Remote Patient Monitoring Adoption for Hypertension Management Among Medicare Beneficiaries.

IMPORTANCE: Remote patient monitoring (RPM), including self-measured blood pressure monitoring with clinician review and telehealth-supported feedback, can support hypertension management. However, RPM use and related care continuity after switching from Medicare fee-for-service (FFS) to Medicare Advantage (MA) remain unclear. OBJECTIVE: To compare RPM adoption, clinician continuity, and hypertension-related acute care utilization among beneficiaries who remained in Medicare FFS vs switched to MA plans, categorized as value-based contract (VBC) proxy or non-VBC. DESIGN, SETTING, AND PARTICIP

Identifying Dual-Eligible Beneficiaries With Long-Term Services and Supports Use in Medicare Enrollment Data.

Identifying dual-eligible beneficiaries who use Medicaid-funded long-term services and supports (LTSS) is difficult, hindering efforts to monitor use and improve quality. We demonstrate a strategy that uses only Medicare data to identify nursing home (NH) users and Medicaid home-and-community-based-service (HCBS) waiver participants by leveraging the fact that these groups exclusively qualify for zero cost-sharing in Part D. In a multistate cohort of low-income older adults, we identified Part D LTSS beneficiaries (dual-eligible beneficiaries with zero Part D cost-sharing) in Medicare enrollme

Comparing Medicare Fee-for-Service Beneficiaries with ESKD Who Switched to Medicare Advantage versus Remained in Traditional Medicare.

Patients choosing Medicare Advantage versus Medicare fee-for-service (FFS) differ with respect to race, socioeconomic status, and burden of disease. However, it is unclear whether these differences also occur among patients with kidney failure, who were newly allowed to switch to Medicare Advantage after the 21st Century Cares Act. We used data from the United States Renal Data System to examine differences in characteristics of dialysis patients and kidney transplant recipients who switched from FFS to Medicare Advantage compared with those who stayed with FFS in 2021, the first year such swi

Group-based trajectory modeling to identify adherence patterns for direct oral anticoagulants in Medicare beneficiaries with atrial fibrillation: a real-world study on medication adherence.

BACKGROUND: Suboptimal adherence to direct oral anticoagulants (DOACs) among atrial fibrillation (AF) patients remains currently a major concern due to the increased risk of cardiac and thromboembolic events. AIM: To identify longitudinal distinct trajectories of DOAC adherence and sociodemographic and clinical factors associated with each trajectory. METHOD: Patients with AF who were prescribed with DOAC from July 2016-December 2017 were identified among patients enrolled in the Medicare Advantage Plan. Patients were followed up for a year after the index date to calculate the monthly propo

Artificial intelligence-enabled causal estimate of Medicare drug plan integration in cancer care: A doubly robust machine learning instrumental variable analysis.

BACKGROUND: Artificial intelligence (AI) methods are increasingly used to strengthen policy evaluation in managed care pharmacy. Among Medicare beneficiaries with cancer, which is one of the most clinically complex and costly populations, prescription drug coverage is obtained through either integrated Medicare Advantage Prescription Drug plans (MA-PDs) or stand-alone Prescription Drug Plans (PDPs). However, causal evidence of plans' impact remains limited because of nonrandom enrollment. OBJECTIVE: To apply an AI-enabled causal inference framework to estimate the causal effect of PDP vs MA-P

Prior Authorization Requirements in Medicare Advantage and County Social Vulnerability.

OBJECTIVE: To assess whether Medicare Advantage (MA) beneficiaries in socially vulnerable counties face more restrictive prior authorization (PA) requirements. STUDY DESIGN: Cross-sectional analysis of 2945 US counties in 2022. METHODS: The primary outcome was the mean percentage of Medicare-covered service categories requiring PA across all MA plans in a county, adjusted for service mix and plan enrollment and derived from the CMS MA benefit, landscape, and enrollment files. Secondary outcomes included county-level PA rates for selected service categories, the number of plans with more vs l

Assessing The Inclusion Of Federally Qualified Health Centers In Medicare Advantage Networks.

Medicare Advantage (MA) disproportionately enrolls low-income and racial and ethnic minority populations that may benefit from access to federally qualified health centers (FQHCs). Using 2022 FQHC provider and MA network data, we examined the inclusion of FQHCs in MA provider networks. On average, these networks included 57 percent of FQHCs in their service areas, but 30 percent of contracts had narrow FQHC networks, defined as including fewer than one-quarter of available FQHCs. Asian/Pacific Islander, Hispanic, and Black beneficiaries were more likely to be enrolled in a narrow-network contr

Availability and Pricing of Drugs Prescribed by Otolaryngologists Through a Direct-to-Consumer Pharmacy: Medicare Savings Implications.

High prescription drug prices can financially strain patients and insurers, with substantial clinical repercussions. In recent years, direct-to-consumer (DTC) pharmacies have emerged as potentially lower-cost alternatives for patients to fill prescriptions. We evaluated whether drugs commonly prescribed by otolaryngologists were available at a national DTC pharmacy (Mark Cuban Cost Plus Drug Company [MCCPDC]) and estimated potential Medicare savings from DTC pricing. We identified drugs and prices paid by Medicare Part D plans using the 2021 Drug Spending Dashboard. Our analysis included 16 ge

Quality of Treatment Selection for Medicare Beneficiaries With Cancer.

