The study aimed to investigate the safety, optimal biologic dose, and preliminary efficacy of topical neostigmine ophthalmic solution (TNOS) in myasthenia gravis (MG) patients with ptosis. A dose-escalation study comprising cohorts of three patients in each step was performed. The first cohort received 1.5 mg/mL TNOS and was evaluated for safety and efficacy. Then, the subsequent cohorts received the selected dose of TNOS according to the safety and efficacy protocols to determine the optimal biologic dose. Vital signs, visual acuity, intraocular pressure, pupillary size, slit-lamp examination…

We report the case of a previously healthy 27-year-old man who developed acute pulmonary edema shortly after receiving neostigmine-atropine for neuromuscular blockade reversal during appendectomy under general anesthesia. Before extubation, he developed severe hypoxemia, increased airway pressures, pink frothy tracheal secretions, and bilateral diffuse pulmonary infiltrates on chest radiography. Cardiogenic pulmonary edema, fluid overload, negative-pressure pulmonary edema, and perioperative hypersensitivity were considered unlikely based on the clinical findings, bedside cardiac ultrasound, l…

Delayed gastric emptying and autonomic neuropathy have been documented in patients with diabetes mellitus. Some medications used to treat delayed gastric emptying enhance release of acetylcholine from autonomic neurons to strengthen gastric contractions. Autonomic coordination among gastric regions may be altered in diabetes resulting in poor outcomes in response to prokinetic drugs. Fundus, antrum, and pylorus from STZ or control guinea pigs were treated with neostigmine to mimic release of acetylcholine from autonomic neurons by prokinetic agents. In diabetic animals, neostigmine-induced con…

PURPOSE: Rapid and complete reversal of residual neuromuscular blockade (rNMB) is essential for safe postoperative recovery in patients with myasthenia gravis (MG) undergoing video-assisted thoracoscopic (VATS) thymectomy. Although sugammadex provides a novel approach for reversing rocuronium-induced neuromuscular blockade, its efficacy in this high-risk population under routine-practice conditions-where reversal timing relies on clinical judgment-remains incompletely characterized. This multicenter randomized controlled trial (RCT) compared the efficacy of sugammadex versus neostigmine for re…

INTRODUCTION: Sugammadex has the steroid-encapsulating effect that reverses neuromuscular block induced by aminosteroid neuromuscular-blocking agents. Sugammadex can interact with other drugs that have the same steroidal structure with rocuronium, such as corticosteroids. Corticosteroids play a crucial role in the immunosuppression of kidney transplantation. The purpose of this study was to determine if there are any differences in grafted kidney function in recipients of kidney transplantation when sugammadex or neostigmine is given to the recipient.
METHODS: The study included 42 recipients…

The ability of edrophonium and neostigmine to antagonize nondepolarizing neuromuscular blockade produced by steady-state infusions of atracurium, pancuronium, and vecuronium was studied in 71 adult patients anesthetized with nitrous oxide and halothane. Infusion rates of blocking drugs were adjusted so that single twitch depression as measured by the evoked integrated EMG of the hypothenar muscles was kept at 10% of control. Two minutes after the termination of the infusion either edrophonium (0.75 mg/kg) or neostigmine (0.05 mg/kg) was administered. Single twitch depression and train-of-four …

INTRODUCTION: Reversal of rocuronium-induced neuromuscular blockade (NMB) is possible with either neostigmine or sugammadex to prevent residual NMB. In adults, sugammadex provides faster reversal and has fewer side effects compared to neostigmine, even when combined with glycopyrrolate. In elderly patients, rocuronium has a longer onset time and duration of action; however, little is known about the reversal of rocuronium-induced NMB with either sugammadex or neostigmine and the recovery of muscle function postoperatively. The aim of this study was to determine the time to full recovery of neu…

Postoperative cognitive dysfunction is a major clinical concern, particularly in elderly patients, leading to reduced quality of life and increased mortality. The impact of neuromuscular blockade reversal agents, such as sugammadex and neostigmine, on cognitive recovery remains uncertain. A systematic search of PubMed, EMBASE, and Cochrane databases identified randomized controlled trials (RCTs) and observational studies comparing sugammadex and neostigmine (± anticholinergics) for reversal of neuromuscular blockade in adults under general anesthesia for elective surgery. Studies published up …

BACKGROUND: The optimal pharmacological reversal strategy for neuromuscular blockade remains undefined even in the setting of strong recomme … used electromyography-based quantitative monitoring. Pharmacological reversal was accomplished with neostigmine if the train-of-four (TOF) ratio was 0.40 to 0.89 and with sugammadex for deeper levels of blockade. The primary end point was the incidence of postoperative residual neuromuscular blockade (PRNB), defined as TOF ratio <0.9 at time of extubation. We …

BACKGROUND: Postoperative cognitive dysfunction is a common postoperative complication in elderly patients. In elderly patients, the decline of organ function and neuromuscular junction function make them more sensitive to muscle relaxants. They are more likely to experience residual muscle relaxation after surgery, which may cause various adverse events. Neostigmine, a commonly used muscle relaxant antagonist, can reduce the expression of inflammatory factors, thereby reducing the pro-inflammatory response and neurodegeneration of the cerebral cortex and hippocampus after surgery. The study a…

BACKGROUND: Neuromuscular blockade and pneumoperitoneum (PP) are important factors to ensure successful laparoscopic surgery. However, residual neuromuscular blockade (rNMB) and PP are associated with many unfavorable complications. The aim of this study is to compare the cost-effectiveness of using sugammadex versus neostigmine in laparoscopic surgery in China.
METHODS: A decision tree model was developed with a time horizon based on laparoscopic surgery related hospitalization duration. 2000 patients using sugammadex or neostigmine were simulated within the model. The model outcomes include…
