Mechanisms of Tolerance Induction in Liver Transplantation: Lessons Learned from Fetomaternal Tolerance, Autoimmunity and Tumor Immunity.

Since the first published report of experimental kidney transplantation in dogs in 1902, there were many experimental and clinical trials of organ transplantation, with many sacrifices. After the establishment of the surgical technique and the discovery of immunosuppressive drugs, transplantation became the definitive treatment strategy for patients with terminal organ failure. However, this is not a common therapy method due to the difficulty of solving the fundamental issues behind organ transplantation, including the shortage of donor graft, potential risks of transplant surgery and economi

Ablation of hemophilic FVIII inhibitors with FVIII priming, cyclophosphamide immune suppression, and rapid tapering of FVIII immune tolerance.

The efficacy and toxicity of factor VIII (FVIII) priming, cyclophosphamide immune suppression, and rapid tapering of concurrent FVIII immune tolerance for subjects with hemophilic inhibitors were evaluated. Four subjects with hemophilic inhibitors were studied. Before treatment, inhibitors were present for a median of 8 months (mean 13 +/- 14.0 months). The median FVIII inhibitor titer was 16 BU/mL (mean 27.2 +/- 29.2 BU/mL). Following FVIII priming (80.0 +/- 70.2 U/kg), subjects received cyclophosphamide 1,418 +/- 636 mg/M2 i.v. q3 weeks for 4.4 +/- 1.7 courses. Subjects concurrently received…

Immune status of mice tolerant of living cells. III. Presence and evolution of cells cytotoxic to the tolerated strain.

Spleen cells from CBA mice neonatally rendered highly tolerant to A/Jax (keeping a skin graft in perfect shape for more than 1 year and without detectable hemagglutinating or cytotoxic antibodies) contain cells cytotoxic for YAC 222 (A/Jax) in Cr release assay. The degree of the cytotoxicity depends on the age of the mouse, following a curve lower than, but parallel to, the one followed by the cytotoxicity of cells from CBA rendered immune by injecting them with A/Jax cells 1 week previously. The maximum of the cytotoxicity curve is reached during the 9th and 10th weeks. Normal CBA cells thems

Determinants of Immune Tolerance Induction Outcomes in 237 Persons With Severe Hemophilia A.

Immune tolerance induction (ITI) to eradicate an immune response against factor VIII (FVIII) has been used in hemophilia A (HA) with various success rates, costs, and treatment burden. The present study aimed to identify markers predictive of ITI success. Data were accrued from 237 patients with severe HA (FVIII level < 1%), history of persistent FVIII inhibitors ≥ 1 BU/mL and having performed ITI. Constitutional and clinical variables associated with ITI outcome were evaluated and multivariable logistic regression models developed. The F8 variant, age at inhibitor detection, and historical pe…

Interleukin-33 at the maternal-fetal interface: From immune tolerance to obstetric syndromes.

Successful viviparous reproduction depends on a finely tuned state of maternal-fetal immune tolerance. Among the mediators that shape this balance, the alarmin cytokine interleukin-33 (IL-33) has drawn growing attention as a key regulatory checkpoint. Here, we bring together recent findings to interpret IL-33 function through a "Two-Wave" framework that helps reconcile its seemingly opposing roles in pregnancy. In early gestation, a controlled, spatially confined release of IL-33 appears essential for processes such as decidualization, spiral artery remodeling, and the establishment of a Type …

Functional xenogeneic hematopoietic cells maintaining donor-dominant identity and immune tolerance enable therapy.

BACKGROUND: The clinical supply of hematopoietic cells is severely constrained by the limitations of donor donation and inefficient in vitro generation. While generating these cells within interspecies chimeras presents a groundbreaking alternative, the fundamental biological properties and therapeutic potential of the resulting xenogeneic hematopoietic cells remain poorly characterized. METHODS: We generated xenogeneic hematopoietic cells in rodent models via blastocyst complementation and bone marrow transplantation. A comprehensive comparative analysis of these cells was conducted using si

IL-15 in alopecia areata: the immune tolerance mechanistic insights and translational perspectives.

Alopecia areata (AA) is a highly prevalent human autoimmune disease. It is now regarded by most scientists as a defect of immune privilege ( … porary century, there has been a gradually growing recognition of their critical role in modulating immune tolerance (via Treg cell). Low-dose IL-2 preferentially induces Treg cell, recovering the balance status between Treg cell and effector T cell and representing an original strategy for autoimmune systematic diseases. Recent study found IL-15 was the guardian …

Therapeutic immune tolerance for central nervous system autoimmune diseases - Prospects, challenges and pitfalls.

Autoimmune diseases of the central nervous system (CNS), including multiple sclerosis (MS), neuromyelitis optica spectrum disorders (NMOSD), and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD), are characterized by chronic inflammation driven by autoreactive immune responses against CNS antigens. Successes in the development of efficacious immunotherapies are hampered by the risks of broad immunosuppression and the consequences of compromising physiological immune functions. Antigen-specific immune tolerance offers a promising alternative by selectively silencing pathog…

PTPN6/SHP-1 in autoimmune disease: immune tolerance, regulatory mechanisms, and therapeutic targeting.