PURPOSE: The Medicare part D Low-Income Subsidy (LIS) improves access to oral cancer drugs, but provides no assistance for clinician-administered/part B drugs. This analysis assessed the association between LIS participation and receipt of optimal cancer treatment. METHODS: We investigated initial systemic therapy using SEER-Medicare data (2015-2017) and National Comprehensive Cancer Network (NCCN) Evidence Blocks (EB) as the standard for treatment recommendations. We included cancer clinical scenarios wherein (1) ≥one treatment was optimal (higher efficacy and safety scores) versus other tre

Contraceptive Use Among Traditional Medicare And Medicare Advantage Enrollees.

Medicare is the primary source of health insurance coverage for reproductive-age people with Social Security Disability Insurance. However, Medicare does not require contraceptive coverage for pregnancy prevention, and little is known about contraceptive use in traditional Medicare and Medicare Advantage. We analyzed Medicare and Optum data to assess variations in contraceptive use and methods used by traditional Medicare and Medicare Advantage enrollees, as well as among enrollees with and without noncontraceptive clinical indications. Clinically indicated contraceptives are used for reasons

Racial And Ethnic Disparities In Buprenorphine Receipt Among Medicare Beneficiaries, 2015-19.

We examined Medicare Part D claims from the period 2015-19 to identify state and national racial and ethnic disparities in buprenorphine receipt among Medicare disability beneficiaries with diagnosed opioid use disorder or opioid overdose. Racial and ethnic disparities in buprenorphine use remained persistently high during the study period, especially for Black beneficiaries, suggesting the need for targeted interventions and policies.

Factors Associated With Initial Treatment Choice, Engagement, and Discontinuation for Patients With Opioid Use Disorder.

OBJECTIVE: Pharmacotherapy for opioid use disorder is effective but underused from a clinical perspective, and average treatment duration is shorter than current recommendations. In this analysis, the authors examined factors associated with initiation of, engagement in, and duration of treatment among patients with opioid use disorder. METHODS: Using the OptumLabs Data Warehouse (a large, national, deidentified database of commercial or Medicare Advantage plan enrollees), the authors identified a sample of 204,225 patients with opioid use disorder between July 1, 2010, and April 1, 2019. Analysis of the Utilization and Out-of-Pocket Costs of Authorized Generics and Independent Generics Among Medicare Beneficiaries.

OBJECTIVE: To assess the utilization trends of authorized generics (AGs) within Medicare, evaluate differences in out-of-pocket (OOP) costs for Medicare beneficiaries between AGs and independent generics (IGs) when both were simultaneously available in the market, and estimate the adjusted mean difference in OOP costs between these generic types while controlling for potential confounders such as benefit phase and Medicare plan. STUDY SETTING AND DESIGN: This retrospective analysis examined Medicare Part D claims from 2012 to 2020, focusing on prescription-level OOP expenditures. Claims were

Consequences of Medicare Advantage for Beneficiaries and Politics: Revisiting The Delegated Welfare State.

The Delegated Welfare State (Morgan and Campbell 2011) explored the causes and possible consequences of the 2003 Medicare reform boosting private managed care plans in the delivery of Medicare benefits. In this article, the authors review scholarship on beneficiary experience (access, costs, outcomes) and political feedbacks arising from the delegated governance reform to evaluate whether predictions about consumer behavior and policy entrenchment have come true. They find that beneficiary experiences and satisfaction do not differ significantly between Medicare Advantage (MA) and traditional

A Medicare Physician Fee Schedule Analysis of Reimbursement Trends in Laryngology from 2000 to 2021.

OBJECTIVE: The purpose of this study is to characterize Medicare reimbursement trends for laryngology procedures over the last two decades. METHODS: This analysis used CMS' Physician Fee Schedule (PFS) Look-Up Tool to determine the reimbursement rate of 48 common laryngology procedures, which were divided into four groups based on their practice setting and clinical use: office-based, airway, voice disorders, and dysphagia. The PFS reports the physician service reimbursement for "facilities" and global reimbursement for "non-facilities". The annual reimbursement rate for each procedure was av

Medicare Advantage: A Disadvantage for Complex Cancer Surgery Patients.

PURPOSE: Nearly half of all Medicare beneficiaries are enrolled in privatized Medicare insurance plans (Medicare Advantage [MA]). Little comparative information is available about access, outcomes, and cost of inpatient cancer surgery between MA and Traditional Medicare (TM) beneficiaries. We set out to assess and compare access, postoperative outcomes, and estimated cost of inpatient cancer surgery among MA and TM beneficiaries. METHODS: Retrospective cohort analysis of MA or TM beneficiaries undergoing elective inpatient cancer surgery (for cancers located in lung, esophagus, stomach, pancr

The recent reversal of the growth trend in MRI: a harbinger of the future?

PURPOSE: Diagnostic imaging services have been repeatedly targeted as a source of excess health care expenditure. In particular, MRI is considered a high-tech and high-cost imaging service that saw rapid increases in utilization in the early 2000s. However, the most recent trends in the utilization of MR are not known. The aim of this study was to quantify trends in MR utilization overall and by body system from 1998 to 2010 in the Medicare population. METHODS: Medicare Part B data sets were obtained for 1998 to 2010 for all MR examinations performed in the Medicare population. Using Current
