Ischemic stroke and transient ischemic attack associated with neuroborreliosis: a Danish retrospective cohort.

UNLABELLED: Central nervous system (CNS) vasculitis and stroke are rare manifestations of neuroborreliosis (NB). We aimed to describe the clinical course, paraclinical findings, treatments, and outcomes of patients with ischemic stroke and transient ischemic attack (TIA) associated with NB from a comprehensive patient cohort and compare these findings to existing literature. We retrospectively analyzed data from 3841 patients who underwent intrathecal Borrelia burgdorferi sensu lato antibody index testing at Herlev and Gentofte Hospital between 2016 and 2024. Patients who fulfilled the EFNS cr

Recurrent Ischemic Stroke and Transient Ischemic Attack: Risk of Single and Multiple Recurrence.

Background/Objectives: Efforts have been made toward primary or secondary stroke or transient ischemic attack (TIA) prevention. However, little attention has been paid to recurrent stroke or TIA. This study investigated risk factors for multiple or single recurrent stroke or TIA. Methods: Data from 3646 patients with ischemic stroke or TIA were obtained from the Korea University Ansan Hospital Stroke Center between March 2014 and December 2021, using the prospective institutional database of the Korea University Stroke Registry. The associations between clinical features and recurrent stroke o

Hemoglobin Concentration and Clinical Outcomes After Acute Ischemic Stroke or Transient Ischemic Attack.

Background Anemia or low hemoglobin can increase the risk of stroke. However, the association between hemoglobin and outcomes after stroke is uncertain. In this study, we aimed to investigate the association between hemoglobin and clinical outcomes, including mortality, poor functional outcome, stroke recurrence, and composite vascular events at 1 year. Methods and Results We included the patients diagnosed with acute ischemic stroke or transient ischemic attack from the Third China National Stroke Registry. We used the Cox model for mortality, stroke recurrence, and composite vascular events

Optimal Duration of Aspirin Plus Clopidogrel After Ischemic Stroke or Transient Ischemic Attack.

Background and Purpose- The role of aspirin plus clopidogrel (A+C) therapy compared with aspirin monotherapy in patients presenting with acute ischemic stroke (IS) or transient ischemic attack remains uncertain. We conducted this study to determine the optimal period of efficacy and safety of A+C compared with aspirin monotherapy. Methods- Ten randomized controlled trials (15 434 patients) were selected using MEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials (CENTRAL) (inception June 2018) comparing A+C with aspirin monotherapy in patients with transient ischemic attack o

Antithrombotic management for transient ischemic attack and ischemic stroke (other than atrial fibrillation).

The new definition and risk stratification for transient ischemic attack (TIA) have clear implications for the urgency of evaluation and treatment. The optimal antithrombotic treatment for TIA is being intensively studied. New guidelines for prevention of non-cardioembolic stroke in patients with stroke or TIA recommend the use of antiplatelet agents rather than oral anticoagulation. New antiplatelet drugs are being used in cardiovascular patients, and their role in cerebrovascular patients is being studied. The impact of genetic CYP2C19 polymorphisms is becoming clarified in cardiovascular pa

Prevalence, knowledge, and treatment of transient ischemic attacks in China.

OBJECTIVES: To investigate the prevalence, knowledge, and treatment of TIA in a Chinese adult population. METHODS: We conducted a complex, multistage, probability sampling-designed, cross-sectional, nationwide survey of 98,658 Chinese adults in 2010. Possible TIA cases were first identified by symptoms recall or self-reported history of TIA through face-to-face interviews, and the final diagnosis was then made by expert neurologists through phone interviews or record review. RESULTS: The age-standardized prevalence of TIA was 2.27%. Clinically, only 16.0% of the participants were diagnosed b

Transient ischemic attacks and inhibitory seizures in elderly patients.

