Non-fatal burden of disease due to mental disorders in the Netherlands.

PURPOSE: To estimate the disease burden due to 15 mental disorders at both individual and population level. METHODS: Using a population-based survey (NEMESIS, N = 7,056) the number of years lived with disability per one million population were assessed. This was done with and without adjustment for comorbidity. RESULTS: At individual level, major depression, dysthymia, bipolar disorder, panic disorder, social phobia, eating disorder and schizophrenia are the disorders most markedly associated with health-related quality of life decrement. However, at population level, the number of affected

"I Go Upstairs and Eat in Front of the Television:" the Cooking and Eating Behaviors of Mexican Older Women Living Alone.

The increasing tendency of older people living alone may pose a number of challenges including factors related to food insecurity, such as functional impairment, social isolation, and financial vulnerability. The purpose of this study was to explain the cooking and eating behaviors of Mexican older women living alone using a life course perspective. A qualitative research methodology was chosen and deductive thematic analysis was used to examine accounts given by 14 older women through one-on-one interviews. Findings showed that age-related transitions, role changes, and the social context in …

Born and bred to burn out: A life-course view and reflections on job burnout.

Burnout is a response to prolonged stressors at work, and is defined as a chronic syndrome including exhaustion, cynicism, and reduced professional efficacy. The 40 years of research on burnout have yielded thousands of studies on its measurement, antecedents, correlates, and consequences. However, most of these studies have used a cross-sectional design, and only very few have addressed burnout from a life-course perspective. In the first part of this article, we reflect on the ideas that inspired our multidisciplinary "A 35-Year Follow-Up Study on Burnout Among Finnish Employees," and the ch

Childhood abuse as a risk factor for sleep problems in adulthood: evidence from a U.S. national study.

BACKGROUND: Accumulating evidence indicates that stress impairs sleep quality. Few studies, however, have examined the extent to which early life stress can jeopardize sleep in adulthood. PURPOSE: Guided by a life course epidemiological perspective on health, this study examined associations between childhood abuse and adult sleep problems. METHODS: We used data from 835 respondents in the National Survey of Midlife Development in the United States (MIDUS). Self-report measures assessed the frequency of physical, emotional, and sexual abuse in childhood, as well as global and component indic

Entwined life events: The effect of parental incarceration timing on children's academic achievement.

Parental incarceration has negative effects on children's educational outcomes. Past studies have only analyzed, and therefore only treated as consequential, parental incarceration that occurs during childhood rather than prenatally. Such analyses that emphasize the importance only of events that occur during one's lifetime are common in life course studies. This paper introduces an "entwined life events" perspective, which argues that certain events are so consequential to multiple persons' lives that they should be analyzed as events within multiple independent life courses; parental incarce

Concepts of recovery: competing or complementary?

PURPOSE OF REVIEW: Within the last 5 years, concepts of recovery have taken center stage in psychiatry as the overarching goal of mental health services. In the course of this shift towards recovery, clinicians and consumers (and many others) have struggled to make the concept of recovery both measurable and meaningful. The clinical concept of recovery has focused upon the remission of symptoms and restoration of functioning. A rehabilitation model of recovery has been a more subjective and consumer-oriented concept that focuses on the full lives that are lived within the context of enduring d

Gender Differences: A Lifetime Analysis of the Economic Burden of Alzheimer's Disease.

BACKGROUND: Gender is one of the best-established differences in risk for Alzheimer's disease (AD) and other forms of dementia, with women being at greater risk. However, the financial implications are unknown. This study aims at understanding the economic burden of AD by gender. METHOD: This study takes a life-time perspective to investigate the burden of AD over the course of the disease. Nationally representative Medicare Current Beneficiary Survey data were used to estimate the course of illness of AD from age 65 to death and the incremental costs of AD on Medicare and Medicaid. Responding to Suffering:Development Course and Social Dimensions of Palliative Care.

