Left atrial appendage closure (LAAC) is noninferior to oral vitamin K antagonist therapy for the reduction of nonvalvular atrial fibrillation-related stroke risk. Currently, the procedure is most widely accepted in patients who cannot tolerate oral anticoagulants. This patient population is generally comorbid, making any reduction in procedural complications paramount. LAAC has important complications described in the periprocedural and postprocedural periods. The prevention and management of complications regarding vascular access, transseptal puncture, pericardial effusion, device embolizati…

Percutaneous left atrial appendage (LAA) closure is a viable alternative to chronic oral anticoagulation for reducing thromboembolic risk in non-valvular atrial fibrillation (AF) patients. To date, no clinical data have been published on the novel nonpacifier LAA closure device evaluated herein. We therefore conducted a prospective, non-randomized, multi-center registry across 6 Chinese hospitals to provide the first clinical evidence of its safety and effectiveness among AF patients with an increased risk of stroke, using performance goals to assess 6-month effective LAA closure and 12-month

INTRODUCTION: Patients with atrial fibrillation (AF) on anticoagulation (AC) have increased mortality after trauma. Left atrial appendage closure (LAAC) procedures may reduce the need for AC in patients with AF. The purpose of this study is to evaluate the outcomes of trauma patients with AF on AC compared with patients not taking AC, and assess the frequency in which trauma patients are candidates for LAAC procedures.
METHODS: A retrospective review of adult trauma patients after a fall was performed. AF patients on AC (+AC) were compared with patients not on AC (-AC). A CHA2DS2-VASc score i…

BACKGROUND The left atrial appendage (LAA) is an organ with neuroendocrine function. It remains unclear whether left atrial appendage closure (LAAC) has physiological effects on neuroendocrine function in patients with nonvalvular atrial fibrillation (NVAF). In the present study, we aimed to investigate the effects of LAAC on neuroendocrine function in patients with NVAF. MATERIAL AND METHODS We enrolled 20 patients with NVAF treated by LAAC in Jiangsu Taizhou People's Hospital from October 2019 to October 2020. Blood samples were collected 1 day before LAAC and 12 months after LAAC. Plasma co…

BACKGROUND: Atrial fibrillation (AF) is the most common cardiac arrhythmia and is a major cause of embolic stroke. In patients with hereditary bleeding disorders such as haemophilia, management of AF particularly anticoagulation can be quite challenging. Left atrial appendage (LAA) closure is an emerging option in AF patients who are not eligible for oral anticoagulation therapy because of contraindications or high bleeding risk.
CASE SUMMARY: A 67-year-old man with permanent AF and haemophilia was referred for further evaluation of our cardiology clinic by his primary haematologist. The CHA2

BACKGROUND: The PRAGUE-17 (Left Atrial Appendage Closure vs Novel Anticoagulation Agents in Atrial Fibrillation) trial demonstrated that left atrial appendage closure (LAAC) was noninferior to nonwarfarin direct oral anticoagulants (DOACs) for preventing major neurological, cardiovascular, or bleeding events in patients with atrial fibrillation (AF) who were at high risk.
OBJECTIVES: This study sought to assess the prespecified long-term (4-year) outcomes in PRAGUE-17.
METHODS: PRAGUE-17 was a randomized noninferiority trial comparing percutaneous LAAC (Watchman or Amulet) with DOACs (95% ap…

BACKGROUND: Peri-device leakage (PDL) is one of the major complications of left atrial appendage closure (LAAC). However, there is a lack of predictive models in clinical practice. The aim of this study was to develop and validate a prediction model for PDLs after LAAC based on preoperative CT and CT-based radiomic features. METHODS: This retrospective cohort study included 100 patients with nonvalvular atrial fibrillation who underwent LAAC between August 2023 and August 2024 at two centers. A clinical model was constructed via binary logistic regression with clinical information, and a radio…

