The intracarotid amytal procedure (IAP) was used twice to assess the suitability of three male patients for two successive neurosurgical procedures to relieve intractable temporal lobe epilepsy. First an amygdalohippocampectomy was performed, then further tissue was removed in a temporal lobe resection because their seizures had failed to remit. Repetition of the IAP following amygdalohippocampectomy when there was a known excision allowed inferences to be made regarding its validity in assessing lateralization of language functioning, memory functioning, and lateralization of seizure focus. INTRODUCTION: Ehlers-Danlos syndrome (EDS) is a connective tissue disorder that has been linked to several neurological problems including Chiari malformations, atlantoaxial instability (AAI), craniocervical instability (CCI), and tethered cord syndrome. However, neurosurgical management strategies for this unique population have not been well-explored to date. The purpose of this study is to explore cases of EDS patients who required neurosurgical intervention to better characterize the neurological conditions they face and to better understand how neurosurgeons should approach the management

BACKGROUND: The robot-assisted neurosurgical procedures have recently benefited of the evolution of intraoperative imaging, including mobile CT unit available in the operating room. This facilitated use paved the way to perform more neurosurgical procedures under robotic assistance. Endoscopic third ventriculocisternostomy requires both a safe transcortical trajectory and a smooth manipulation.
METHOD: We describe our technique of robot-assisted endoscopic third ventriculocisternostomy combining robotic assistance and intraoperative CT imaging.
CONCLUSION: Robot-assisted endoscopic third ven

Venous air embolism (VAE) is a life-threatening complication of some surgical procedures. Though occurrence of VAE is frequent during neurosurgical procedures, coagulopathy following VAE has not previously been reported. Coagulation abnormalities are more commonly reported associated with fat or amniotic fluid embolism, but rarely with VAE. We present a case of massive VAE in sitting position leading to fatal coagulopathy even after successful resuscitation following the event. Coagulation abnormalities and bleeding can produce catastrophic consequences in neurosurgical patients. This report e

OBJECTIVEActive-duty neurosurgical coverage has been provided at Bagram Air Force Base in Afghanistan since 2007. Early operative logs were reflective of a large number of surgical procedures performed to treat battlefield injuries. However, with maturation of the war effort, the number of operations for battlefield injuries has decreased with time. Consequently, procedures performed for elective neurosurgical humanitarian care (NHC) increased in number and complexity prior to closure of the Korean Hospital in 2015, which resulted in effective termination of NHC at Bagram. To elucidate the association between medical procedures and sporadic Creutzfeldt-Jakob disease (sCJD), we analyzed medical procedures (any surgical procedure, neurosurgery, ophthalmic surgery, and blood transfusion) for patients registered by the CJD Surveillance Committee in Japan during 1999-2008. We conducted an age-stratified case-control study with 753 sCJD patients and 210 controls and a study of patients who underwent neurosurgical or ophthalmic surgical procedures at the same hospital. Although the control group was relatively small, no evidence was found that prion disease was transmi

In selecting an anesthetic agent to be used for neurosurgical procedures, the anesthesia provider must consider the agent's effects on intracranial pressure (ICP), cerebral blood flow (CBF), and cerebral metabolic rate of oxygen consumption (CMRO2). The anesthetic of choice for neurosurgical procedures for many decades has been thiopental. It meets the strict requirements for neurosurgical procedures because it protects the brain from ischemia and herniation by lowering ICP through decreases in CBF and CMRO However, new drugs, including etomidate and propofol, have been introduced that offer a

Of all the available halogenated anaesthetic agents, isoflurane appears to be the most advantageous for the neurosurgical patient. Concentrations which produce satisfactory anaesthesia for neurosurgical procedures cause little or no depression of myocardial function, and no increase in intracranial pressure. Cerebral perfusion pressure is maintained. Autoregulation remains effective at concentrations of up to 1.5 MAC. Vascular reactivity to carbon dioxide is also maintained. Cerebral metabolic rate is decreased in a dose-related fashion and a cerebral protective effect, maximal at clinical con

BACKGROUND: This study evaluates the outcomes and management considerations associated with using extracorporeal membrane oxygenation (ECMO) in patients with traumatic brain injury (TBI), including those requiring neurosurgical intervention. It examines the feasibility of managing patients with TBI and neurosurgical patients on ECMO by synthesizing institutional data with the existing literature, emphasizing observed clinical contexts, anticoagulation strategies, and peri-procedural factors relevant to safe ECMO use. Our aim is to elucidate the potential for ECMO to be safely applied in patien

