Results following treatment of traumatic dislocations of the knee joint.

In a retrospective study, 31 patients were identified over a period of 25 years as having sustained traumatic knee dislocations without associated fractures. Three patients had early amputation secondary to vascular complications. Ten popliteal artery disruptions and five peroneal nerve palsies were diagnosed. At the time of follow-up evaluation (average, 40 months), 16 patients were examined. Operatively treated patients (n = 6) tended to have better motion (129 degrees) than nonoperatively treated patients (108 degrees). Varus instability was seen in only two patients treated nonoperatively.…

Vascular injury associated with low-velocity dislocations of the knee.

Complete dislocation of the knee is a relatively rare condition. When it occurs as a result of high-velocity injury, such as in a road traffic accident, associated vascular injury is generally suspected. In low-velocity injuries, however, distal pulses are often maintained throughout, and the possibility of vascular injury may erroneously be discounted. We report four cases of low-velocity dislocation of the knee, only one of which had an overt vascular disruption, but three of which had arterial damage. On the basis of our experiences, we recommend arteriography in all cases of complete dislo

The known and unknown reality of knee dislocations: A systematic review.

BACKGROUND: Knee dislocations (KD) are limb threatening injuries known to cause significant dysfunction and disability. This review aims to summarize KD knowledge and uncover areas where more research is needed. METHODS: The published literature was queried according to PRISMA guidelines. Studies eligible for inclusion were clinical studies of knee dislocations. Studies published before 1990, or that focused on patellofemoral dislocation were excluded. RESULTS: A total of 132 studies involving 43,869 knee dislocations were analyzed. The majority of patients were young adult males, with high-…

A systematic review on management and outcome of irreducible knee dislocations.

BACKGROUND: Irreducible knee dislocations (IKD) are rare and can often be missed or misdiagnosed. The incidence of knee dislocation is quoted between 0.01% and 0.2% of all orthopaedic injuries, with up to 4% of these dislocations sub-classified as irreducible. The primary aim of this systematic review was to analyse cases of IKD described in the literature, with a secondary aim of producing a streamlined approach for managing these patients. PATIENTS AND METHODS: A systematic review of the literature was conducted on 1st September 2021 in accordance with the PRISMA guidelines using the online…

Managing dislocations of the hip, knee, and ankle in the emergency department.

Dislocation of the major joints of the lower extremities--hip, knee, and ankle--can occur due to motor-vehicle crashes, falls, and sports injuries. Hip dislocations are the most common, and they require emergent management to prevent avascular necrosis of the femoral head. Knee dislocations are uncommon but potentially dangerous injuries that can result in amputation due to the potential for missed secondary injury, especially if they are reduced spontaneously. Isolated ankle dislocations are relatively rare, as most ankle dislocations involve an associated fracture. This review presents an al…

Meniscal tears and articular cartilage damage in the dislocated knee.

PURPOSE: Knee dislocations can cause significant damage to intra-articular knee structures, but currently there are limited data reporting articular cartilage and meniscal injuries in this setting. The purpose of this study is to (1) report the rate of concomitant intra-articular injuries at the time of multiligament reconstruction for knee dislocation, (2) determine whether the pattern of ligament injury is associated with the presence of chondral and meniscal injuries, and (3) assess the relationship between timing of surgery and incidence of chondral and meniscal injuries. METHODS: The rec…

The changing demographics of knee dislocation: a retrospective database review.

BACKGROUND: Knee dislocations are uncommon but devastating orthopaedic injuries. Little is known about their frequency and the types of patients who are affected. QUESTIONS/PURPOSES: Using a large national insurance database, we determined (1) the incidence of knee dislocation in patients with orthopaedic injuries and examined the incidence as a function of (2) year of diagnosis, (3) dislocation type (open versus closed, direction), and (4) patient demographic factors (sex, age). METHODS: We searched the PearlDiver database, a national database of private insurance records consisting of 11 m…

[The role of MRI in dislocations of the patella and other knee pathologies].

