Current meningitis outbreak in Ghana: Historical perspectives and the importance of diagnostics.

Bacterial meningitis continues to be one of the most dreaded infections in sub-Saharan Africa and other countries that fall in the "meningitis belt" due to recurrent nature of the infection and the sequel of deliberating effects among survivors even after treatment. Ghana has had recurrent epidemics in the past but has been free from high mortality levels. Whereas reasons for the low reported number of deaths in the past are unclear, we hypothesize that it may be due to increased vaccination from expanded program on immunization (EPI) and consequent herd immunity of the general population. As

Why Asymptomatic Plasmodium falciparum Infections Are Common in Low-Transmission Settings.

Plasmodium falciparum infections in low-transmission settings are often asymptomatic with low parasite densities despite low herd immunity. Based on studies in Zanzibar, this may be due to parasitic (nonvirulence) rather than host (immunity) factors. In high-transmission settings, high replication rate and virulence represents a competitive advantage, whereas in low-transmission settings nonvirulent parasites escape both competition and treatment. Such parasites also survive longer in low-transmission settings due to lower host immunity response and less frequent indirect drug exposure. This h

A seroprevalence study of poliovirus antibody in the population of northern Greece.

This study assessed immunity to poliomyelitis in a representative sample of 1064 persons living in northern Greece. Antibody prevalences in the individuals tested were 91.1% (95% confidence interval (CI): 89.4-92.8), 92.1% (95% CI: 90.5-93.7) and 83.1% (95% CI: 80.8-85.4) for poliovirus types 1, 2 and 3, respectively. For poliovirus type 3, a gap in immunity was found in individuals aged 10-29 years. Re-vaccination of adolescents living in northern Greece is suggested to ensure herd immunity and to minimise the risk of importation of wild poliovirus from endemic countries.

Evaluation of the national immunisation programme in the Netherlands: immunity to diphtheria, tetanus, poliomyelitis, measles, mumps, rubella and Haemophilus influenzae type b.

The immunity to vaccine-preventable diseases included in the Dutch immunisation programme in the general population and among orthodox reformed individuals who refuse vaccination was assessed. The programme induces good protection. However, a large proportion of adults lacks diphtheria and tetanus immunity. Measles, mumps and rubella seroprevalence was somewhat lower among vaccinated compared to unvaccinated cohorts. The prevalence of HibPS antibodies declined during 2.5 years after the fourth vaccination. However, protection occurs also by memory immunity. Herd immunity is sufficient among th

Mathematical model of voluntary vaccination against schistosomiasis.

Human schistosomiasis is a chronic and debilitating neglected tropical disease caused by parasitic worms of the genus Schistosoma. It is endemic in many countries in sub-Saharan Africa. Although there is currently no vaccine available, vaccines are in development. In this paper, we extend a simple compartmental model of schistosomiasis transmission by incorporating the vaccination option. Unlike previous models of schistosomiasis transmission that focus on control and treatment at the population level, our model focuses on incorporating human behavior and voluntary individual vaccination. We i

Prevention of respiratory virus transmission by resident memory CD8+ T cells.

An ideal vaccine both attenuates virus growth and disease in infected individuals and reduces the spread of infections in the population, thereby generating herd immunity. Although this strategy has proved successful by generating humoral immunity to measles, yellow fever and polio, many respiratory viruses evolve to evade pre-existing antibodies1. One approach for improving the breadth of antiviral immunity against escape variants is through the generation of memory T cells in the respiratory tract, which are positioned to respond rapidly to respiratory virus infections2-6. However, it is unk

Childhood vaccinations: Hidden impact of COVID-19 on children in Singapore.

Although the direct health impact of Coronavirus disease (COVID-19) pandemic on child health is low, there are indirect impacts across many aspects. We compare childhood vaccine uptake in three types of healthcare facilities in Singapore - public primary care clinics, a hospital paediatric unit, and private paediatrician clinics - from January to April 2020, to baseline, and calculate the impact on herd immunity for measles. We find a 25.6% to 73.6% drop in Measles-Mumps-Rubella (MMR) uptake rates, 0.4 - 10.3% drop for Diphtheria-Tetanus-Pertussis-inactivated Polio-Haemophilus influenza (5-in-

The Importance of Prioritizing Pre and Posttransplant Immunizations in an Era of Vaccine Refusal and Epidemic Outbreaks.

Vaccine-preventable infections are occurring at epidemic rates both nationally and internationally. At the same time, rates of vaccine hesitancy and refusal are increasing across the country leading to decreased herd immunity. For immunosuppressed transplant recipients, this situation poses great risk. Currently, 1 in 6 pediatric solid organ transplant recipients is hospitalized with a vaccine-preventable infection in the first 5 years posttransplant. For many recipients, these infections result in significant morbidity, mortality, and increased hospitalization costs. Surprisingly, despite thi

Vaccine Development for Varicella-Zoster Virus.

