Validation of DSD Interpreter, a Mobile Application for Point-of-Care Evaluation of Infants with Atypical Genitalia.

OBJECTIVES: To develop and validate Disorders of Sexual Development (DSD) Interpreter, a point-of-care mobile application, to guide the management of infants with atypical genitalia. METHODS: The DSD interpreter offers clinical guidance based on key clinical and diagnostic inputs. The retrospective validation involved comparing its guidance with that of two pediatric endocrinologists, a neonatologist, a pediatrician, and a pediatric trainee, against the final clinical diagnosis in 55 children with atypical genitalia (XX DSD in 21, XY DSD in 27, ovotesticular DSD in one, and DSD mimics in six)

"At Some Point, You Have to Cut the Cord": Physician Perceptions of NICU-to-PICU Transfers.

OBJECTIVE: Patients undergoing nonemergent neonatal intensive care unit (NICU) to pediatric ICU (PICU) transfer in the United States have complex medical needs and high relative mortality, but there are no best practices guiding these transfers. This qualitative study was designed to address this gap by characterizing facilitators and barriers to effective NICU-to-PICU transfer among physicians. PATIENTS AND METHODS: Semistructured individual interviews were conducted with pediatric intensivists and neonatologists purposively sampled from previous national surveys on transfer processes. Inter…

Survey of a nutrition management method for very low birthweight infants: Status before wide use of breast milk banks in Japan.

BACKGROUND: The importance of breast-feeding for very low birthweight (VLBW) infants has been pointed out. Some overseas studies suggested that the standardization of enteral nutrition (EN) leads to improved prognosis in VLBW infants. In Japan, however, physicians in charge of infants are responsible for making nutrition management decisions on an individual basis. We conducted an online survey to clarify the course of nutrition management of VLBW infants currently implemented in Japan. METHODS: We mailed a notice to 300 representative neonatologists throughout Japan requesting their particip…

Neonatal Point-of-Care Lung Ultrasonography: Techniques and Normal Findings.

Point-of-care lung ultrasonography (POC-LUS) has become a routine bedside tool for evaluating respiratory status in many neonatal intensive care units. This article walks neonatologists through the fundamentals of its application-probe selection, system setup, and image acquisition. Lung aeration can be assessed using a 6-zone scanning approach. We review the normal sonographic appearance of the neonatal lung, including lung sliding, A-lines, and the B-lines commonly seen during transitional physiology, and describe how M-mode confirms pleural motion when sliding is equivocal. Common pitfalls …

Reconciling ethical and legal aspects in neonatal intensive care.

During the last two decades there has been an enormous development in treatment possibilities for the extremely premature infants and the Neonatologists have to face in their daily practice many decisional problems and ethical, moral and legal dilemmas. These concern decisions to initiate or withhold treatment directly at birth, decision to withdrawn treatment with the possible consequence that the child will die. The debate between "sanctity" and "quality" of life, aggressive treatment in relation to discrimination toward the disabled, the principle of "beneficence" and the question of "propo…

Patent ductus arteriosus evaluation by serial echocardiography in preterm infants.

BACKGROUND: Patent ductus arteriosus (PDA) is associated with morbidity and mortality in premature neonates. METHODS: The effect of serial echocardiography performed by a neonatologist and early targeted medical PDA treatment was evaluated and compared to historical controls. One hundred ninety-two infants <1,500 g were included and 45 infants had a PDA. RESULTS: Serial echocardiography allowed significantly earlier identification and treatment of PDA versus awaiting the evolution of clinical signs. Severe intraventricular haemorrhage and ventilator days were significantly decreased in the s

The floppy infant: contribution of genetic and metabolic disorders.

The floppy infant syndrome is a well-recognized entity for pediatricians and neonatologists. The condition refers to an infant with generalized hypotonia presenting at birth or in early life. The diagnostic work up in many instances is often complex, and requires multidisciplinary assessment. Advances in genetics and neurosciences have lead to recognition of newer diagnostic entities (several congenital myopathies), and rapid molecular diagnosis is now possible for several conditions such as spinal muscular atrophy (SMA), congenital muscular dystrophies (CMD), several forms of congenital myopa…

Practical guidelines for screening and treatment of retinopathy of prematurity in Saudi Arabia.

Retinopathy of Prematurity (ROP) is one of the leading causes of bilateral blindness in childhood. Early detection and effective treatment can prevent blindness. Efficient and timely screening examination of the retina by an experienced ophthalmologist who deals with preterm neonates with ROP is the mainstay in the management of this disease. All neonatologists and pediatricians who care for these at-risk preterm neonates should also be aware of this timing. This practical guideline intends to provide guidance to ophthalmologists, neonatologists and allied health care professionals in Saudi Ar…

Risk factors at delivery and the need for skilled resuscitation.

A prospective study was devised to investigate how risk factors at delivery contribute to outcome at birth as measured by the type of resuscitation required ('skilled' or 'not skilled') and the Apgar score. In addition the number of unexpected 'crash calls' was recorded. In a 3-month sample, neonatologists attended 494 (38.6%) of 1279 deliveries. Of those attended, 188 (37.2%) required skilled resuscitation including 5 of 20 crash calls. Four risk factors at delivery were associated with skilled resuscitation: meconium, emergency caesarean section, prematurity and multiple births. Following th…

[Interpretation of the "European consensus guidelines on the management of respiratory distress syndrome: 2025"].

Neonatal respiratory distress syndrome (RDS) is a critical disease caused by pulmonary surfactant (PS) deficiency, leading to respiratory distress soon after birth. Comprehensive perinatal interventions can significantly improve outcomes. The core goal of RDS management is to maximize infant survival while minimizing lung injury. Based on the latest evidence, European neonatologists and obstetricians jointly developed the "European consensus guidelines on the management of respiratory distress syndrome: 2025". The guidelines provide evidence-based clinical recommendations for the management of…

Metabolic bone disease in the preterm infant: Current state and future directions.

Neonatal osteopenia is an important area of interest for neonatologists due to continuing increased survival of preterm infants. It can occur in high-risk infants such as preterm infants, infants on long-term diuretics or corticosteroids, and those with neuromuscular disorders. Complications such as rickets, pathological fractures, impaired respiratory function and poor growth in childhood can develop and may be the first clinical evidence of the condition. It is important for neonatologists managing such high-risk patients to regularly monitor biochemical markers for evidence of abnormal bone…