Thrombus formation in atrial fibrillation mostly occurs in the left atrial appendage (LAA). LAA closure (LAAC) seems logical to reduce thrombo-embolic AF-related stroke. Early device trials, which predominantly compared LAAC against oral anticoagulant (OAC) with warfarin, showing after 5 years less bleeding and possibly similar ischemic stroke rates with LAAC. New trials (CHAMPION-AF, OPTION, CLUSTER-AF and ASAP TOO) comparing LAAC versus direct OACs, or standard of care for the OAC-intolerant, have reported apparently discordant results, most probably related to the patient phenotype than LAA

Atrial fibrillation (AF) affects approximately 3%-5% of adults and is projected to double in prevalence by 2060, significantly increasing the burden of thromboembolic stroke. This risk is largely attributed to thrombus formation within the left atrial appendage (LAA), particularly in nonvalvular AF, where the LAA's trabeculated anatomy, diverse morphologies (e.g., chicken wing, windsock, cactus, cauliflower), and impaired contractility allows for blood stasis and thrombogenesis. As a result, the LAA has become a focus for stroke prevention strategies. This review demonstrates current evidence

BACKGROUND: Left atrial appendage (LAA) closure reduces stroke in patients with atrial fibrillation.
CASE SUMMARY: A 63-year-old man with chronic atrial fibrillation had undergone mitral valve replacement and endocardial suture closure of the LAA 5 years before presentation. One year postoperatively, a giant LAA was noted, and coronary angiography revealed a newly developed coronary-LAA fistula. Progressive enlargement of the LAA was confirmed 5 years postoperatively, with expansion of the fistulous tract on coronary angiography, ultimately leading to surgical LAA resection.
DISCUSSION: Inco

BACKGROUND: Spontaneous echo contrast (SEC) with atrial fibrillation (AF) is known to increase stroke risk. However, the effect of SEC grades on outcomes after left atrial appendage closure (LAAC) remains unclear.
OBJECTIVES: This study aimed to evaluate the impact of SEC grades on clinical outcomes after LAAC.
METHODS: A total of 1,276 consecutive patients undergoing LAAC were analyzed. They were classified into SEC 0+, SEC 1/2+, and SEC 3/4+ groups based on preprocedural transesophageal echocardiography. Clinical outcomes were compared among the groups.
RESULTS: There were 595 (46.6%), 50…

PURPOSE: Left atrial appendage (LAA) closure has become the standard of care for patients with atrial fibrillation (AF) at high risk of thromboembolism who are intolerant or unwilling to take anticoagulants. LAA occlusion device infection is a challenging complication, and there is a paucity of data on the management and outcomes of LAA occlusion device infection. We aimed to summarize the existing literature and highlight the knowledge gap in this area.
METHODS: A detailed search was conducted through the databases PubMed/MEDLINE, EMBASE, Web of Science, CINAHL, and Cochrane Central using th

INTRODUCTION: The Centers for Medicare & Medicaid Services (CMS) required a shared decision-making (SDM) interaction, with an "independent" physician, before left atrial appendage closure (LAAC). The purpose of this study is to better understand how this requirement is implemented in clinical practice.
METHODS: We surveyed LAAC-performing centers. The characteristics of respondent and nonrespondent hospitals were compared using the CMS Provider of Services File for 2017.
RESULTS: We received 86 responses out of 269 surveys mailed (32%). Respondent and nonrespondent hospital affiliations were…

BACKGROUND: When performing transcatheter left atrial appendage (LAA) closure, peridevice leaks and device-related thrombus (DRT) have been associated with worse clinical outcomes-hence, their risk should be mitigated.
OBJECTIVES: The authors sought to assess whether use of preprocedural computational modeling impacts procedural efficiency and outcomes of transcatheter LAA closure.
METHODS: The PREDICT-LAA trial (NCT04180605) is a prospective, multicenter, randomized trial in which 200 patients were 1:1 randomized to standard planning vs cardiac computed tomography (CT) simulation-based plan
