Ocular injuries in children after major trauma.

OBJECTIVE: To determine the variety of ocular injuries sustained by children during major trauma. SUBJECTS AND METHODS: The pediatric trauma registry of The Children's Hospital in Denver, Colorado, was surveyed for children with ocular injuries and an Injury Severity Score higher than 15. The injuries were tabulated and correlated with mechanisms of injury to determine identifiable injury patterns. RESULTS: In a pediatric trauma center, ocular injuries were nearly twice as frequent among children with major trauma (Injury Severity Score > 15). Two-thirds of the children with an Injury Severi…

Associations between initial injury severity in acute hand, wrist or forearm injuries and disability ratings and time to return to work.

BACKGROUND: This study aimed to examine the relationships between the initial anatomic severity of hand, wrist and forearm injuries, as evaluated by the Modified Hand Injury Severity Score (MHISS), and each of the following parameters: disability rating and time to return to work. METHODS: In this study, 94 patients who underwent operations due to acute hand, wrist and forearm injuries were included. MHISS was used to assess the severity of the injury. Disability rates of the patients were calculated six months after injury in accordance with the 'Regulation on Disability Criteria, Classifica…

Impact of Accidental Hypothermia on Pulmonary Complications in Multiply Injured Patients With Blunt Chest Trauma - A Matched-pair Analysis.

BACKGROUND/AIM: Blunt chest trauma is one of the major injuries in multiply injured patients and is associated with an increased risk of acute respiratory distress syndrome (ARDS) and ventilator-associated pneumonia (VAP). Accidental hypothermia is a common accompaniment of multiply injured patients. The objective of this study was to analyze the influence of accidental hypothermia on pulmonary complications in multiply injured patients with blunt chest trauma. PATIENTS AND METHODS: Multiply injured patients [injury severity score (ISS) ≥16] with severe blunt chest trauma [abbreviated injury …

On-scene predictors of injury severity, deterioration, and death following road injury: a systematic scoping review of information available to responders and its use in triage tools.

BACKGROUND: Road injury including motor vehicle collisions (MVCs) remain a leading cause of death and disability worldwide. Early recognition of injury severity at the scene is critical to optimise emergency response, yet existing triage systems under- and over-triage substantial numbers of patients. This scoping review aimed to identify physiological, mechanistic, and contextual indicators available at the scene that predict injury severity, deterioration, or death, and to map these indicators across clinical responders, professional bystanders, and lay responders. METHODS: We conducted a sy

Review of trauma scoring systems in geriatric patients.

As the aging population progresses, more and more traumatic events involve the elderly. Given the systemic "frailty" of the elderly, there is currently no universally accepted method for geriatric trauma assessment. The existing methods for assessing geriatric trauma patients - the Geriatric Trauma Outcome Score (GTOS), the Elderly Mortality After Trauma (EMAT), and Geriatric Trauma Mortality Score (GERtality)-were reviewed. GTOS, GERtality and EMAT have 3, 5 and 26 parameters, respectively. EMAT contains almost every aspect of clinical scenarios, such as demographic data, comorbidities, injur

Initial identification of severely injured pediatric trauma patients-Clinical signs, scoring systems, and anticipating acute intervention needs: What you need to know.

Severely injured children are often first evaluated at nonpediatric designated hospitals where high-acuity pediatric trauma is seldom encountered, and delays in recognition of physiologic compromise can adversely affect outcomes. While the general principles of initial assessment and triage parallel those used in adults, children have important anatomic and physiologic differences that warrant a tailored approach in the early evaluation and resuscitation. Effective stabilization of critically injured children depends not only on clinical expertise but also on institutional preparedness and coo

Eyes Are the Windows to the Soul: Reviewing the Possible Use of the Retina to Indicate Traumatic Brain Injury.

Traumatic brain injury (TBI) induces complex molecular and cellular responses, often leading to vision deterioration and potential mortality. Current objective diagnostic methods are limited, necessitating the development of novel tools to assess disease severity. This review focuses on the retina, a readily approachable part of the central nervous system (CNS), as a potential indicator of TBI. We conduct a targeted database search and employ a blinded scoring system, incorporating both human and artificial intelligence (AI) assessments, to identify relevant articles. We then perform a detaile

Damage control laparotomy outcomes in a major urban trauma centre.

BACKGROUND: Damage control laparotomy (DCL) is associated with high mortality. The purpose of this study was to review the outcomes of DCL. METHODS: All patients undergoing DCL for penetrating trauma from May 2015 to July 2017 were reviewed. Data retrieved were demographics, mechanism of injury, vitals, and biochemical parameters. Injury severity was described by the revised trauma score (RTS), penetrating abdominal trauma index (PATI), injury severity score (ISS) and trauma and injury severity score (TRISS). Indications for DCL, length of ICU stay, number of procedures and primary abdominal …

Influence of Severe Thoracic Trauma on Choosing the Correct Surgical Strategy in Patients with Polytrauma from Kosovo.

BACKGROUND Severe thoracic trauma (TT) has a significant impact on the selection of treatment strategy in patients with polytrauma. Our aim was to assess the impact of severe TT on choosing the optimal surgical procedure to decrease mortality. MATERIAL AND METHODS Overall, 66 patients with polytrauma and significant TT were analyzed. Demographic data, trauma history, admittance imaging, injury details, injury severity scores, conservative treatment, surgical treatment, days of hospital stay, and mortality data were gathered. Frequencies of thoracic surgical procedures and other treatments were…

[Management of third and fourth degree perineal tears: A systematic review].

This was a comprehensive literature review using Pubmed, Medline, Embase and Cochrane, whose aim was to analyse the prevalence of anal sphincter injuries, their risk factors, their management and their functional prognosis. The prevalence of 3rd and 4th degree perineal tears varies between studies from 2.95% regardless the parity to 25% in nulliparous women. Twenty-eight percent to 48% of these lacerations were clinically occult. Perineal tears were associated with (multivariate analysis) forceps (6.021 [IC 95% 1.23-19.45]), nulliparity (OR 9.8 [IC 95% 3.6-26.2]), gestational age over 42 SA (O