In addition to ultrasound and conventional x-ray, magnetic resonance imaging (MRI) plays an important role in the visualization of lesions of the knee. It allows the orthopaedic surgeon to safely detect ruptures of the cruciate ligaments, the meniscus, osteochondral lesions and other ligamentary structures such as the collateral ligaments and the medial patellofemoral ligament (MPFL). In patellar dislocations, risk factors for recurrent dislocations can be determined and the therapy can be adjusted accordingly. This articles aims to give the reader a comprehensive overview on current concepts a

Knee dislocations: is reconstruction of the posterior cruciate ligament crucial?

The purpose of the present study was to investigate our 6-10 year results for knee dislocations with posterolateral corner (PLC) involvement, where a primary repair was performed laterally, the anterior cruciate ligament (ACL) reconstructed, but the posterior cruciate ligament (PCL) was left without surgery. Four consecutive patients with knee dislocation with complete rupture of the ACL, the PCL and the PLC were operated on by the same surgeon with similar technique. There were no other major injuries. We used strict inclusion criteria to get as homogenous population as possible. The ACL inju…

Late presentation of congenital dislocation of the knee: a case report.

Congenital dislocation of the knee is a rare disorder. Late presentation of congenital dislocation of the knee at an older age is a therapeutic challenge. A 12-year-old girl presented to us with congenital dislocation of the knee and with complaints of limp, short limb, and pain on weight bearing. Two-stage surgery was performed with quadricepsplasty followed by gradual distraction in the first stage and repeat quadricepsplasty, anterior capsular release, and open reduction in the second stage. The result was fair to good with a stable and painless knee on walking.

A minimally invasive treatment protocol for the congenital dislocation of the knee.

BACKGROUND: Congenital dislocation of the knee is a rare condition for which the treatment is difficult and remains controversial. For severe cases associated with neuromuscular disorders treatment has usually consisted of extensive surgical reconstruction. The purpose of this study is to assess the short-term results of a new method of treatment for this patient population that involves casting and less extensive surgery. METHODS: We retrospectively reviewed the cases of 8 consecutive patients, 4 girls and 4 boys, with 16 congenitally dislocated knees that had been diagnosed and treated by a

The MR dimple sign in irreducible posterolateral knee dislocations.

While posterolateral knee dislocation is a rare entity, its identification has important implications. The invagination of skin seen in posterolateral knee dislocations is referred to as the "dimple sign" on physical exam. We report a case where the "dimple sign" was also demonstrated on magnetic resonance imaging. Failure to recognize this finding on imaging will render the knee irreducible, as a result of interposed tissues, until discovered by the orthopedic surgeon intraoperatively. The incarcerated medial capsule and medial collateral ligament could also be misinterpreted as torn meniscus…

Posterolateral dislocation of the knee joints: analysis of 9 cases.

OBJECTIVE: To analyze the traumatic pathological characteristics of posterolateral dislocation of knee joints and its treatment. METHODS: Nine cases of posterolateral dislocation of knee joint, 5 cases of fresh injuries (the fresh injury group) and 4 cases of old injuries ( the old injury group) were reviewed and analyzed. In the fresh injury group 4 cases failed in close reduction due to "buttonholing" through the medial joint, among them 3 cases underwent repair of the damaged ligaments. In the old injury group 2 cases underwent ACL and MCL repair only in acute stage, but re-dislocated. Of

[Diagnosis and therapy of vascular injuries in posterior knee dislocation].

AIM: We demonstrate the management and treatment of dislocation of the knee associated with vascular injury. The goal of the treatment is to avoid complications due to ischemia. The injured vessel can be repaired either by direct suture or by interposition of a saphenous vein graft. Capsule and ligaments should be reconstructed secondarily. PATIENTS AND METHODS: The charts of ten patients treated in the Division of Traumatology of the University of Zurich between 1979 and 1996 have been retrospectively checked. RESULTS: In eight of ten patients the injured vessel has been reconstructed with
