Case Report: Acute Transverse Myelitis after Zika Virus Infection.

We report here one case of Zika virus (ZIKV) infection associated with auto-immunity directed against the central nervous system in a Brazilian woman who developed acute transverse myelitis 9 days after recovery from an acute episode of fever with generalized erythema. Imaging of the spinal cord showed an elongated area on the T1-T10 level with gadolinium uptake. The diagnostic of the ZIKV infection was confirmed by cerebrospinal fluid and serum analysis. This patient had serum positivity for autoantibodies against myelin oligodendrocyte glycoprotein (MOG), a specific antibody against the myel

Evaluation of idiopathic transverse myelitis revealing specific myelopathy diagnoses.

OBJECTIVE: To evaluate specific myelopathy diagnoses made in patients with suspected idiopathic transverse myelitis (ITM). METHODS: A total of 226 patients 18 years and older were referred to Mayo Clinic Neurology for suspected ITM from December 1, 2010, to December 31, 2015. Electronic medical records were reviewed for detailed clinical presentation and course, laboratory and electrophysiologic investigations, and neuroimaging to determine the etiology. Current diagnostic criteria for ITM and alternative myelopathy diagnoses were applied. All cases where any discrepancy was suspected from th

Neuromyelitis optica spectrum disorders: beyond longitudinally extensive transverse myelitis.

AIM: To describe the neuroradiological features and their prevalence in patients with neuromyelitis optica (NMO). MATERIALS AND METHODS: Two neuroradiologists independently reviewed 35 spinal cord and 37 brain MRI studies from patients with NMO. The examinations were analysed for the presence of lesion, topography, enhancement, and brain lesions suggestive of multiple sclerosis and/or NMO. RESULTS: Seventy percent of the spinal cord lesions involved over three or more vertebral segments. Seventy-eight percent of brain scans were abnormal, and the most prevalent findings were non-specific foc

Idiopathic Acute Transverse Myelitis in Children: A Retrospective Series.

OBJECTIVE: Acute transverse myelitis (ATM) is a focal inflammatory disorder of the spinal cord, resulting in motor, sensory, and autonomic dysfunction. In this study, we delineate the clinical manifestations, neuroimaging characteristics, and outcome-associated risk factors in children with idiopathic ATM. METHODS: We retrospectively reviewed the medical charts and neuroimages in nine children aged younger than 18 years diagnosed with ATM between January 2006 and August 2014. RESULTS: The mean onset age was 5 years and 9 months. Infectious prodromes were observed in six patients. Leg weaknes

Idiopathic aquaporin-4 antibody negative longitudinally extensive transverse myelitis.

BACKGROUND: Longitudinally extensive transverse myelitis (LETM) is a characteristic manifestation of neuromyelitis optica (NMO). However, not all patients with LETM are positive for aquaporin-4 (AQP4) antibodies. We evaluated the characteristics of idiopathic isolated LETM negative for AQP4 antibodies. METHODS: From the National Cancer Center registry of inflammatory diseases of the central nervous system, patients with LETM as an initial manifestation and follow-up for at least two years were enrolled. Their medical records and MRIs were reviewed retrospectively. AQP4 antibody was confirmed

Acute transverse myelitis associated with human parvovirus b19 infection.

We describe severe acute transverse myelitis in a previously healthy 9-year-old boy in whom primary human parvovirus B19 infection was confirmed serologically and B19 DNA was detected in his serum and cerebrospinal fluid. Besides erythema infectiosum, parvovirus B19 infection has various clinical manifestations; however, central nervous system involvement is rare. Even more unusual is parvovirus B19-associated acute transverse myelitis.

Long-term follow-up of acute partial transverse myelitis.

BACKGROUND: Acute partial transverse myelitis (APTM) may be the first clinical symptom of multiple sclerosis (MS) or may remain a monophasic event. OBJECTIVES: To evaluate the risk of conversion to MS and long-term disability, and to determine prognosis factors for disability. DESIGN: We identified patients with no previous history of neurological disease who experienced APTM between January 1998 and December 2005 and were followed up at 3 university hospitals in France. Data on the patients' demographics and clinical states during follow-up, as well as data on cerebrospinal fluid (CSF) anal

Transverse myelitis and polymyositis associated with antiphospholipid antibody syndrome.

Antiphospholipid antibody syndrome (APS) has been widely recognized to be associated with various neurological complications. In addition to the classical notion of APS as a thrombotic disorder, APS has been suggested to be an autoinflammatory disease as well. We present a previously healthy 46-year-old man who concurrently developed transverse myelitis and polymyositis whose laboratory studies were significant for the elevated antiphospholipid antibodies such as anti-cardiolipin (CL)/beta2-glycoprotein I (beta 2GPI) antibody. This report further enhances the recognized clinical phenotypes of

Idiopathic recurrent transverse myelitis with syringomyelia: a case report.

Idiopathic transverse myelitis is a monophasic demyelinating disease of the central nervous system (CNS) and its recurrence is a rare entity. Existence of syringomyelia in the spinal cord in inflammatory CNS disease is not usually encountered. This case study describes a 45-year-old man who was admitted with features of a cervical demyelinating disease. The history of the patient indicated that this could be a recurring attack. Cervical MRI and laboratory examination of the patient showed evidence of recurrent transverse myelitis. The patient was managed with medical therapy. The follow-up cer

Acute transverse myelitis secondary to Salmonella paratyphi B infection.

Acute transverse myelitis (ATM) is a rare entity with an annual incidence of 4.6 per one million people. Its hallmark lesions are medullary perivenular inflammation and demyelination. The parainfectious form of ATM constitutes the most common subset of this entity. We report a woman with ATM secondary to Salmonella paratyphi B infection (paratyphoid fever) who was successfully treated with a specific parenteral antibiotic. Although, the association of Salmonella typhi infection and ATM is very rarely found in the literature, we believe this is the first report of ATM secondary to S. paratyphi

Acute transverse myelitis in a 7-month-old boy.

Acute transverse myelitis (ATM) is an acute intramedullary spinal cord disease characterized by an abrupt onset of rapidly progressive weakness of the extremities accompanied by a loss of sensation and sphincter control. Although ATM has an important place in childhood spinal cord disorders, its occurrence remains extremely uncommon in the infant population (0 to 12 months old). This report of a 7-month-old boy with ATM is followed by a discussion of the case and review of the literature regarding etiology, diagnosis, and prognosis for neurological recovery.

Acute transverse myelitis. A localized form of postinfectious encephalomyelitis.

We analysed the clinical, imaging, electrophysiological, laboratory findings, course and prognostic factors in 31 patients with acute transverse myelitis (20 men and 11 women; mean age, 30 years; range, 18-51 years). All patients were assessed for maximal clinical deficit 'deficit score'; pattern-shift visual, auditory and somatosensory evoked potentials were measured, CSF was examined, and neuroimaging of the spinal cord and brain (MRI or CT myelography) was carried out. The myelitis was preceded by febrile illness in 25 (81%) of the patients. The site of the lesion was cervical in 11 (36%),

MR imaging of acute transverse myelitis and AIDS myelopathy.

Acute transverse myelitis (ATM) is a well recognized clinical entity, though its etiology remains obscure. Only a few reports of magnetic resonance imaging of ATM appear in the literature. These reports describe conflicting findings with respect to the signal intensity of the spinal cord on long repetition time (TR) sequences. The purpose of this study is to present our experience with five cases of ATM in which long TR sequences demonstrated abnormal increase in signal intensity of the cord. Magnetic resonance imaging also demonstrated extension of abnormal cord signal intensity over at least