PTPN6 encodes SHP-1, a central regulator of immune tolerance and a critical signaling brake across innate and adaptive immunity in autoimmune disease. Predominantly expressed in hematopoietic cells, SHP-1 restrains receptor-proximal signaling downstream of antigen, cytokine, and pattern-recognition receptors, thereby setting activation thresholds that preserve immune homeostasis and limit autoreactivity. Converging evidence from human genetics, patient-derived immune cells, and animal models indicates that diminished SHP-1 activity promotes persistent autoreactive lymphocyte responses, exagger…

Regulatory B cells, the key regulator to induce immune tolerance in organ transplantation.

In solid organ transplantation, especially renal transplantation, for the induction of immune tolerance, accumulating evidence has revealed that Regulatory B cells (Breg) play a crucial role in stimulating immune tolerance, alleviating immune responses, and improving graft survival. We describe the heterogeneous nature of Bregs, focusing on their defining surface markers and regulatory functions. Meanwhile, the major cytokine secretion function and the correlation between Breg and Treg or other immune checkpoints to balance the immune responses are addressed. Furthermore, we summarized the int…

Exploring the Interplay Between Depression, Anxiety, and Immune Tolerance in Liver Transplant Recipients.

OBJECTIVE: Liver transplantation requires lifelong immunosuppression, which has significant psychological and physiological implications. While depression and anxiety are prevalent among transplant recipients, their potential relationship with immune tolerance remains unclear. This study aims to evaluate the association between depression, anxiety, and immune tolerance in liver transplant (LT) recipients. METHODS: This cross-sectional study included 179 adult LT recipients, categorized as tolerant ( n =49) and nontolerant ( n =130) based on immunosuppressive status. Depression and anxiety lev…

ACC1 is a dual metabolic-epigenetic regulator of Treg stability and immune tolerance.

OBJECTIVE: Regulatory T cells (Tregs) are essential in maintaining immune tolerance and controlling inflammation. Treg stability relies on transcriptional and post-translational mechanisms, including histone acetylation at the Foxp3 locus and FoxP3 protein acetylation. Additionally, Tregs depend on specific metabolic programs for differentiation, yet the underlying molecular mechanisms remain elusive. We aimed to investigate the role of acetyl-CoA carboxylase 1 (ACC1) in the differentiation, stability, and function of regulatory T cells (Tregs). METHODS: We used either T cell-specific ACC1 kn…

Role of maternal-fetal immune tolerance in the establishment and maintenance of pregnancy.

Normal pregnancy is a contradictory and complicated physiological process. Although the fetus carries the human leukocyte antigen (HLA) inherited from the paternal line, it does not cause maternal immune rejection. As the only exception to immunological principles, maternal-fetal immune tolerance has been a reproductive immunology focus. In early pregnancy, fetal extravillous trophoblast cells (EVTs) invade decidual tissues and come into direct contact with maternal decidual immune cells (DICs) and decidual stromal cells (DSCs) to establish a sophisticated maternal-fetal crosstalk. This study …

TCA metabolism regulates DNA hypermethylation in LPS and Mycobacterium tuberculosis-induced immune tolerance.

Severe and chronic infections, including pneumonia, sepsis, and tuberculosis (TB), induce long-lasting epigenetic changes that are associated with an increase in all-cause postinfectious morbidity and mortality. Oncology studies identified metabolic drivers of the epigenetic landscape, with the tricarboxylic acid (TCA) cycle acting as a central hub. It is unknown if the TCA cycle also regulates epigenetics, specifically DNA methylation, after infection-induced immune tolerance. The following studies demonstrate that lipopolysaccharide and Mycobacterium tuberculosis induce changes in DNA methyl…

Double Negative T Regulatory Cells: An Emerging Paradigm Shift in Reproductive Immune Tolerance?

Immune regulation of female reproductive function plays a crucial role in fertility, as alterations in the relationship between immune and reproductive processes result in autoimmune subfertility or infertility. The breakdown of immune tolerance leads to ovulation dysfunction, implantation failure, and pregnancy loss. In this regard, immune cells with regulatory activities are essential to restore self-tolerance. Apart from regulatory T cells, double negative T regulatory cells (DNTregs) characterized by TCRαβ+/γδ+CD3+CD4-CD8- (and negative for natural killer cell markers) are emerging as effe…

Redirecting reproductive immunology research toward pregnancy as a period of temporary immune tolerance.

Referring to two recent publications, we here propose that clinical reproductive immunology has for decades stagnated because reproductive medicine, including assisted reproduction (AR), has failed to accept embryo implantation as an immune system-driven process, dependent on establishment of maternal tolerance toward the implanting fetal semi-allograft (and complete allograft in cases of oocyte donation). Pregnancy represents a biologically unique period of temporary (to the period of gestation restricted) tolerance, otherwise only known in association with parasitic infections. Rather than i
