CT-derived muscle quality predicts recurrence following ventral incisional hernia repair.

PURPOSE: Incisional hernia repair is challenging because of its high recurrence rate. However, the impact of sarcopenia on the outcomes of incisional hernia remains controversial. This study aimed to investigate the clinical effects of computed tomography (CT)-derived muscle quality in patients who underwent incisional hernia repair. METHODS: Clinical data and images of patients who underwent ventral incisional hernia repair between 2016 and 2018 at a university-based tertiary referral center were retrospectively collected. Muscle quality was assessed via CT using the hounsfield unit average …

Unsupervised Clinical Phenotyping Identifies Distinct Risk Profiles in Incisional Hernia Repair.

Background and Objectives: Patients undergoing incisional hernia repair constitute a clinically heterogeneous population with variable postoperative outcomes. Conventional risk models based on isolated risk factors may inadequately capture this complexity. This study aimed to identify data-driven clinical phenotypes and evaluate their association with surgical outcomes. Methods and Materials: A retrospective cohort of 1262 patients undergoing retromuscular incisional hernia repair (Rives-Stoppa technique) was analyzed. Unsupervised clinical phenotyping was performed using latent class analysis…

Early outcomes of contaminated midline incisional hernia repair with mesh suture.

PURPOSE: Mesh suture is a novel reinforcement construct designed to provide enhanced mechanical support during midline fascial closure in comparison to conventional sutures while minimizing tissue dissection and foreign body burden in comparison to use of a planar mesh. Its use in contaminated fields remains understudied. This study evaluates the early clinical outcomes following mesh suture closure in clean-contaminated and contaminated incisional hernia repairs. METHODS: A retrospective review was conducted of patients undergoing incisional hernia repair with mesh suture closure between Jan…

Peritoneal flap repair for large (>8 cm width) recurrent incisional ventral hernia.

The recurrence of large (>8 cm width) incisional hernias remains a challenging problem in surgical practice. We present a case report of a large recurrent incisional hernia following multiple prior procedures. The patient underwent a tension-free hernia repair using the hernia sac as a peritoneal flap. The surgical approach involved careful dissection and mobilisation of the hernia sac, which was then used to create peritoneal flaps on the anterior and posterior aspects of the mesh. This technique obviated the need for traditional component separation procedures, thereby preserving the integri…

Impact of methocarbamol on opioid use after ventral incisional hernia repair.

BACKGROUND: Alternatives to opioid analgesia are needed to reduce the risk of abuse, misuse, and diversion. Musculoskeletal pain is a significant contributor to postoperative pain after ventral hernia repair (VHR). We report the impact of methocarbamol on opioid prescribing after VHR. METHODS: Review of all robotic and open VHR, Jan 2020-July 2022. Data was collected in the Abdominal Core Health Quality Collaborative (ACHQC) with additional chart review to assess for opioid refills. A 2:1 propensity score match was performed comparing opioid prescribing in patients prescribed vs not prescribe

INCISIONAL HERNIA AFTER BARIATRIC SURGERY: ONLY THE PHYSICAL EXAMINATION IS ENOUGH?

BACKGROUND: Incisional hernia is characterized by a bulging of the abdominal wall caused by the prolapse of intracavitary structures, such as a segment of the small intestine, through the trocar orifice. Ultrasonography and physical examination are used in the diagnosis of incisional hernia. AIMS: This study aimed to evaluate the difference between physical examination and abdominal ultrasonography at the diagnosis of incisional hernia in patients who underwent laparoscopic bariatric surgery. METHODS: A total of 123 patients who underwent Roux-en-Y gastric bypass type bariatric surgery perfo…

Diastasis recti is associated with incisional hernia after midline abdominal surgery.

PURPOSE: Incisional hernia occurs in up to 20% of patients after abdominal surgery and is most common after vertical midline incisions. Diastasis recti may contribute to incisional hernia but has not been explored as a risk factor or included in hernia risk models. We examined the association between diastasis recti and incisional hernia after midline incisions. METHODS: In this single-center study, all patients undergoing elective gastrointestinal surgery with a midline open incision or extraction site in a prospective surgical quality collaborative database between 2016 and 2020 were includ…

Intestinal perforation after surgical treatment for incisional hernia: iatrogenic or idiopathic?

Intestinal perforation (IP) is a life-threatening gastroenterological condition requiring urgent surgical care, which may present itself as an uncommon complication following incisional hernia repair surgery, most often because of iatrogenic traumatism occurring during the procedure. However, we report a case where a spontaneous onset can be hypothesised. A 60-years-old patient underwent repair of an abdominal laparocele, through rectus abdominis muscle plasty, 5 years after development of an incisional hernia due to exploratory laparotomy for the treatment of acute appendicitis. Xipho-pubic s…

Epigastrium: unusual site of incisional hernia from a 5 mm trocar.