A review of the development course of palliative care reveals that its goal of alleviating suffering determines the indispensability of the social dimension in this field. As palliative care emerges in response to the general trends such as aging and disease spectrum changes of social development,it naturally becomes one of the key solutions to social issues including the difficulties in caregiving and hospitalization for end-of-life patients.Beyond its social value, the comprehensiveness and localization of care emphasized by palliative care also require the support and intervention of theoreti

Fears born and bred: toward a more inclusive theory of fear acquisition.

Alleged differences between associative and non-associative perspectives are sometimes more apparent than real. The non-associative model describes a pathway to fear that is complementary to associative pathways. It does not seek to usurp conditioning models as applied to evolutionary-neutral fear. We discuss vexing definitional issues surrounding what qualifies as a conditioning event and what characterises the non-associative pathway. Genetic findings are shown to be consistent with the non-associative model of fear. Following discussion of the relation between stress-diathesis models and th

Treatment of Metastatic Spinal Disease; what the Radiologist needs to know.

Advancements in technology and multidisciplinary management have revolutionized the treatment of spinal metastases. Imaging plays a pivotal role in determining the treatment course for spinal metastases. This article aims to review the relevant imaging findings in spinal metastases from the perspective of the treating clinician, describe the various treatment options, and discuss factors influencing choice for each available treatment option. Cases that once required radical surgical resection or low-dose conventional external beam radiation therapy, or both, are now being managed with separat

Global paediatric advanced life support: improving child survival in limited-resource settings.

Nearly all global mortality in children younger than 5 years (99%) occurs in developing countries. The leading causes of mortality in children younger than 5 years worldwide, pneumonia and diarrhoeal illness, account for 1·396 and 0·801 million annual deaths, respectively. Although important advances in prevention are being made, advanced life support management in children in developing countries is often incomplete because of limited resources. Existing advanced life support management guidelines for children in limited-resource settings are mainly empirical, rather than evidence-based, writ

Obesity, incretin-based therapies, and cardiovascular disease in women.

Obesity in women is a growing and multifaceted public health challenge, driven by complex interactions among biological, hormonal, and psychosocial factors across the life course. From menarche to menopause, hormonal fluctuations influence fat distribution, energy balance, and metabolic risk, contributing to sex-specific trajectories of weight gain. Moreover, women-specific conditions, such as pregnancy-related weight retention, hypertensive disorders of pregnancy, and polyendocrine metabolic ovarian syndrome, further amplify long-term cardiometabolic risk. Women with obesity have an increased

Divergent associations of social isolation with body composition in Chinese middle-aged and older adults.

BACKGROUND: Social isolation represents a critical challenge in the context of global population aging, yet its effects on body composition in older adults remain paradoxical. This study aimed to systematically examine, from a life-course perspective, the differential associations of social isolation on sarcopenia and obesity among middle-aged and older adults in China, thereby evaluating the proposed divergent associations hypothesis. METHODS: Using data from 5,525 participants in the China Health and Retirement Longitudinal Study (CHARLS), we constructed life-course patterns of social isola

Beyond urinalysis: a review of life-course chronic kidney disease screening across health examination systems in Japan.

Chronic kidney disease (CKD) is a major public health problem. Most international guidelines recommend targeted testing of individuals at high risk rather than population-wide screening. In Japan, urinalysis is embedded in health examination systems across the life course. However, the existence of repeated urinalysis opportunities has not necessarily meant that these programs functioned as a coherent CKD screening pathway. Repeated urinalyses were introduced under different legal mandates and for different public health purposes, with substantial variation in target conditions, test component

Pre-migration Trauma Exposure and Psychological Distress for Asian American Immigrants: Linking the Pre- and Post-migration Contexts.

Drawing on the life course perspective and the assumptive world theory, this paper examines whether pre-migration trauma exposure is associated with psychological distress through post-migration perceived discrimination for Asian American immigrants. The study is based on cross-sectional data from the National Latino and Asian American Study (N = 1639). Structural equation model is used to estimate the relationship between pre-migration trauma, post-migration perceived discrimination, and psychological distress. Additional models are estimated to explore possible variations across ethnic group…