Varicella-zoster virus (VZV) is the first and only human herpesvirus for which a licensed live attenuated vaccine, vOka, has been developed. vOka has highly safe and effective profiles; however, worldwide herd immunity against VZV has not yet been established and it is far from eradication. Despite the successful reduction in the burden of VZV-related illness by the introduction of the vaccine, some concerns about vOka critically prevent worldwide acceptance and establishment of herd immunity, and difficulties in addressing these criticisms often relate to its ill-defined mechanism of attenuat

The Influence of Influenza and Pneumococcal Vaccines on Community-Acquired Pneumonia (CAP) Outcomes Among Elderly Patients.

Elderly are at high risk for hospitalization for community-acquired pneumonia (CAP), especially due to Streptococcus pneumoniae, and seasonal influenza viruses. Data suggest PPV23's influence on various CAP-related outcomes among the elderly may depend upon how many years have elapsed since they received this vaccine. PPV23's protection against invasive pneumococcal disease and CAP hospitalizations are often limited to moderately ill elderly, who are less than 75 years old, or female. PCV13 demonstrates broad protection against a variety of CAPs, but ultimately, its influence on their outcomes

Meningococcal conjugate vaccine in adolescents and children.

Disease caused by Neisseria meningitidis is associated with high mortality rates and significant sequelae. Quadrivalent (A, C, Y, W-135) polysaccharide meningococcal vaccine has been available for more than 20 years, and although used widely for adolescents entering college, it has a number of limitations, including short duration of immunity and lack of a herd effect. A quadrivalent meningococcal conjugate vaccine has recently been licensed and endorsed for use in adolescents 11 to 12 years of age, at age 15, or at college entry. Among the key features of the new meningococcal conjugate vacci

Pertussis immunization of adolescents in the United States: an economic evaluation.

The incidence of reported pertussis has increased during the past decade and poses a growing health and economic burden in developed countries, despite high rates of primary vaccination. Administration of a booster dose of acellular pertussis vaccine to adolescents may help reduce this burden, not only by reducing infections in vaccinated individuals but also by reducing transmission of Bordetella pertussis to other individuals, particularly infants. An epidemiologic model was created to assess the health and economic impact of implementing a program of routine acellular pertussis immunization

Behavior change and infection induced immunity led to the decline of the 2022 Mpox outbreak in Berlin.

BACKGROUND: Mpox denotes a viral zoonosis caused by the Orthopoxvirus monkeypox (MPXV), which is endemic in West and Central Africa. In spring 2022, notable outbreaks of MPXV clade IIb were recorded in several high-income countries, predominantly affecting men who have sex with men (MSM). At the peak of the outbreak, over 200 new mpox cases per week were reported in Berlin, which constitutes one of the largest MSM population in Europe. Within the same year, the outbreak significantly declined, and it is unclear which factors contributed to this rapid decrease. METHODS: To investigate the conc

Estimating the number of US children susceptible to measles resulting from COVID-19-related vaccination coverage declines.

Measles elimination hinges on vaccination coverage remaining above 95% to retain sufficient community protection. Recent declines in routine measles vaccinations due to the COVID-19 pandemic coupled with prior models indicating the country was close to the 92% herd immunity benchmark are a cause for concern. We evaluated population-level measles susceptibility in the US, including sensitivity analyses accounting for pandemic-related impacts on immunization. We estimated the number of children aged 0-18 currently susceptible to measles and modeled susceptibility proportions in decreased vaccina

Population Immunity and Polio Eradication.

The Global Polio Eradication Initiative made immense progress after its establishment in 1988 as a consequence of high coverage with various poliovirus vaccines in all populations of the world. Problems have arisen in recent years, however, related to security issues in some countries, to the circulation of vaccine-derived polioviruses, and to the recognition that individuals with certain immune deficiencies can remain infected and infectious for many months or years. As natural infection and different vaccines have different effects on the immune system, the patterns of humoral and mucosal im

What is the best way to use conjugate vaccines?

PURPOSE OF REVIEW: The present review discusses recent advances, challenges and opportunities for the best use of conjugate vaccines now and in the future. RECENT FINDINGS: Direct protection in young children may be short-lived and programme effectiveness may depend heavily on indirect protection (herd immunity). Pneumococcal carriage serotype replacement has been widely reported following vaccine implementation. Use of pneumococcal conjugate vaccines is being trialled in the elderly. Vaccination in west Africa against Neisseria meningitidis serogroup A (Men A), a new monovalent conjugate vac