UNLABELLED: The observation of an unusual case of incisional hernia, found in the epigastric zone at the site of a 5 mm trocar incision for a cholecystectomy, has caused us to describe it and to review the literature. C.A. is a male aged 59 and He came to our attention in 2014, complaining about the presence, for about three months, a swelling in the epigastric area, without occlusive symptoms. An objective examination showed an epigastric mass at the scar of the insertion site of a 5mm trocar during the cholecystectomy operation. The patient was hospitalized and underwent traditional surgery:…

Flap surgical techniques for incisional hernia recurrences. A swine experimental model.

BACKGROUND AND AIM: In the age of synthetic prostheses most of hernia studies include a careful examination of the various types of prosthesis, their characteristics and their repair indications. Biological prostheses are also beginning to draw attention. But in terms of recurrence especially for poor or developing countries, the discussion is different, due to their high cost which makes them difficult to afford. In this article we present new flap reconstruction techniques for the reconstruction of the abdominal wall versus mesh repair, applied on swine models, outline the results of each te

Abdominal wall reconstruction for large incisional hernia restores expiratory lung function.

BACKGROUND: Respiratory complications secondary to intermittent intra-abdominal hypertension and/or atelectasis are common after abdominal wall reconstruction for large incisional hernias. It is unknown if the respiratory function of this patient group is affected long term or impairs activities of daily living. We hypothesized that abdominal wall reconstruction for large incisional hernia would not lead to improved, long-term pulmonary function or respiratory quality of life. METHODS: Eighteen patients undergoing open abdominal wall reconstruction with mesh for a large incisional hernia (hor…

Risk of incisional hernia after minimally invasive and open radical prostatectomy.

PURPOSE: The number of radical prostatectomies has increased. Many urologists have shifted from the open surgical approach to minimally invasive techniques. It is not clear whether the risk of post-prostatectomy incisional hernia varies by surgical approach. MATERIALS AND METHODS: In the linked Surveillance, Epidemiology and End Results (SEER)-Medicare data set we identified men 66 years old or older who were treated with minimally invasive or open radical prostatectomy for prostate cancer diagnosed from 2003 to 2007. The main study outcome was incisional hernia repair, as identified in Medic…

Laparoscopic versus open surgical techniques for ventral or incisional hernia repair.

BACKGROUND: There are many different techniques currently in use for ventral and incisional hernia repair. Laparoscopic techniques have become more common in recent years, although the evidence is sparse. OBJECTIVES: We compared laparoscopic with open repair in patients with (primary) ventral or incisional hernia. SEARCH STRATEGY: We searched the following electronic databases: MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, metaRegister of Controlled Trials. The last searches were conducted in July 2010. In addition, congress abstracts were searched by hand. SELECTION CRIT…

Accessory spleens, mixed hiatal hernia, and incisional hernias: a complex case of multidisciplinary surgical management.

BACKGROUND: Accessory spleens, arising from incomplete fusion during embryogenesis, are frequent developmental anomalies detected incidentally in abdominal imaging studies. Despite surgical advancements, post-laparotomy incisional hernias persist, while hiatal hernias, common in older adults, often present asymptomatically. CASE PRESENTATION: A 55-year-old male presented with abdominal pain, chronic vomiting, and gastrointestinal bleeding. Evaluation unveiled a mixed hiatal hernia alongside multiple accessory spleens, necessitating surgical intervention. A midline incision facilitated adhesio…

Neglected Incisional Hernia Becomes a Life-Threatening Emergency: A Case Report.

Although complications are common following abdominal surgeries, the nontraumatic spontaneous rupture and obstruction with skin gangrene of an incisional hernia leading to bowel evisceration is unusual. This report describes the case of a 40-year-old female with a long-standing incisional hernia that presented as a spontaneous evisceration of an obstructed bowel with evidence of skin rupture and gangrene. She was managed with an emergency laparotomy with myofascial anterior component separation and onlay mesh repair.

Early preoperative progressive pneumoperitoneum for a symptomatic giant abdominal incisional hernia.

INTRODUCTION AND IMPORTANCE: Early preoperative progressive pneumoperitoneum (PPP) is a technique that helps large eventrations with loss of domain to reintroduce protruded organs. However, a standardized technique has not been developed. This technique has been proved in elective patients, but the evidence is scarce in patients with a high risk of incarceration/strangulation. CLINICAL FINDINGS AND INVESTIGATIONS: We present a 61-year-old patient with history of a thoracoabdominal aneurysm repair, developed a massive incisional hernia with loss of domain. At admission, he presented with abdom…

Incarcerated giant uterine leiomyoma within an incisional hernia: a case report.

Uterine leiomyomas presenting as incarcerated or strangulated hernias in surgical emergencies are extremely rare and should be considered in the differential diagnosis in patients with known uterine fibroids and an irreducible ventral abdominal wall hernia. Detailed history and multidisciplinary approach optimize the diagnosis and decision making toward surgical treatment.
