PURPOSE OF REVIEW: This review summarizes the evidence describing functional mitral stenosis (MS) following surgical mitral valve repair.
RECENT FINDINGS: Functional MS is an evolving concept and has been implicated in the repair of functional and organic mitral regurgitation. Mitral valve replacement using a cryopreserved mitral homograft was performed in a 49 year old patient with calcified mitral stenosis. The surgical technique is described. The postoperative course was uneventful. Transoesophageal echo performed 4 months later showed a normal function of the mitral homograft.

From June 1978 to Dec. 1987, 35 cases of MVR were performed. All of these patients had closed mitral valvocommissurotomy before. Our experiences show: 1. The main causes of high mortality and morbidity are pericardial adhesion and blood oozing, poor exposure of mitral valve, and prolonged operative time. Therefore, it is wise not to do too much dissection, and to use biatrial incision. With these techniques, good results were obtained. Closed mitral commissurotomy (CMC) was performed at the National Heart Institute in 303 patients (73% women, 27% men; mean age 40 years) with acquired isolated mitral stenosis between 1954 and 1980. The average mean mitral valve gradient decreased from 14.2 +/- 0.4 to 5.3 +/- 0.4 mm Hg (p less than 0.001), and mitral valve area index increased from 0.7 +/- 0.03 to 1.4 +/- 0.9 cm2/m2 (p less than 0.001). We report a surgical case of active infective prosthetic biologic mitral valve endocarditis with hemodynamic deterioration with increasing obstruction of mitral valve. The patient developed rapid hemodynamic deterioration, therefore, we performed emergent re-do mitral valve replacement using SJM prosthetic valve in the setting of supra-annular position. OBJECTIVE: To evaluate the therapeutic effects of mitral valve repair for the treatment of the anterior leaflet prolapse of mitral valve.

A 56-year-old woman was underwent mitral valve repair for prolapse of the posterior mitral leaflet. Intraoperative transesophageal echocardiography (TEE) showed systolic anterior motion (SAM) of the mitral valve at the weaning from cardiopulmonary bypass (CPB). Sliding technique was easily performed at the second pump run. Intraoperative TEE demonstrated no SAM or residual mitral regurgitation after the second pump run.