BACKGROUND: The new definition of a transient ischemic attack (TIA) makes it more difficult to distinguish it from an inhibitory seizure (IS). PURPOSE: The present study compares the characteristics of patients with an IS to those with a TIA, taking into account the old and the new definition of a TIA. PATIENTS AND METHODS: The clinical characteristics and the electroencephalogram (EEG) findings of 25 patients with an IS were compared to 252 patients with a TIA according to the old definition. Subgroups of 10 patients with an IS without structural lesions and 195 TIA patients according to th

[Shrinkage of carotid plaque in a patient with transient ischemic attack].

A 65-year-old man visited to our hospital, because he felt dull pain on the right side of the neck and had transient weakness of the left hand repeatedly. Carotid ultrasonography revealed a large plaque with severe stenosis and ulceration at the bifurcation of right common carotid artery. The plaque was mostly hypoechoic and only the basal part was hyperechoic. Magnetic resonance (MR) study showed multiple high-intensity spots in superficial borderzone area of the right cerebral hemisphere on fluid attenuated inversion recovery images, although diffusion-weighted images revealed no abnormality

Cerebral Embolism as a Cause of Stroke and Transient Ischemic Attack.

The most frequent cause of stroke and transient ischemic attacks is cerebral embolism. Cardiogenic cerebral embolization is common among patients with any cause of atrial fibrillation (AF) but particularly in AF resulting from rheumatic and arteriosclerotic heart disease. Rare causes of cerebral embolism include fat entering the bloodstream after trauma, tumor cells arising from atrial myxomata, and gas embolism. Cerebral embolic infarctions and their sources of origin can now be confirmed during life by many invasive (I) and noninvasive (NI) procedures including computerized tomography (CT) s

[A case of transient ischemic attacks preceded by postprandial hypotension].

A 78-year-old man with a history of total gastrectomy, myocardial infarction and hypertension was admitted to our hospital because of frequent transient attacks of consciousness disturbance, aphasia and right hemiparesis after meals. His attacks occurred one to three times a day, 30 to 60 minutes after meals and lasted 120 to 180 minutes. At the time of the attacks his blood pressure decreased, but his pulse rate did not change. Cerebral angiography demonstrated atherosclerotic occlusion of the left internal carotid artery. There was no myocardial uptake of 123I-metaiodobenzylguanidine (MIBG).

Transient ischemic attacks. Strategies for minimizing stroke risk.

In patients with transient ischemic attack (TIA), the risk of stroke increases greatly, especially in the months immediately following the initial attack. Diagnosis of TIA is based primarily on the patient's cerebrovascular history, since results of neurovascular examination are usually normal. TIA is often related to atherosclerotic arterial disease but can have numerous causes. Migraine, focal seizures, and other neurologic conditions can closely mimic TIA. Surgical and medical therapies help minimize the risk of stroke. The choice of therapy depends on the vascular territory of ischemia, th

[Management of transient ischemic cerebral attacks (author's transl)].

Practitioners should be familiar with the description of transient ischemic attacks (TIA) as 40% are followed more or less rapidly by a stroke. Analysis of the natural history of TIA shows that patients die twice as often from cardiac manifestations of atheroma than from cerebral lesions. This demonstrates the significance of diagnosis and treatment of risk factors. Patients are at increased risk of stroke during the six to twelve months following TIA. Anticoagulants and inhibitors of platelet aggregation seem to reduce this risk. Carotid surgery is indicated only for localized vascular lesion

[Transient ischemic attacks with intracranial tumors (author's transl)].

Transient ischemic attacks are not only the consequence of cerebral atherosclerosis. A woman of 48 years had transient ischemic attacks because of a meningeom narrowing the internal carotid artery. A steal syndrome in tumor vessels of a glioblastoma must be presumed in a man of 67 showing initial hemisyndrome with first transient, later on remaining palsy.

Activity and participation after transient ischemic attack: a scoping review.

Transient ischemic attack (TIA) is traditionally regarded as a condition with full recovery. However, emerging evidence suggests that some individuals experience residual symptoms, limiting their activity and participation in everyday life. The objective was to systematically map and synthesize existing evidence on activity limitations and participation restrictions following TIA. A scoping review was conducted following the framework proposed by Arksey and O'Malley and further outlined by Levac et al. A comprehensive search was conducted across four electronic databases.
