Development of a pragmatic measure for evaluating and optimizing rapid response systems.

OBJECTIVES: Standard metrics for evaluating rapid response systems (RRSs) include cardiac and respiratory arrest rates. These events are rare in children; therefore, years of data are needed to evaluate the impact of RRSs with sufficient statistical power. We aimed to develop a valid, pragmatic measure for evaluating and optimizing RRSs over shorter periods of time. METHODS: We reviewed 724 medical emergency team and 56 code-blue team activations in a children's hospital between February 2010 and February 2011. We defined events resulting in ICU transfer and noninvasive ventilation, intubatio

Moving Toward a Population Level Measure of Person-Centered Contraceptive Need in the United States.

BACKGROUND: Health care systems should be designed to provide the care that people need and want; person-centered metrics are therefore critical data inputs to this process. MATERIALS AND METHODS: We used multiple national datasets to develop a more person-centered metric for estimating need for contraception, comparing results made using a conventional approach with this new approach. The conventional metric estimates the number of individuals who have a likely need for contraceptive services based on their sexual experience and presumed risk for pregnancy. The new metric estimates the numbe

Predicting semantic segmentation quality in laryngeal endoscopy images.

Endoscopy is a major tool for assessing the physiology of inner organs. Contemporary artificial intelligence methods are used to fully automatically label medical important classes on a pixel-by-pixel level. This so-called semantic segmentation is for example used to detect cancer tissue or to assess laryngeal physiology. However, due to the diversity of patients presenting, it is necessary to judge the segmentation quality. In this study, we present a fully automatic system to evaluate the segmentation performance in laryngeal endoscopy images. We showcase on glottal area segmentation that th

The design and development of a dashboard for improving sustainable healthy food choices.

Over the last decade, several digital tools have been designed to provide consumers with nutritional and environmental impact information about their food choices post-consumption. Many of these tools lack behavioral change modules, have low user engagement, and ignore inherent environmental nutrition trade-offs to stimulate dietary change. This study presents the design and development of a decision support system to enhance consumer health while meeting sustainability goals from a pre-consumption perspective. The proposed decision support system, Dashboard for Improving Sustainable Healthy (

Markerless head motion tracking and event-by-event correction in brain PET.

Objective.Head motion correction (MC) is an essential process in brain positron emission tomography (PET) imaging. We have used the Polaris Vicra, an optical hardware-based motion tracking (HMT) device, for PET head MC. However, this requires attachment of a marker to the subject's head. Markerless HMT (MLMT) methods are more convenient for clinical translation than HMT with external markers. In this study, we validated the United Imaging Healthcare motion tracking (UMT) MLMT system using phantom and human point source studies, and tested its effectiveness on eight18F-FPEB and four11C-LSN31721

Quantification of the aperture modulation in radiotherapy treatment plans.

BACKGROUND: Many international guidelines, including those from the American Association of Physicists in Medicine (AAPM), emphasize the importance of quantifying plan modulation and recommend clearly documenting the modulation amount and range of radiotherapy treatment plans. However, there is no standardization or consensus on how to quantify plan modulation, partly due to the limitations of existing metrics. PURPOSE: This study introduces two novel metrics to quantify the modulation of treatment plans: the plan aperture modulation (PAM) and the modulation factor (MF). The aim of these metr

PsT1: A Low-Cost Optical Simulator for Psychomotor Skills Training in Neuroendoscopy.

BACKGROUND: Well-developed psychomotor skills are important for competence in minimally invasive surgery. Neuroendoscopy is no exception, and adaptation to different visual perspectives and careful handling of the surgical instruments are mandatory. Few training systems, however, focus on developing psychomotor skills for neuroendoscopy. Here, we introduce a new training system called PsT1 that provides visual feedback via the use of simple optics that emulate the endoscope at 0° and 30°. Time and error metrics are generated automatically with integrated software to ensure objective assessment

Strategies to Reduce Failure to Rescue after Cardiac Surgery.

Failure to rescue (FTR), defined as mortality after a postoperative complication, is now a central quality metric in cardiac surgery, where high-acuity patients and resource-intensive care increase vulnerability to deterioration. Although complications are common, survival depends on timely recognition, effective escalation, and coordinated multidisciplinary management. This article synthesizes current evidence on the epidemiology, mechanisms, and system-level drivers of FTR, highlighting how patient, provider, and institutional factors influence rescue success. We outline practical, evidence-

A free-fall determination of the newtonian constant of gravity.

Recent determinations of the Newtonian constant of gravity have produced values that differ by nearly 40 times their individual error estimates (more than 0.5%). In an attempt to help resolve this situation, an experiment that uses the gravity field of a one-half metric ton source mass to perturb the trajectory of a free-falling mass and laser interferometry to track the falling object was performed. This experiment does not suspend the test mass from a support system. It is therefore free of many systematic errors associated with supports. The measured value was G = (6.6873 +/- 0. 0094) x 10(

Advancing Intensive Care Unit Early Mobility Measurement, Practice and Outcomes: The Out-of-Bed Index.

ICU early mobility improves some patient outcomes, yet practice adoption remains inconsistent owing in part to variable terminology, documentation and measurement. This article describes a decade-long learning health system approach to the study and practice of ICU early mobility measurement, leading to the development of the Out-of-Bed Mobility Eligible Index (OOB-E Index), an automated metric quantifying the frequency of out-of-bed events per mobility-eligible ICU day. Integrated into a mobility dashboard alongside other key metrics, the OOB-E Index supports supports standardized assessment,

Navigating fairness in artificial intelligence-based prediction models: theoretical constructs and practical applications.

Artificial intelligence (AI)-based prediction models, including risk scoring systems and decision support systems, are being increasingly adopted in health care. Addressing AI fairness is essential to fighting health disparities and ensuring equitable model performance and patient outcomes. However, numerous and conflicting definitions of fairness complicate this effort. In this Viewpoint, we aim to support the transition of AI fairness from theory to practice using appropriate fairness metrics. We assess the relation of 27 fairness definitions identified in the literature to the model's inten

Voice Pathology Detection Using Modulation Spectrum-Optimized Metrics.

There exist many acoustic parameters employed for pathological assessment tasks, which have served as tools for clinicians to distinguish between normophonic and pathological voices. However, many of these parameters require an appropriate tuning in order to maximize its efficiency. In this work, a group of new and already proposed modulation spectrum (MS) metrics are optimized considering different time and frequency ranges pursuing the maximization of efficiency for the detection of pathological voices. The optimization of the metrics is performed simultaneously in two different voice databa

Refining the concept of hydrological connectivity for large floodplain systems: Framework and implications for eco-environmental assessments.

Recent years, the hydrological connectivity has gained popularity in various research fields, however, its definition and threshold effects at a system scale have not received adequate attention. The current research proposes a promising framework to refine the concept of surface hydrological connectivity by combining hydrodynamic modeling experiments, threshold effects and geostatistical connectivity analysis, exemplified by the flood-pulse-influenced Poyang Lake floodplain system (China). To enhance the inherent linkage between hydrological connectivity and eco-environments, total connectivi

A framework for addressing Alzheimer's disease: Without a frame, the work has no aim.

Confronting Alzheimer's disease (AD) involves patients, healthcare professionals, supportive services, caregivers, and government agencies interacting along a continuum from initial awareness to diagnosis, treatment, support, and care. This complex scope presents a challenge for health system transformation supporting individuals at risk for, or diagnosed with, AD. The AD systems preparedness framework was developed to help health systems identify specific opportunities to implement and evaluate focused improvement programs. The framework is purposely flexible to permit local adaptation across
