All that glitters is not gold: A spinal epidural empyema following epidural steroid injection.

BACKGROUND: Therapeutic epidural spinal injections (ESIs) of steroids are one of the most common nonsurgical management modalities employed for alleviating pain due to chronic persistent lumbar spinal disease. However, it is well documented that they have significant risks and complications without any long-term efficacy. ESI may result in epidural empyema which may be difficult to diagnose with delays resulting in significant permanent neurological sequelae. CASE DESCRIPTION: A 45-year-old female presented with a lumbar spinal epidural empyema after receiving ESI for low back and right leg p

Acute epidural haematoma following epidural steroid injection in a patient with spinal stenosis.

Extradural haematoma is a rare but known complication of neuraxial procedures. Although the literature quotes an incidence of 1:150,000 and 1:220,000 after epidural and spinal procedures, respectively, there is only a limited number of case reports described in pain practice. This case report describes an unusual occurrence of thoracic extradural haematoma following a lumbar epidural steroid injection. An entirely unique feature was its occurrence at a level distant to the site of injection, which has not been reported previously unless associated with catheterisation. We would like to highlig

Lumbar Epidural Hematoma Following Interlaminar Fluoroscopically Guided Epidural Steroid Injection.

OBJECTIVE: Spinal epidural steroid injections are generally considered a safe, effective treatment for radicular pain in a variety of spinal conditions. Complications secondary to these injections, although rare, can result in devastating neurologic symptoms. Patients with preexisting moderate-to-severe spinal stenosis and recent use of chronic anticoagulation pose a challenging dilemma when making treatment decisions, as a history of both can increase the risk of complications. CASE REPORT: The following case study provides a description of a patient with spinal stenosis who discontinued ant

Symptomatic spinal epidural lipomatosis after a single local epidural steroid injection.

Spinal epidural lipomatosis is a rare disorder that can manifest with progressive neurological deficits. It is characterized by abnormal accumulation of unencapsulated epidural fat commonly associated with the administration of exogenous steroids associated with a variety of systemic diseases, endocrinopathies, and Cushing syndrome (Fogel et al. Spine J 5:202-211, 2005). Occasionally, spinal epidural lipomatosis may occur in patients not exposed to steroids or in patients with endocrinopathies, primarily in obese individuals (Fogel et al. Spine J 5:202-211, 2005). However, spinal lumbar epidur

Epidural steroid injections: update on efficacy, safety, and newer medications for injection.

The best evidence for epidural injection appears to be in the setting of radicular pain with epidural steroid and non-steroid injections more efficacious than non-epidural injections. Studies showed the efficacy of non-particulate steroid to approach the efficacy of particulate steroid and very limited comparisons demonstrated no significant difference between epidural steroid and epidural non-steroid (local anesthetic) injection. Preliminary studies evaluating epidural injection of disease modifying anti-rheumatic drugs such etanercept and tocilizumab showed conflicting results and had signif

Inadvertent Subdural Injection during Cervical Transforaminal Epidural Steroid Injection.

Serious complications following cervical epidural steroid injection are rare. Subdural injection of local anesthetic and steroid represents a rare but potentially life threatening complication. A patient presented with left sided cervical pain radiating into the left upper extremity with motor deficit. MRI showed absent lordosis with a broad left paramedian disc-osteophyte complex impinging the spinal cord at C5-6. During C5-6 transforaminal epidural steroid injection contrast in AP fluoroscopic view demonstrated a subdural contrast pattern. The needle was withdrawn slightly and repositioned.

Dural puncture and subdural injection: a complication of lumbar transforaminal epidural injections.

CASE REPORT: Two cases are presented in which the complication of dural puncture is documented in the context of a lumbar transforaminal epidural steroid injection. The hazard of dural puncture during transforaminal epidural injections, the anatomy of the dural and thecal sac, the potential for subdural injections, and relevant literature are reviewed. DESIGN: Report of two cases. BACKGROUND: Lumbar transforaminal epidural steroid injections are a commonly employed procedure for the treatment of lumbar radiculopathy. The optimal target point lies at the "6 o' clock" position of the pedicle.

Physician Perceptions of Particulate Versus Non-Particulate Steroids in Epidural Steroid Injections.

BACKGROUND: Epidural steroid injections (ESIs) are widely used for the treatment of chronic spinal pain, particularly in cervical and lumbar radiculopathy. Corticosteroids used in ESIs are broadly classified as particulate or non-particulate formulations, each with distinct safety and pharmacologic profiles. Comparative efficacy remains controversial. However, safety concerns regarding particulate steroid use in transforaminal injections are well established. However, physician perception of steroid effectiveness may influence clinical decision-making and practice patterns. OBJECTIVE: To eval

Practice Patterns and Perspectives on Epidural Steroid Injections by Interventional Pain Physicians.

INTRODUCTION: Epidural steroid injections (ESIs) are commonly used to manage chronic spinal pain. However, variations in ESI practices remain prevalent among interventional pain physicians. This study evaluates current practice patterns and perceptions of ESI efficacy to identify areas for potential standardization in clinical application. METHODS: A structured survey was distributed to interventional pain physicians via email and social media outlets, collecting data on several aspects of ESI practice: (1) the importance of precise injectate placement, (2) perceived effectiveness for axial v

Medullary Cord Injury After Interlaminar Cervical Epidural Steroid Injections: Two Case Reports.

BACKGROUND: While the complications of cervical epidural steroid injections have been closely studied, there have been no reports of medullary cord injury, including myelopathy, myelomalacia, or syringomyelia, after these injections. We report here a case of myelopathy progressing to myelomalacia and a case of syringomyelia after properly performed cervical intralaminar epidural steroid injections. CASE REPORT: One case was characterized by a long history of advanced cervical spondylosis, the other by a distant history of cervical trauma. In neither case was there any evidence to suggest that

Safety of Epidural Steroid Injections for Lumbosacral Radicular Pain: Unmet Medical Need.

OBJECTIVE: Epidural steroid injections (ESIs) are a commonly utilized treatment for lumbosacral radicular pain caused by intervertebral disc herniation or stenosis. Although effective in certain patient populations, ESIs have been associated with serious complications, including paralysis and death. In 2014, the US Food and Drug Administration (FDA) issued a safety warning on the risk of injecting corticosteroids into the epidural space. The aims of this article were to review the neurological complications associated with ESIs and to compare the formulations, safety, and effectiveness of comm

Electromyography and Therapeutic Response to Lumbosacral Epidural Steroid Injections: A Systematic Review.

The association between electromyography (EMG)-confirmed lumbosacral (LS) radiculopathy and pain outcomes following epidural steroid injection (ESI) has not been systematically summarized. The primary objective of this systematic review was to summarize the effects of EMG-confirmed LS radiculopathy on pain intensity following ESI. A secondary objective was to summarize the effects of EMG-confirmed LS radiculopathy on physical functioning following ESI. An a priori protocol was registered and a database search conducted by a reference librarian from January 2000 through December 2020. The searc

Iatrogenic neurological injury after radiofrequency ablation and epidural steroid injections: illustrative cases.

BACKGROUND: Neck pain is often chronic and disabling. Cervical facet joint injections and epidural steroid injections are frequently used to manage chronic neck pain and cervicogenic headaches. While minimal side effects are commonly associated with these treatments, severe complications are exceedingly rare. OBSERVATIONS: The authors report 4 cases of iatrogenic neurological injury after radiofrequency ablation (RFA) and epidural steroid injections. One patient experienced left shoulder, scapular, and arm pain with left arm and hand weakness that developed immediately after RFA for chronic n
