Comparison of open reduction and internal fixation with total elbow arthroplasty for intra-articular distal humeral fractures in older age: a retrospective study.

BACKGROUND: Intra-articular distal humeral fractures can be surgically challenging. It remains under discussion whether open reduction and internal fixation (ORIF) or total elbow arthroplasty (TEA) is more beneficial for treatment of the elderly. This study aimed to compare the clinical and functional outcomes of ORIF and TEA for managing intra-articular distal humerus fractures in patients aged 65 years or older. METHODS: Patients who underwent ORIF (n=28) or TEA (n=43) for in intra-articular distal humerus fracture between May 2008 and December 2018 were reviewed. Range of motion, Mayo Elbo

Non-tourniquet strategy with high-position low-perfusion in distal humeral fracture surgery: reduces swelling without increasing blood loss.

INTRODUCTION: To evaluate the feasibility of non-tourniquet strategy combined with "High-position Low-Perfusion" positioning in distal humeral fractures ORIF (DHF-ORIF), and to investigate its effects on perioperative blood loss, postoperative rehabilitation, and functional outcomes. METHODS: Prospective inclusion of patients aged 18-45 with distal humeral fractures, randomly divided into an experimental group without tourniquets (Group A) and a control group with traditional tourniquets (Group B). The Group A adopted a lateral position with high and low perfusion of the affected limb, and re

Distal radius T-shaped locking plate with suture anchors for humeral greater tuberosity fracture with rotator cuff injury: A case report.

Greater tuberosity (GT) fractures with rotator cuff injuries are challenging due to risks of nonunion and functional loss. We report a case treated with a distal radius locking plate combined with suture anchors. A female patient presented with shoulder pain and immobility 4 weeks after trauma, diagnosed with humeral head dislocation and rotator cuff tear. She underwent minimally invasive fixation using this combined technique, which provides stable fracture reduction while repairing the rotator cuff. At the 18-month follow-up, shoulder function was fully restored with no complications. This c

Combined Olecranon Osteotomy and the Posterior Minimal Invasive Plate Osteosynthesis Approach for a Concomitant Injury of the Humeral Shaft and a Distal Intraarticular Humerus Fracture.

A complex fracture involving the distal humerus is a difficult fracture to treat and more so when it is involved with the ipsilateral shaft of the humerus. Open reduction and internal fixation of the humeral shaft with articular reconstruction have been described for a successful outcome of these complex fractures. However, it has drawbacks, especially in terms of soft tissue dissection and subsequent scarring and non-union. A 42-year-old female presented to the emergency department with a fracture of the intercondylar humerus with an ipsilateral shaft of the left humerus. Combined olecranon o

The discriminative ability of peripheral and axial bone measurements to identify proximal femoral, vertebral, distal forearm and proximal humeral fractures: a case control study.

Previous studies evaluating peripheral bone measurement devices have often used discontinued technologies, compared single devices, only evaluated a single fracture syndrome or failed to make a comparison with central densitometry, which is currently the gold standard for fracture discrimination. We have used a case control study to evaluate the ability of different peripheral and central bone techniques to discriminate between fracture cases and controls, determine the impact of different measurement sites, evaluate the role of measuring the cortical or trabecular envelopes using quantitative

A New Surgical Technique and Clinical Outcomes of Operated Proximal Metadiaphyseal Humeral Fractures and Nonunions With the Use of Reversed Anatomic Distal Femoral Locking Plate.

Introduction Proximal humeral fractures complicated with metaphyseal and diaphyseal extension are usually treated operatively with 3.5 mm long anatomic proximal humerus plates. However, frequently these comminuted and segmental fracture types may be associated with delayed union, nonunion, and/or plate failure. We present a technique for addressing this fracture pattern by using an anatomic contralateral 4.5 mm distal femoral plate in a reversed fashion. Methods Eleven patients (eight women and three men) with a mean age of 70 years (range, 52 to 84 years) were operated on with the described t

Total elbow arthroplasty for distal humeral fractures in the elderly population: good functional outcomes and a high implant survival rate can be expected after surgery.