Guidelines are presented for the neurosurgical treatment of chronic pain. In these guidelines a distinction is made between the pain of cancer and neurogenic pain. In cancer pain the survival time and the location of the lesion are the important guidelines. Possible procedures are: opioids via CSF route, lesions in nociceptive pathways and PV-PAG stimulation of the thalamus. In neurogenic pain, neurostimulation procedures, tailored to the location of the pain are procedures of first choice. There are however specific indications for other procedures depending on the aetiology of the pain. Caus

BACKGROUND AND OBJECTIVES: The performance of select neurosurgical procedures is being transitioned to an outpatient setting rather than an inpatient setting to increase healthcare cost-effectiveness. Despite numerous technological advancements in the treatment of unruptured intracranial aneurysms (UIAs), the procedures are solely performed in an inpatient setting. We aimed to compare the rate of short-term outcomes associated with inpatient endovascular treatment of UIAs with those for established outpatient neurosurgical procedures, including anterior cervical discectomy and fusions (ACDFs)

An analysis of the neurosurgical component of the medical support provided by a United Nations peace-keeping mission in Rwanda is presented. The Australian Defence Force contingent provided medical support to the United Nations and the civilian population. Eight hundred thirty-eight procedures were performed during 12 months. A wide range of surgery was encompassed, with neurosurgery accounting for 17 (2%) of the total operations: compound depressed fractured skull, 5; intracranial pressure monitor, 2; burr holes for acute head injury and chronic subdural hematoma, 2; skull osteomyelitis debri

OBJECTIVES: Venous thromboembolisms (VTEs) occur frequently in surgical patients and can manifest as pulmonary emboli (PEs) or deep venous thromboses (DVTs). While many medical therapies have been shown to prevent VTEs, neurosurgeons are concerned about the use of anticoagulants in the postoperative setting. To better understand the prevalence of and the patient-level risk factors for VTE, the authors analyzed data from the National Surgical Quality Improvement Program (NSQIP).
METHODS: Retrospective data on 1,777,035 patients for the years from 2006 to 2011 were acquired from the American Co

This article focuses on the application of neurophysiologic monitoring in uniquely neurosurgical procedures. Neurophysiologic monitoring provides functional testing and mapping to identify neural structures. Once identified, the functionality of the central and peripheral nervous system areas at risk for neurosurgical injury can be monitored. It discusses the use of motor-evoked potentials, sensory evoked potentials, electromyography and electroencephalography to assess neurologic change.

OBJECT: Mycoplasma hominis is a rare cause of infection after neurosurgical procedures. The Mycoplasma genus contains the smallest bacteria discovered to date. Mycoplasma are atypical bacteria that lack a cell wall, a feature that complicates both diagnosis and treatment. The Gram stain and some types of culture media fail to identify these organisms, and typical broad-spectrum antibiotic regimens are ineffective because they act on cell wall metabolism. Mycoplasma hominis commonly colonizes the genitourinary tract in a nonvirulent manner, but it has caused postoperative, postpartum, and postt

PURPOSE OF REVIEW: To review current treatments for trigeminal neuralgia, with an emphasis on determining which patients may benefit from neurosurgical procedures.
RECENT FINDINGS: A detailed history is the most helpful tool for diagnosing trigeminal neuralgia (TN) and predicting response to neurosurgical treatments. Patients with classic trigeminal neuralgia will describe severe, unilateral, intermittent facial pain that is triggered by innocuous sensory stimuli. In most cases, pain is caused by compression of the trigeminal nerve by a blood vessel near the brainstem. Magnetic resonance imag

Pneumocephalus is defined as a collection of air within the cranial cavity. It is a common finding following head injury with skull base fracture and neurosurgical procedures. Tension pneumocephalus is a rare complication. The diagnosis is clinical and radiological with the characteristic Mount Fuji sign seen on computed tomography (CT). It is a neurosurgical emergency, and early recognition and treatment are vital.

Intradiploic cerebrospinal fluid (CSF) collections represent a rare complication of either head trauma or neurosurgical procedures. Their formation is thought to depend partly on violation of the meninges and the inner cortical bone abutting the neuraxis. The authors present two pediatric cases involving diploic CSF collections following neurosurgical interventions. In the first case, a CSF fistula was found within the occipital bone and the petrous portion of the temporal bone 9 years after a suboccipital craniectomy. The second case features the extremely rare occurrence of a CSF fistula wit
