Left-to-right shunt reduction with intra-aortic balloon pump in postmyocardial infarction ventricular septal defect.

This case aims to describe the hemodynamic effects of intra-aortic balloon pump (IABP) in patients with ventricular septal defect (VSD) complicating myocardial infarction (MI). A 79-year-old man with no previous cardiovascular history presented to the emergency department with subacute inferior myocardial infarction associated with mild signs of systemic hypoperfusion. A transthoracic echocardiography revealed a large akinesia of the left ventricular inferior wall with preserved global left ventricular ejection fraction, as well as a large VSD in the midinferior portion of the interventricular

Successful treatment of severe myocardial failure after postpartum haemorrhage with the use of an intra-aortic balloon pump.

We report a 29-year-old primigravid who developed cardiac failure following postpartum haemorrhage unresponsive to volume resuscitation and therapy with catecholamines and phosphodiesterase-inhibitors. Transoesophageal echocardiography (TEE) demonstrated left atrial and ventricular dilatation and global left ventricular hypokinesis. No elevation of serum MB-isoenzyme fraction was detected and other organ functions remained stable. Although emergency cardiac transplantation was considered in the presented patient, the institution of intra-aortic counterpulsation was decided on as a first treatm

Preoperative intra-aortic balloon pump use and clinical outcomes in patients undergoing surgery for mitral regurgitation.

BACKGROUND: Mitral regurgitation (MR) requiring surgical correction poses significant perioperative challenges, particularly in hemodynamically compromised patients. Evidence supporting preoperative intra-aortic balloon pump (IABP) use in this setting remains scarce. This study evaluated the association between preoperative IABP use and clinical outcomes in patients undergoing surgical MR correction. METHODS: We conducted a retrospective cohort study utilizing the Nationwide Readmissions Database (NRD) from 2016 through 2020. Adult patients undergoing surgical mitral valve repair or replaceme

Impact of intra-aortic balloon pump support on cerebral blood flow velocity during venoarterial ECMO.

INTRODUCTION: Venoarterial extracorporeal membrane oxygenation (V-A ECMO) is frequently combined with intra-aortic balloon pump (IABP) support in patients with refractory cardiogenic shock. Although IABP may reduce V-A ECMO-related afterload, its effect on cerebral perfusion remains uncertain. We aimed to assess the impact of IABP support on middle cerebral artery (MCA) blood flow velocity measured by transcranial Doppler ultrasonography in patients receiving V-A ECMO. METHODS: We conducted a retrospective study in our tertiary intensive care unit between November 2021 and May 2022. Adults re

Intra-aortic balloon pump rupture and metallic frame entrapment requiring complex retrieval and hybrid repair.

A 50-year-old woman required an emergent operative exploration after an unsuccessful attempt at bedside removal of a ruptured intra-aortic balloon pump (IABP). Initial fluoroscopy demonstrated that the IABP tip moved dynamically with blood flow, but retraction and advancement caused significant bowing of the proximal external iliac artery. After unsuccessful endovascular snaring and resheathing, an exploratory laparotomy was required to remove the entrapped IABP via a longitudinal external iliac arteriotomy. Proximal to the patch angioplasty, an occlusive dissection flap was identified, requir

Dynamic left ventricular outflow tract obstruction induced by intra-aortic balloon pump in patient with angioedema.

BACKGROUND: Intra-aortic balloon pump is used for temporary mechanical support of failing left ventricle. It works by reducing the arterial afterload during ventricular systole to reduce myocardial work and increasing diastolic proximal aortic pressure to improve coronary perfusion. Rarely, intra-aortic balloon pump (IABP) can become the cause of severe haemodynamic compromise, causing dynamic left ventricular outflow tract obstruction. CASE PRESENTATION: An 88-yo man presented with angiotensin converting enzyme inhibitor (ACEI) - induced angioedema. He received steroids and adrenaline, but p

Concomitant intra-aortic balloon pump and veno-arterial extracorporeal membrane oxygenation for postcardiotomy cardiogenic shock.

