This case report describes a 24-year-old male who developed subacute combined degeneration (SCD) as a result of chronic recreational nitrous oxide (N₂O) abuse over a six-month period. The patient presented with a two-week history of progressive paresthesias, initially affecting the feet and subsequently extending to the hips, along with lower limb numbness. Neurological examination revealed ataxia, hypoesthesia, impaired proprioception, and hyperreflexia, while muscle strength remained intact. Nitrous oxide (N2O, laughing gas) has been used as an anaesthetic and analgesic for almost two centuries, but its cellular targets remain unclear. Here, we present a molecular mechanism of nitrous oxide's selective inhibition of CaV3.2 low-voltage-activated (T-type) calcium channels in pain pathways. Using site-directed mutagenesis and metal chelators such as diethylenetriamine pentaacetic acid and deferoxamine, we reveal that a unique histidine at position 191 of CaV3.2 participates in a critical metal binding site, which may in turn interact with N2O to produce reactive oxygen species (ROS).

A rise in fetal Paco2 was observed after elective cesarean section in patients anesthetized both with a barbiturate and with nitrous oxide/oxygen. Epidural analgesia seemed to be a good alternative in order to attain better blood gas values in the newborn infant. H2O2 is a vital atmospheric oxidant, while its photolysis in the troposphere is negligible in conventional view. For the first time, we find that gas-phase H2O2 can undergo heterogeneous photolysis to produce OH radicals on the surfaces of particles such as SiO2, Fe2O3, and Na2SO4 under solar irradiation, which further convert NO to produce HONO and NO2. The quinones 1,4-naphthoquinone (NQ), methyl-1,4-naphthoquinone (MNQ), trimethyl-1,4-benzoquinone (TMQ) and 2,3-dimethoxy-5-methyl-1,4-benzoquinone (UQ-0) enhance the rate of nitric oxide (NO) reduction by ascorbate in nitrogen-saturated phosphate buffer (pH 7.4).
