Intraindividual comparative analysis of the visual performance after cataract surgery with implantation of a trifocal and a bifocal intraocular lens.

Presbyopia-treating intraocular lenses (IOLs) are now widely available and offer high predictability at all distances, better visual outcomes, and high patient satisfaction. In most patients, the same multifocal IOL is implanted bilaterally. In certain circumstances, such as during complex surgeries, it is important to be prepared with a backup plan to maintain surgical expectations. We report 3 patients who had femtosecond laser-assisted cataract surgeries in both eyes. The complicated eye had implantation of a bifocal IOL and the fellow eye of a trifocal IOL. This intraindividual analysis sh

Criteria for premium intraocular lens patient selection.

PURPOSE OF REVIEW: To discuss available premium intraocular lenses (IOLs), patient selection, and important considerations for each premium IOL. RECENT FINDINGS: We review important topics and considerations for premium IOL selection: specifically, toric, extended depth of focus (EDOF), multifocal/trifocal, light adjustable lenses (LALs), and small aperture IOLs. Toric lenses are an excellent option for patients with astigmatism. However, to achieve optimal patient satisfaction, it is critical to account for the ATR astigmatism contribution from the posterior cornea and high angle alphas. Add

Presumed nonadaptation to a multifocal intraocular lens masking visual loss from a pituitary tumor.

PURPOSE: To report a case of delayed diagnosis of a pituitary tumor impacting vision. The patient had undergone bilateral cataract surgery with multifocal lenses, and even an intraocular lens (IOL) exchange for persistent visual dissatisfaction, which was ultimately due to the tumor and not the visually significant cataracts or the intra-ocular lens. METHODS: Single case report and narrative review. RESULTS: An 81-year-old male patient presented with visually significant nuclear sclerotic cataracts that were consistent with his chief complaint of decreased visual acuity and colors not being

Extended Depth-of-Field Intraocular Lenses: An Update.

Extended depth-of-focus (EDOF) is a new intraocular lens (IOL) technology in the treatment of presbyopia. In contrast to multifocal (MF) IOLs, EDOF lenses create a single elongated focal point, rather than several foci, to enhance depth of focus. In this way, EDOF IOLs aim to reduce photic phenomena, glare, and halos, which have been reported in MF IOLs. A potential disadvantage is a decrease of retinal image quality if the amount of the aberrations is excessively increased. Frequently, EDOF IOLs are combined with MF optical designs; for this reason, EDOF IOLs are commonly a subject of confusi

Modified ab externo 2-point pierce technique for scleral fixation of single-piece intraocular lenses.

Foldable acrylic lenses are the most extensively used 'in the bag' monofocal or multifocal intraocular lenses (IOLs) in cataract surgery, toric lenses in astigmatism, and refractive lens exchange surgeries. When subluxation or dislocation of these lenses is encountered, they must be explanted as they cannot be fixed with conventional scleral fixation techniques. The 'pierce' technique is a novel way of fixing these lenses to the sclera, wherein a 9-0/10-0 polypropylene suture with a straight needle is passed through the IOL at the haptic-optic junction. The ends of the threads are sutured unde…

Multiple zone multifocal phase designs.

New multifocal phase designs aiming at expanding depth of focus in the presbyopic eye are presented. The designs consist of multiple radial or angular zones of different powers or of combined low- and high-order aberrations. Multifocal performance was evaluated in terms of the dioptric range for which the optical quality is above an appropriate threshold, as well as in terms of the area under the through-focus optical quality curves. For varying optical power designs optimal through-focus performance was found for a maximum of three to four zones. Furthermore adding more zones decreased the op

[Current state of knowledge on special lenses].

Patients undergoing lens surgery nowadays have the option to choose between different intraocular lens (IOL) options depending on the ocular situation and personal preferences. For example, it is possible to implant a toric IOL to compensate for extensive corneal distortion or a multifocal IOL to have good visual acuity at various distances. The high level of competition in the lens market leads to fast development of new lens models giving patients the advantage to choose between a variety of options. This review article presents the most commonly used premium IOL options namely, aspheric, to

Basic Optics Underlying Current Intraocular Lenses.

As surgeries using multifocal intraocular lenses (IOLs) to correct both cataracts and presbyopia have become common, it has become essential for clinicians to understand their basic optical characteristics to select the optimal lens for their patients. However, there are relatively few review articles on optics that are directly useful to clinicians who perform surgery on patients. In this paper, we systematically review fundamental concepts, from the basic properties of light, geometric optics, and Gaussian approximation to lens performance metrics like the point spread function and modulatio…

Trifocal versus extended depth of focus (EDOF) intraocular lenses after cataract extraction.

BACKGROUND: Cataract, defined as an opacity of the lens in one or both eyes, is the leading cause of blindness worldwide. Cataract may initially be treated with new spectacles, but often surgery is required, which involves removing the cataract and placing a new artificial lens, usually made from hydrophobic acrylic. Recent advancements in intraocular lens (IOL) technology have led to the emergence of a diverse array of implantable lenses that aim to minimise spectacle dependence at all distances (near, intermediate, and distance). To assess the relative merits of these lenses, measurements of

Toric intraocular lenses: historical overview, patient selection, IOL calculation, surgical techniques, clinical outcomes, and complications.

We present an overview of currently available toric intraocular lenses (IOLs) and multifocal toric IOLs. Relevant patient selection criteria, IOL calculation issues, and surgical techniques for IOL implantation are discussed. Clinical outcomes including uncorrected visual acuity, residual refractive astigmatism, and spectacle independency, which have been reported for both toric IOLs and multifocal toric IOLs, are reviewed. The incidence of misalignment, the most important complication of toric IOLs, is determined. Finally, future developments in the field of toric IOLs are discussed.