We aimed to compare the outcomes of ECMO with and without IABP for postcardiotomy cardiogenic shock. The study included 103 patients who needed ECMO for postcardiotomy cardiogenic shock. Patients were grouped according to the use of IABP into ECMO without IABP (n = 43) and ECMO with IABP (n = 60). The study endpoints were hospital complications, successful weaning, and survival. Patients with IABP had lower preoperative ejection fraction (p = 0.002). There was no difference in stroke (p = 0.97), limb ischemic (p = 0.32), and duration of ICU stay (p = 0.11) between groups. Successful weaning wa

Intra-aortic balloon pump on in-hospital outcomes of cardiogenic shock: findings from a nationwide registry, China.

AIMS: The real-world usage of intra-aortic balloon pump (IABP) in various cardiogenic shocks (CS) and the association with outcomes are lacking. We aimed to investigate IABP adoption in CS in a nationwide registry in China. METHODS AND RESULTS: We retrospectively retrieved data of 30 106 CS patients (age 67.1 ± 14.6 years, 37.6% female patients) in the Hospital Quality Monitoring System registry from 2013 to 2016. Ischaemic heart disease was the leading cause of CS (73.9%). Hypertension, cardiomyopathy, myocarditis, valvular, and congenital heart disease were seen in 36.0%, 7.5%, 2.6%, 7.3%,

Intra-aortic Balloon Pump Use With Extra Corporeal Membrane Oxygenation-A Mock Circulation Loop Study.

Venoarterial extracorporeal membrane oxygenation (ECMO) is used in cardiogenic shock refractory to inotropic support and intra-aortic balloon pump (IABP) support. Peripheral ECMO can lead to ventricular distention, and IABP can be used to mitigate these effects. The aim of this study was to quantify the effects of IABP concomitant with ECMO, under different simulated hemodynamic conditions in a mock circulatory loop. Different simulated states of isolated left ventricular (LV) failure and biventricular failure with graded LV failure severities were supported with ECMO and ECMO with IABP. The i

Is there still a role for the intra-aortic balloon pump in the management of cardiogenic shock following acute coronary syndrome?

The intra-aortic balloon pump has been widely used in the management of cardiogenic shock. Reducing cardiac afterload and myocardial oxygen consumption, and improving coronary blood flow, this safe and simple mechanical circulatory support has been considered the cornerstone of cardiogenic shock management for decades. However, because it failed to provide any clinical benefit in recent randomized trials, the latest guidelines discourage its routine use in this clinical setting. Moreover, new percutaneous circulatory supports providing greater haemodynamic improvement have recently been develo

Answer to case of the month #128. Spectral waveform owing to intra-aortic balloon pump counter pulsation.

The intra-aortic balloon pump (IABP) is inserted percutaneously, usually through the femoral artery with placement in the descending aorta with its tip at the distal aortic arch (below the origin of the left subclavian artery). The balloon is connected to a drive console, consisting of a pressurized gas reservoir, a monitor for ECG and pressure wave recording, adjustments for inflation and deflation timing, triggering selection switches and battery backup power sources. The gases used for inflation are either helium or carbon dioxide. The advantage of helium is its lower density and therefore

Beneficial effect of preventative intra-aortic balloon pumping in high-risk patients undergoing first-time coronary artery bypass grafting-a single center experience.

Although intra-aortic balloon pumping (IABP) has been used widely as a routine cardiac assist device for perioperative support in coronary artery bypass grafting (CABG), the optimal timing for high-risk patients undergoing first-time CABG using IABP is unknown. The purpose of this investigation is to compare preoperative and preventative IABP insertion with intraoperative or postoperative obligatory IABP insertion in high-risk patients undergoing first-time CABG. We reviewed our IABP patients' database from 2002 to 2007; there were 311 CABG patients who received IABP treatment perioperatively.…
