Effects of Gamma-Tocotrienol on Intestinal Injury in a GI-Specific Acute Radiation Syndrome Model in Nonhuman Primate.

The gastrointestinal (GI) system is highly susceptible to irradiation. Currently, there is no Food and Drug Administration (FDA)-approved medical countermeasures for GI radiation injury. The vitamin E analog gamma-tocotrienol (GT3) is a promising radioprotector in mice and nonhuman primates (NHP). We evaluated GT3-mediated GI recovery in total-body irradiated (TBI) NHPs. Sixteen rhesus macaques were divided into two groups; eight received vehicle and eight GT3 24 h prior to 12 Gy TBI. Proximal jejunum was assessed for structural injuries and crypt survival on day 4 and 7. Apoptotic cell death …

Recombinant Lassa Virus Expressing Green Fluorescent Protein as a Tool for High-Throughput Drug Screens and Neutralizing Antibody Assays.

Lassa virus (LASV), a mammarenavirus, infects an estimated 100,000⁻300,000 individuals yearly in western Africa and frequently causes lethal disease. Currently, no LASV-specific antivirals or vaccines are commercially available for prevention or treatment of Lassa fever, the disease caused by LASV. The development of medical countermeasure screening platforms is a crucial step to yield licensable products. Using reverse genetics, we generated a recombinant wild-type LASV (rLASV-WT) and a modified version thereof encoding a cleavable green fluorescent protein (GFP) as a reporter for rapid and q

Statistical considerations for a trial of Ebola virus disease therapeutics.

The 2014 West African outbreak of Ebola virus ravaged Liberia, Sierra Leone, and Guinea, causing hemorrhagic fever and death. The need to identify effective therapeutics was acute. The usual drug development paradigm of phase I, followed by phase II, and then phase III trials would take too long. These and other factors led to the design of a clinical trial of Ebola virus disease therapeutics that differs from more conventional clinical trial designs. This article describes the Ebola virus disease medical countermeasures trial design and the thinking behind it.

Poxvirus Epidemiology.

Illnesses caused by poxviruses have plagued humanity for millennia. The most notable are caused by viruses comprising the orthopoxvirus genus. Smallpox (caused by Variola virus) became the first eradicated disease in 1980. Devotion to smallpox eradication required significant orthopoxvirus research and discovery including vaccine and medical countermeasure development. Here we describe historical and current poxvirus epidemiology, with particular emphasis on orthopoxviruses including Monkeypox virus and Vaccinia virus.

Prophylactically Feeding Manganese to Drosophila Confers Sex-Specific Protection from Acute Ionizing Radiation Independent of MnSOD2 Levels.

Ionizing radiation is a health threat to many, including warfighters, radiological emergency responders, radiotherapy patients, and astronauts. Despite this, no FDA-approved prophylactic medical countermeasures exist to attenuate the symptoms that occur from radiation exposure. Manganese has recently been shown to be critical for radioresistance in a wide range of organisms. In this study, we designed a stringent feeding method to test the prophylactic effects of dietary manganese on Drosophila's lifespan before exposure to acute irradiation. We found that male flies have substantially lower r…

Natural History of Aerosol Exposure with Marburg Virus in Rhesus Macaques.

Marburg virus causes severe and often lethal viral disease in humans, and there are currently no Food and Drug Administration (FDA) approved medical countermeasures. The sporadic occurrence of Marburg outbreaks does not allow for evaluation of countermeasures in humans, so therapeutic and vaccine candidates can only be approved through the FDA animal rule-a mechanism requiring well-characterized animal models in which efficacy would be evaluated. Here, we describe a natural history study where rhesus macaques were surgically implanted with telemetry devices and central venous catheters prior t…

Management of corneal injuries in spaceflight and recommendations for planetary missions.

In February 1968, NASA purchased 400 antigravity ballpoint pens from the Fisher Pen Company for the Apollo Program to prevent potential harm to astronauts and equipment. Mechanical pencils previously used in microgravity posed risks like eye injuries from floating fragments penetrating the cornea. The cornea is vulnerable to abrasions, perforations, and chemical burns in such environments, affecting crewmembers aboard the International Space Station (ISS). While they undergo extensive training for emergency situations, there are inherent complexities when faced with eye injuries. In this chall

Space exploration and lifestyle medicine: a narrative review of its implications for astronaut health and remote Earth-based environments.

BACKGROUND: Space exploration, especially long-duration missions such as those to Mars, presents unique and significant challenges to astronaut health. Space medicine, which focuses on maintaining health in extreme environments without access to definitive medical care, emphasizes preventive measures. Lifestyle medicine (LM), grounded in six pillars such as healthy nutrition, regular physical activity, restorative sleep, stress management, positive social connections, and avoidance of risky substances, has proven effective for disease prevention on Earth. However, its application to spacefligh

Modeling optimal deployment strategies for Nipah vaccines and monoclonal antibodies.

Nipah virus (NiV) is endemic in Pteropus bats across South and Southeast Asia. Spillover into human populations results in usually fatal disease with the potential for human-to-human spread, which has led to NiV being classified by WHO as a priority pathogen. NiV vaccines and monoclonal antibodies (mAbs) are in development. However, it remains unclear how these medical countermeasures could be used most effectively. Here, we consider three different outbreak scenarios: limited, short-lived outbreaks as seen almost annually in Bangladesh and India; livestock amplified outbreaks as seen in Malay…

Fatal pneumococcal meningitis: failure of acute care focus of medical practice.

A 27-year-old soldier with a childhood history of splenectomy died as a result of pneumococcal meningitis. There was no documentation of pneumococcal vaccine in his 8-year military medical record. In light of the soldier's 58 documented encounters with the military medical system, this fatal case illustrates the health care system's failure because of its focus on acute care medicine. Health care that addresses only the medical management of acute illnesses and injuries allows this type of fatal case to occur, because preventive measures are ignored. Comprehensive health care must focus on wel

Biodefense and emergency use authorization: different originations, purposes, and evolutionary paths of institutions in the United States and South Korea.

BACKGROUND: Emergency-use-authorization (EUA) is the representative biodefense policy that allows the use of unlicensed medical countermeasures or off-label use of approved medical countermeasures in response to public health emergencies. This article aims to determine why the EUA policies of the United States and South Korea produced drastically different outcomes during the COVID-19 pandemic, and how these outcomes were determined by the originations and evolutionary paths of the two policies. METHOD: Historical institutionalism (HI) explains institutional changes-that is, how the instituti…
