Management of mycoses in daily practice.

The guideline recommendations, albeit founded on thorough reviews of clinically relevant literature data, are often not immediately adaptable to everyday life. Considering the marked heterogeneity of superficial mycoses, each of them requires specific management in a real life context; in all cases diagnostic confirmation is required with microscopic and culture examination. In tinea capitis oral therapy is necessary (minimum six weeks) and should be continued until clinical and, above all, mycological healing. In cases of tinea corporis, cruris or pedis, it may be necessary to associate oral …

Approach to the diagnosis of the endemic mycoses.

The endemic mycoses are often overlooked in the evaluation of community-acquired pneumonia, and appropriate testing is not performed until the patient has failed to improve on antibacterial therapy. Antigen detection and fungal serology may be helpful in diagnosis of endemic mycoses as causes for pneumonia. Guidelines for recognition and evaluation of endemic mycoses as causes for community-acquired pneumonia are proposed. Performance of antigen testing on bronchial washings or lavage fluid may improve the sensitivity for diagnosis over microscopic examination and the speed of diagnosis over c…

The trend of imported mycoses in Japan.

Pathogenic fungi that are non-native in Japan are highly virulent and present a significant health hazard to persons in the environment into which they are introduced. Little is known, however, about the precise trend of infection by imported mycoses in Japan. To clarify this issue, all available cases were collected through a search of MEDLINE and Japana Centra Revuo Medicine and analyzed. Also included in this analysis were cases not reported in the literature for which the authors provided identification of the fungi, diagnosis, or treatment. The analysis revealed that, for three diseases, …

Respiratory and systemic mycoses: an overview.

Respiratory and systemic mycoses are globally emerging as a problem of increasing importance in infectious diseases. This is attributed to the growing population of immunocompromised patients due to epidemic outbreak of AIDS or to other factors such as use of immunosuppressive drugs in recipients of organ transplantation. The available evidence has unequivocally established the endemic occurrence of blastomycosis, histoplasmosis and penicilliosis mameffei in India. In fact, pencilliosis marneffei has emerged as a major endemic mycosis of AIDS patients in Southeast Asia. It has manifestations s…

[Mycoses of the paranasal sinus system].

BACKGROUND: During the last few years, in increasing number of fungal infections in the paranasal sinus system has been observed. Aspergillus species as well as mucor and candida albicans are especially responsible for these mycoses. PATIENTS: Twenty-seven cases of a fungal infection of the paranasal sinuses were observed between 1986 and 1997. The majority of the patients showed a chronic noninvasive form with affection of the maxillary sinus. Other forms (fulminant invasive form, chronic invasive form, allergic fungal sinusitis) are described. Typical features for fungal infection do exist …

The deep mycoses in HIV infection.

The deep mycoses are uncommon infections, usually acquired from the inhalation or ingestion of fungal spores, sometimes from the soil in areas of endemicity, such as in the Americas and south-east Asia, or from decaying vegetable matter. They are also seen in immunocompromised persons and, increasingly, in HIV-infected persons. Respiratory involvement is frequent, with granuloma formation, and mucocutaneous involvement may be seen. Oral lesions of the deep mycoses are typically chronic but non-specific, though nodular or ulcerative appearances are common. Person-to-person transmission is rare.…

[Metamphocin in the treatment of ocular mycoses].

Metamphocin (methyl ether of amphotericin B), a new antifungal polyene antibiotic was used in the treatment of ocular mycoses. The procedure and results of its use are described. The clinical trials included 8 patients with keratomycosis and 2 patients with fungal endophthalmitis. Metamphocin was administered as drop-like intravenous injections in combination with local instillation of ketoconazole and dexamethasol. The clinical trials showed that metamphocin was an efficient antifungal drug useful in the treatment of ocular mycoses. Metamphocin proved to be less toxic than amphotericin B.

[Lamisil in the therapy of foot mycoses].

A new antimycotic drug lamisil (Sandoz, Switzerland) was given as pills to 35 patients with pedal mycosis (23 of them suffered from onychomycosis). Pedal mycosis was cured for 2 weeks, onychomycosis for 12 weeks. In 2 cases the drug was discontinued because of gastrointestinal toxicity. By tolerance and efficacy, lamisil is superior to griseofulvin and nizoral.

Orofacial manifestations of the systemic mycoses.

Aspergillosis, cryptococcosis and zygomycosis (mucormycosis) are overall the most common systemic mycoses but histoplasmosis is particularly endemic in parts of central USA and other areas worldwide. Orofacial lesions caused by systemic mycoses have rarely been reported in the past though they have been recorded particularly in outdoor workers from geographic areas with a high prevalence of infection and occasionally in immunocompromised individuals. Increasing world-wide travel, and the dramatic increase in numbers of immunocompromised persons, especially those with human immunodeficiency vir…

Oral lesions in the systemic mycoses.

Oral lesions have rarely been reported in systemic mycoses, though over the past few years they have been recorded particularly in immunocompromised individuals. The dramatic increase in numbers of immunocompromised persons, especially those infected with human immunodeficiency virus, has almost certainly been responsible for the increase in reports of oral disease caused by systemic mycoses, particularly aspergillosis, cryptococcosis, and histoplasmosis. However, reports of coccidioidomycosis, blastomycosis, and paracoccidioidomycosis have, as yet, increased little in this population. Dentist…

[Imidazoles in the treatment of ocular mycoses].

The use of a new class of broad spectrum antifungal drugs i.e. imidazoles and in particular ketoconazole in treatment of severe affections of the eye such as mycoses is described. The clinical trials included 40 patients with various forms of mycosis: mycotic canaliculitis (6 patients), mycotic blepharitis (4 patients), mycotic conjunctivitis (7 patients), keratomycosis (17 patients) and mycotic endophthalmitis (6 patients). Ketoconazole was used in the form of tablets and instillations. The combined treatment included nonsteroid antiinflammatory drugs and antibacterial agents. The clinical tr…

Review of animal mycoses in Australia.

This review covers the available literature on the mycoses of animals in Australia since the last review published in 1967. Of the cutaneous infections, dermatophytoses have been recorded in a wide range of animals: cattle, horses, goats, pigs, sheep, cats, dogs, mice, guinea-pigs, rabbits, a lion, kangaroos, a camel, koalas and wallabies. These infections were caused by several species and varieties of the genera, Microsporum and Trichophyton. Eight agents of ringworms have been recorded in the horse. Two subcutaneous mycoses, phaeohyphomycosis and sporotrichosis have been reported. Phaeohyph…

The pathogenesis of opportunistic mycoses in man.

A variety of fungi capable of growth at around 37 degrees C are potential pathogens of suitably compromised animals, including man. While awareness of diseases by these opportunistic fungi is increasing, details of their pathogenesis are largely unknown, although ability of the host to inhibit spore germination appears to be central to resistance to infection.