INTRODUCTION: Distal humeral fractures in elderly patients are difficult to treat due to poor bone quality and high complication rates. Total elbow arthroplasty (TEA) is an alternative for non-reconstructable fractures. This study aimed to evaluate long-term outcomes and implant survival of elderly subjects treated with linked TEA for acute distal humeral fractures. METHODS: A retrospective study was conducted on data from patients who underwent TEA between January 2017 and December 2024 with fractures unsuitable for open reduction and internal fixation (ORIF). AO/OTA type C3 distal humeral fractures in patients aged 75 years and older: Is ORIF with double precontoured anatomical locking plates a reliable treatment?

INTRODUCTION: The incidence of osteoporotic distal humeral fractures (DHF) is on the rise. Their operative management is demanding. Fixation with non-locking reconstruction plates was associate with a high number of complications. Elbow arthroplasty (total or hemi) has been proposed as an alternative treatment, in spite of lifetime activity restrictions, and risk of complications, unknown implant survival and problematic revision surgery. Precontoured anatomical locking plates have increased the strength of the fixation in complex fractures. HYPOTHESIS: double plating ORIF with precontoured a

Outcome of total elbow replacement for distal humeral fractures in the elderly: a comparison of primary surgery and surgery after failed internal fixation or conservative treatment.

We analysed the outcome of the Coonrad-Morrey total elbow replacement used for fracture of the distal humerus in elderly patients with no evidence of inflammatory arthritis and compared the results for early versus delayed treatment. We studied a total of 32 patients with 15 in the early treatment group and 17 in the delayed treatment group. The mean follow-up was 56.1 months (18 to 88). The percentage of excellent to good results based on the Mayo elbow performance score was not significantly different, 84% in the early group and 79% in the delayed group. Subjective satisfaction was 92% in bo

Intramedullary nailing using K-wires for high-energy distal humeral metaphyseal-diaphyseal fractures accompanying radial nerve palsy in a 2-year-old toddler: A case report.

RATIONALE: Pediatric distal humeral diaphyseal fractures are rare and challenging to manage due to anatomical and biomechanical complexities. In addition, humeral shaft fractures in adults are often accompanied by radial nerve paralysis (RNP), but there are no studies on the incidence or treatment of pediatric humeral shaft fractures accompanied by RNP in toddlers. The authors present the outcomes of closed reduction and intramedullary nailing using Kirschner wires (K-wires) for high-energy distal humeral metaphyseal-diaphyseal open fractures accompanied by RNP in a toddler. Clinical comparison of double-plate fixation by the perpendicular plate method versus parallel plate method for distal humeral fracture: a multicenter (TRON group) study.

INTRODUCTION: Double-plating methods are popular, with perpendicular and parallel plate methods being widely used surgical method for the rigid fixation of distal humeral fracture (DHF). However, which plate method is better for DHF remains controversial. The aim of this study was to compare patient outcomes including the incidences of complications and reoperation between the two plate methods. METHODS: We extracted 383 patients with DHF undergoing surgery between 2011 and 2020 from our multicenter database, which is named TRON. We divided the subjects into two groups: perpendicular plating

Hemiarthroplasty or total elbow arthroplasty for unreconstructable distal humeral fractures in patients aged over 65 years : a systematic review and meta-analysis of patient outcomes and complications.

AIMS: Arthroplasty is being increasingly used for the management of distal humeral fractures (DHFs) in elderly patients. Arthroplasty options include total elbow arthroplasty (TEA) and hemiarthroplasty (HA); both have unique complications and there is not yet a consensus on which implant is superior. This systematic review asked: in patients aged over 65 years with unreconstructable DHFs, what differences are there in outcomes, as measured by patient-reported outcome measures (PROMs), range of motion (ROM), and complications, between distal humeral HA and TEA? METHODS: A systematic review of
