Meconium aspiration syndrome (MAS) is a common cause of severe respiratory distress in term infants, with an associated highly variable morbidity and mortality. MAS results from aspiration of meconium during intrauterine gasping or during the first few breaths. The pathophysiology of MAS is multifactorial and includes acute airway obstruction, surfactant dysfunction or inactivation, chemical pneumonitis with release of vasoconstrictive and inflammatory mediators, and persistent pulmonary hypertension of newborn (PPHN). This disorder can be life threatening, often complicated by respiratory fai…

Meconium ileus is a life-threatening presentation of neonates with cystic fibrosis (CF). Notwithstanding, today the long-term prognosis of such patients is comparable to that of CF patients not diagnosed in screening programs and not suffering from this insult,1-3 as confirmed by the article of Johnson et al. in this issue of Pediatric Pulmonology. Good news then for CF patients and CF caretakers, working with the modern interdisciplinary setting of neonatal intensive care, including radiology, anesthesiology, and pediatric surgery experts. State of the art management of life-threatening neona…

BACKGROUND: Amnioinfusion is thought to dilute meconium present in the amniotic fluid and so reduce the risk of meconium aspiration.
OBJECTIVES: To assess the effects of amnioinfusion for meconium-stained liquor on perinatal outcome.
SEARCH STRATEGY: We searched the Cochrane Pregnancy and Childbirth Group's Trials Register (May 2009).
SELECTION CRITERIA: Randomised trials comparing amnioinfusion with no amnioinfusion for women in labour with moderate or thick meconium-staining of the amniotic fluid.
DATA COLLECTION AND ANALYSIS: Two review authors assessed eligibility and trial quality, an…

BACKGROUND: Meconium aspiration syndrome (MAS) is a major cause of severe respiratory distress in newborns and the role of antibiotics in its management is not well defined.
OBJECTIVE: To determine the role of routine antibiotic therapy in the management of MAS.
METHODS: After excluding the possibility of sepsis, 144 patients with MAS were randomised into two groups. Group A (study group) received ampicillin and gentamicin for 7 days, commencing between 24 and 36 hours of life, and group B, the controls, received no antibiotics. Both groups received similar supportive management. The primary…

BACKGROUND: It is uncertain whether amnioinfusion (infusion of saline into the amniotic cavity) in women who have thick meconium staining of the amniotic fluid reduces the risk of perinatal death, moderate or severe meconium aspiration syndrome, or both.
METHODS: We performed a multicenter trial in which 1998 pregnant women in labor at 36 or more weeks of gestation who had thick meconium staining of the amniotic fluid were stratified according to the presence or absence of variable decelerations in fetal heart rate and then randomly assigned to amnioinfusion or to standard care. The composite…

BACKGROUND/PURPOSE: Increasing evidence of physiologic in utero defecation supports the hypothesis that bowel damage in gastroschisis may be meconium dependent. In this study, the author investigated the role of meconium on parameters of bowel damage in a fetal rat model of gastroschisis.
METHODS: Pregnant rats underwent laparotomy at 18 1/2 days gestational age (GA). There were 4 experimental groups of 11 fetuses each; the G(M) group consisted of fetuses with isolated gastroschisis and was considered to have moderate meconium contamination of the amniotic fluid (MCAF); the G(L) group consist…

Meconium aspiration syndrome (MAS) is a common cause of respiratory distress in neonates. In many affected children, the complex nature of meconium aspiration syndrome contributes to the apparent lack of response to standard therapies. Over the past decade, several new therapies have been suggested to be more effective than "conventional" management in treating meconium aspiration syndrome. These include: anti-inflammatory drugs, medications to counter the effect of prostaglandin-related compounds, high-frequency ventilation, exogenous surfactant, inhaled nitric oxide and liquid ventilation. T…

OBJECTIVES: We hypothesized that the concentration of major essential mineral elements in meconium correlate with gestational age (GA) or birth weight. To verify this premise we determined the concentration in meconium of calcium, magnesium, phosphorus, copper, zinc, iron, and manganese.
METHODS: Thirty-four appropriate for age singleton infants without major congenital anomalies were divided into four GA groups (in weeks): 24 to 28; 29 to 33; 34 to 37; 38 to 42, or in birth weight groups (in g): < 1500; 1500-1999; 2000-2499; > or = 2500. Meconium was collected until the appearance of transit…

Neonatal intussusception in premature infants is uncommon and may mimic other abdominal disorders. We present a case of intussusception in a very low birthweight neonate. Presenting signs initially suggested necrotizing enterocolitis and meconium disease, and the diagnosis was confirmed at laparotomy.

OBJECTIVE: To compare pregnancy outcome between deliveries complicated by new onset of meconium during labor following prior evidence of clear amniotic fluid and labors in which meconium was present to begin with.
METHODS: A retrospective cohort study of all singleton term (≥37 + 0 weeks) deliveries complicated by intrapartum meconium-stained amniotic fluid in a tertiary referral medical center during the year 2012. Outcome was compared between deliveries with new onset of meconium during labor following prior evidence of clear amniotic fluid (secondary meconium group) and those in which meco…

Meconium aspiration syndrome (MAS) is a clinical condition characterized by respiratory distress in neonates born through meconium-stained amniotic fluid (MSAF). Despite advances in obstetric practices and perinatal care, MAS remains an important cause of morbidity and mortality in term and post-term newborns. Since the 1960s, there have been significant changes in the perinatal and postnatal management of infants born through MSAF. Routine endotracheal suctioning is no longer recommended in both vigorous and non-vigorous neonates with MSAF. Supportive care along with new treatments such as su…
