Intestinal lactase status of adults in Papua New Guinea.

Fifty hospital patients in Port Moresby (mean age 30 yr) without evidence of malnutrition or gastro-intestinal disease, who came from many parts of Papua New Guinea (P.N.G.), were given 50 g lactose orally after an overnight fast. In 49 blood glucose rise was less than 1.1 mmol 1(-1), indicating hypolactasia. Thirty-three gave a clear history of symptoms, usually diarrhoea, after lactose. Adult hypolactasia approaches 100% in P.N.G. There is no indication that the genetic structure of any part of the population of P,N.G. with regard to lactase has been influenced by genetic drift from peoples …

Temproary intestinal lactase deficiency in light-treated jaundiced infants.

The intestinal lactase activity in six newborn jaundiced light-treated infants with diarrhea and in eight normal controls were compared by lactose tolerance test (LTT). The ability to hydrolyze lactose was minimal in the jaundiced infants during light-treatment compared to the controls which could absorb lactose very well. Peroral intestinal biopsies were taken from the newborn jaundiced infants during light-treatment. By histochemical technique no intestinal lactase activity was found in these intestines. When the jaundiced infants with diarrhea were given lactose-free diet, the stools normal…

Electrical assessment of functional lactase activity in conscious man.

Using an electrical technique for measuring transjejunal potential differences (PDs) in conscious man, we have estimated the electrogenic absorption of the hexoses liberated by hydrolysis of lactose which was infused into the jejunum of one normal control and 21 patients with diarrhoea. The results were compared with jejunal lactase levels determined from biopsy specimens taken from the recording site immediately after infusion. The PD evoked by 100 mM lactose was very significantly lower in patients with lactase levels below 4 units (lactase deficient) compared with subjects with normal lacta…

Suitability of the azocoupling reaction with 1-naphthyl-beta-D-glucoside for the histochemical demonstration of lactase (lactase-beta-glucosidase complex) in human enterobiopsies.

The suitability of the simultaneous azocoupling reaction with 1-naphthyl-beta-D-glucoside and hexazonium-p-rosanilin in the detection of the activity of lactase (or lactase-beta-glucosidase complex) in jejunal biopsies of patients with various forms of the malabsorption syndrome was tested. Results were compared with those obtained with the indigogenic method using 4-Cl-5-Br-3-indolyl-beta-D-fucoside which is the method of choice. Both methods gave identical results as far as the relative intensity of the brush border staining was concerned. The azocoupling method applied in unfixed cold micro…

Congenital Lactase Deficiency, An Unusual Cause of Infantile Hypercalcemia.

Infantile hypercalcemia is an uncommon presentation, even for a pediatric endocrinologist. However, the combination of diarrhea and hypercalcemia makes the situation particularly intriguing. In this report, the authors present the case of an infant referred to them for hypercalcemia, who had been experiencing watery diarrhea, abdominal distension, and failure to thrive since early neonatal age. The eventual diagnosis was congenital lactase deficiency. Targeted management with lactose-free feeds normalized calcium levels within a week of starting the therapy. During the first follow-up visit, t…

Simple Strategy to Protect Lactase Activity in Solid Formulation.

BACKGROUND: Lactose intolerance is characterized by the absence of the enzyme lactase (beta-galactosidase) and affects two thirds of the world adult population. Our aim was to evaluate a lactase gastro-resistant formulation to ensure increased activity in the action site of the enzyme (lumen of the small intestine). Simultaneously, we also evaluated the commercial product stability and enzyme activity, because the product containing beta-galactosidase is classified as food supplement according to the Food and Drug Administration (FDA), so it is free to pass quality testing, efficacy and stabil…

Primary lactase deficiency and past malarial endemicity in Sardinia.

BACKGROUND: It has recently been suggested that primary lactase deficiency might have been selected for by malaria, as occurred for beta-thalassaemia and glucose 6-phosphate dehydrogenase deficiency. However, recently we have found that the prevalence of primary lactase deficiency in the area of Sassari (Northern Sardinia), where, in the past, there was intermediate malarial endemicity, is comparable to that observed in the adult population from other areas of Southern Italy where malaria was less endemic. AIMS: To address the problem further, we have determined the prevalence of primary lact…

[Secondary lactase deficiency in children and its epidemiologic implications].

The main contributions in the knowledge of secondary lactase deficiency in children are reviewed. We present the clinical features of fermentative diarrhea and the current physiopathological issues, diagnostic procedures, and dietetic treatments, related to this diarrhea, as well as a review of the diseases associated with it. Finally, we discuss the epidemiological implications of the deficiency.

[Lactase polymorphism in representatives of different ethnic-territorial groups].

Lactase polymorphism was studied in the native population of West Siberia and also in Buryatia. LAC*R frequency observed is-Khants- 0.8367, Mansi - 0.8660, Nenets - 0.8944, Buryats - 0.6883. The data obtained are considered to be the result of natural selection under traditional historical economical-cultural environment of the ethnic groups in question.

[Lactase deficiency among representatives of various nationalities of Siberia].

Some features of lactase tolerance were investigated in representatives of varying groups of indigenous population in Siberia-Mansi, Khanty, Nenets, Buryats. The indirect method was used to evaluate lactase activity. A total of 92 subjects were investigated. It was found that 32% of the representatives of the Urals population group and 53% of Buryats were capable of lactase assimilating. The data obtained correspond to the cultural-genetic hypothesis on the nature of lactase tolerance in the representatives of different ethnic groups.

Delayed complete functional lactase sufficiency in breast-fed infants.

Colonic fermentation of unabsorbed lactose was evaluated by measuring breath hydrogen (BH2) after a regular feeding in 17 white, normal, exclusively breast-fed infants 4-5 weeks of age. Interval breath samples were collected with a specially modified face mask and analyzed by gas chromatography for H2 and CO2 concentrations. Five infants (29%) produced 20 ppm or more of H2, four of whom underwent repeated testing. Three infants stopped producing over 20 ppm of H2 as they grew older. On a repeat evaluation, one of these three infants had levels over 20 ppm when other foods were introduced into

Congenital lactase deficiency. A clinical study on 16 patients.

There are at least 20 rare autosomal recessive disorders that are excessively common in Finland of which congenital lactase deficiency is one. During the last 17 years we have found 16 cases. In each case the mother noted watery diarrhoea, generally after the first feed of breast milk, and at the latest, by age 10 days. The lactose malabsorption was verified at a mean age of 36 (range 3-90) days, by which time the infants were dehydrated and 15 of them weighed less than at birth (mean weight for age was -2.8 SDs). On a lactose-free elimination diet (a group of 6 on Nutramigen and a group of 10…
