or Subluxation After Hip Arthroscopy: A Systematic Review.

PURPOSE: To determine patient- and surgery-specific characteristics of patients sustaining postarthroscopic hip dislocation or subluxation. METHODS: A systematic review of multiple medical databases was registered with PROSPERO and performed using Preferred Reporting Items for Systemic Reviews and Meta-Analysis guidelines. Level I to IV clinical outcome studies reporting the presence of hip dislocation or subluxation after hip arthroscopy were eligible. Length of follow-up was not an exclusion criterion. All patient- and surgery-specific variables were extracted from each, specifically evalua…

Anterior hip capsuloligamentous reconstruction for recurrent instability after hip arthroscopy.

A variety of complications, including iatrogenic anterior hip instability, have been reported after hip arthroscopy. We present a case of a patient sustaining a postoperative anterior hip dislocation after cam resection for treatment of femoroacetabular impingement. Our patient developed persistent instability and required anterior capsuloligamentous reconstruction with iliotibial autograft. We present a comprehensive review of the literature on postoperative hip instability after hip arthroscopy, including the role of capsulotomy closure, and use of postoperative orthotics and weight-bearing …

Trochanteric osteotomy in total hip replacement for congenital hip disease.

We studied the effect of trochanteric osteotomy in 192 total hip replacements in 140 patients with congenital hip disease. There was bony union in 158 hips (82%), fibrous union in 29 (15%) and nonunion in five (3%). The rate of union had a statistically significant relationship with the position of reattachment of the trochanter, which depended greatly on the pre-operative diagnosis. The pre-operative Trendelenburg gait substantially improved in all three disease types (dysplasia, low and high dislocation) and all four categories of reattachment position. A persistent Trendelenburg gait post-o…

Hip arthroscopy after surgical hip dislocation: is predictive imaging possible?

PURPOSE: Our purpose was to study the sensitivity, specificity, and predictive values for hip adhesions, labral tears, and articular cartilage lesions in patients who had open treatment for femoroacetabular impingement, had persistent symptoms, and had both magnetic resonance arthrography (MRA) with radial slices and hip arthroscopy. METHODS: Of 750 patients, 21 patients (6 male and 15 female patients; mean age, 28 years [range, 16 to 41 years]) with persistent groin pain after open osteochondroplasty and femoroacetabular impingement were included. The mean time between open osteochondroplast…

[Surgical treatment of hip osteoarthritis: update in total hip arthroplasty].

Total hip replacement is one of the most successful procedures in orthopaedic surgery. There are two different technologies for implant fixation in total hip replacement: cemented and cementless, both can be combined, which is called Hybrid arthroplasty. Long term implant stability results in long term function. The most important factor that limits longevity of well-fixed implants is the wear of the articular surfaces. Wear of the polyethylene from the acetabulum generates particles that access the implant bone or the implant-cement-bone interface. This produces an inflammatory reaction, oste…

Natural history of congenital hip dislocation (CDH) and hip dysplasia.

For normal development of the hip joint to occur, there must be a delicate, genetically determined balance between growth of the acetabular and triradiate cartilages and a well-centered femoral head. This balance may be profoundly affected by the intrauterine environment. Without treatment, it is uncertain how many dysplastic, unstable hips will retain their dysplastic features throughout life. The natural history of untreated complete dislocation varies considerably and is affected by societal considerations. There may be little, if any, functional disability in many cases. Significant roentg…

[Juvenile hip pain. 2. Femur head epiphysiolysis, hip dysplasia, tumors].

The early symptom in hip joint diseases in children is pain. Pain is localized in the groin and thight, but mostly in the knee. Other important signs are limping and reduced internal rotation. If a hip disease is suspected it is necessary to take X-rays in two planes. If diagnosis is early and special therapy started immediately, the results are usually excellent without deformation of the hip. Otherwise early osteoarthritis can develop. This is important because osteoarthritis in the hip joint is in 75% of the cases due to hip joint diseases in childhood. The problems of diagnosis and treatme…

Metal-on-metal dysplasia cup total hip arthroplasty for hip osteoarthritis secondary to developmental dysplasia of the hip.

OBJECTIVES: This study aims to evaluate the clinical and radiological results of metal-on-metal dysplasia cup total hip arthroplasty (THA) for hip osteoarthritis secondary to developmental dysplasia of the hip (DDH). PATIENTS AND METHODS: Between May 2009 and October 2011, THA was performed on 27 hips (7 Crowe type II, 9 Crowe type III, 11 Crowe type IV) of 22 patients (2 males, 20 females; mean age 43 years; range 25 to 63 years) with hip osteoarthritis secondary to DDH. All patients were evaluated clinically and radiographically. RESULTS: Average follow-up period was 34.2 months (range 24-…

[Total hip prosthesis in coxarthrosis following congenital hip dislocation (65 hips reviewed at 5 years)].

In 45 patients, 65 hips with CDH were given a THR after 5 years or more, with an average follow up of 8.5 years. The results and the technical problems are discussed. Of those 65 hips, there were 46 anterior dislocations, 15 intermediate dislocations and 4 cases of posterior dislocation. The mean shortening was 58.3 mm. In all cases, the authors performed a total arthroplasty on the original acetabulum, after lowering and reconstruction of the acetabulum by grafts taken from the femoral head. There are some immediate postoperative complications: 12 thromboembolic complications, hematomas and 2…

The anterior hip capsule is thinner in dysplastic hips: a study comparing different young adult hip patients.

PURPOSE: To investigate the thickness and intra-substance change of anterior capsule of the hip joint, and compare the difference of the capsular features in patients with different statuses of hip stability. METHODS: A retrospective study was performed to review a hip preservation database. Using the lateral center edge angle(LCEA), patients with borderline dysplasia of the hip (BDH) of 20° ≤ LCEA ≤ 25°, femoracetabular impingement(FAI) with LCEA > 30° and dysplasia of the hip (DH) of LCEA < 20° were enrolled and stratified into different treatment groups. The patients' imaging was reviewed …

Ultrasonography of the hip.

A complete physical examination of the hip is often difficult due to its size and deep position. During the last two decades, ultrasonography (US) of the hip has been widely accepted as a useful diagnostic tool in patients with hip pain and /or limited range of motion. It is commonly used in both adults and children. This technique allows evaluation of different anatomical structures and their pathological changes, such as joint recess (joint effusion, synovial hypertrophy), changes within the bursae (bursitis), tendons and muscles (tendinopathy, ruptures, calcifications), as well as changes i…

Arthrodiastasis of the hip.

BACKGROUND: Hip distraction in Legg-Calvé-Perthes disease unloads the joint, which negates the harmful effect of the stresses on the articular surface, which may promote the sound healing of the areas of necrosis. METHODS: Nonarticulated arthrodiastasis without soft tissue release using an Ilizarov external fixator was applied to 29 patients with Legg-Calvé-Perthes disease (older than 8 y at onset and lateral pillar type C or B). RESULTS: Follow-up period ranged from 2.5 to 11 years with an average of 7.5 years. Twenty-seven cases (93%) had improvement of the range of motion postoperatively.…

Arthroscopy of the hip.

The past decade has brought a number of advances in the use of hip arthroscopy to diagnose and treat intra-articular pathology of the hip joint. As such, hip arthroscopy has been used to treat patients who otherwise may have needed a complicated open procedure or have gone untreated. The history and physical examination of the patient as well as imaging modalities, such as magnetic resonance imaging, play a vital role in determining a successful outcome. Patient selection also is an important component of success. As the technique and instrumentation of hip arthroscopy improves, this procedure…
