Since its original description by Barlow over a quarter of a century ago, mitral valve prolapse has become one of the commonest heart diseases around the world. It is commoner in women than men, in thinner than heavier subjects, and in younger than older persons. A unifying concept of a valvular-ventricular disproportion serves to explain the various conditions in which mitral valve prolapse occurs. The etiology of chest pain which is the most frequent symptom that brings the patient with mitral valve prolapse to a physician is multifactorial. Diagnosis of mitral valve prolapse is based on cli…

Mitral Valve Prolapse does not cause chest pain or other symptoms and is extremely unlikely to give rise to complications. In practice it is difficult to differentiate a 'prolapsing' valve from one that is 'normal' no matter what methods are used. Since diagnosing 'heart disease' is itself a well known cause of ill health, the most stringent criteria must be adopted before the mitral valve is labelled as being abnormal. The need for the diagnosis at all is, in most cases, doubtful.

In asymptomatic or symptomatic patients with an audible click and late systolic murmur, mitral valve prolapse can be assumed to be present, the pathologic-anatomical substrate of which is characterized by myxomatous changes in the mitral valve leaflets and collagen degeneration of the chordae tendineae. The conclusion that all persons with a systolic click have a diseased mitral valve and are at risk of complications is probably excessive. In the presence of an unequivocally-audible click and/or late systolic murmur, an echocardiogram for confirmation of the diagnosis is not necessary. If the …

Mitral valve prolapse is extremely common in children. It is diagnosed by the presence of a nonejection click with or without an associated murmur. In isolated mitral valve prolapse, the prognosis is excellent, but regular office visits are necessary for cardiac status review and infective endocarditis prophylaxis. Patients with significant dysrhythmias represent a small subset with an unknown long-term prognosis.

Mitral valve prolapse, usually a benign condition, is aggravated in 15% of the cases by one or the other of five different complications. Mitral regurgitation may develop progressively or abruptly following rupture of the chordae tendinae and requiring prompt surgical repair. Bacterial endocarditis has been observed in 2.9% of the cases, hence the need for preventive antibiotic therapy prior to dental treatments or surgical operations in patients with holosystolic or end-systolic mitral murmur. Among arrhythmias, only ventricular extrasystoles (which are frequent and most often occur in pairs …

Of 46 female agoraphobic patients, three had definite and four had probable mitral valve prolapse--a total prevalence of only 15%. There were no significant differences between patients with mitral valve prolapse and the rest of the sample on demographic and clinical variables, nor did the groups differ significantly on measures of psychological symptoms assessed before and after treatment. These negative findings support recent reports indicating that agoraphobia associated with mitral valve prolapse may be indistinguishable from agoraphobia without mitral valve prolapse.

A contemporary "in" diagnosis in the field of cardiology is the mitral valve prolapse syndrome. This abnormality is evident in populations ranging from very young children to older adults. It is being recognized with increasing frequency among college and university students. In many cases, the syndrome is of limited consequence, while in others, it may be life threatening. This paper (1) defines what it is, (2) reports on its occurrence, (3) depicts some relationships with related health entities and (4) discusses certain implications for health professionals and heart health education.
