Oncologic causes of oculopalatal tremors: neurophysiology and treatment.

Oculopalatal tremor (OPT) is an acquired pathology characterized by continuous and rhythmical soft palatal movements combined with pendular nystagmus. Aside from vascular lesions, oncological masses affecting the dentatorubro-olivary pathway can impair brainstem and/or cerebellar pathways, manifesting as dyssynchronous movement. In this review, we delve into the neurophysiology of OPT along with oncological causes and treatment options based on the most recent clinical trial data. This literature review includes medication treatment data from clinical trials enrolling individuals with features

Viewing condition dependence of the gaze-evoked nystagmus in Arnold Chiari type 1 malformation.

Saccadic eye movements rapidly shift gaze to the target of interest. Once the eyes reach a given target, the brainstem ocular motor integrator utilizes feedback from various sources to assure steady gaze. One of such sources is cerebellum whose lesion can impair neural integration leading to gaze-evoked nystagmus. The gaze evoked nystagmus is characterized by drifts moving the eyes away from the target and a null position where the drifts are absent. The extent of impairment in the neural integration for two opposite eccentricities might determine the location of the null position. Eye in the

Vertigo in brainstem and cerebellar strokes.

PURPOSE OF REVIEW: The aim of this study is to review the recent findings on the prevalence, clinical features, and diagnosis of vertigo from brainstem and cerebellar strokes. RECENT FINDINGS: Patients with isolated vertigo are at higher risk for stroke than the general population. Strokes involving the brainstem and cerebellum may manifest as acute vestibular syndrome, and acute isolated audiovestibular loss may herald impending infarction in the territory of the anterior inferior cerebellar artery. Appropriate bedside evaluation is superior to MRI for detecting central vestibular syndromes.

Pure upbeat nystagmus in association with bilateral internuclear ophthalmoplegia.

A 66-year-old man developed primary position upbeat nystagmus and bilateral internuclear ophthalmoplegia (INO). Video-oculography showed primary position upbeat nystagmus with exponentially decreasing slow phases, which disappeared in darkness. Brain MRI disclosed enhancing lesions involving bilateral dorsomedial pons extending from the middle to upper portion. Upbeat nystagmus in association with bilateral INO may be attributed by the damage of the cell groups of the paramedian tracts (PMT), the projections from the interstitial nucleus of Cajal (INC) to PMT, or the connections between INC an

Pseudonystagmus--clinical features and quantitative characteristics.

BACKGROUND: A 60 year-old woman with multiple sclerosis (MS) and chronic hearing loss presented with head tremor and vestibular hypofunction, the combination of which can produce oscillopsia--a visual sensation that steady objects in the visual field are oscillating. This case highlights the fact that in patients with pseudonystagmus, oscillopsia is attributable to the association of head tremor and vestibular hypofunction. INVESTIGATIONS: Eye and head movements were measured simultaneously with the search coil technique. DIAGNOSIS: Patients with MS can present with oscillopsia, and spontane

The Faden operation. Technical aspects and indications.

Within 15 years the Faden operation (Cüppers' operation) has proven to be a valuable tool in cases of variable angle of squint and a number of other indications. In this paper the theoretical background of the Faden effect, the indications, the operative technic as well as the results are presented.

Pendular Nystagmus: a Novel Feature of ANO10-Related Disorders.

ANO10-related ataxia is characterized by cerebellar and pyramidal signs, with prominent oculomotor abnormalities, including saccadic abnormalities, strabismus, and ptosis. While nystagmus is frequently observed, the specific subtype of horizontal pendular nystagmus has not been emphasized. This report describes two siblings with early-onset spastic-ataxia with horizontal pendular nystagmus, ultimately diagnosed with SCAR10. This report details the ocular abnormalities spectrum of ANO10-related ataxia by highlighting horizontal pendular nystagmus as a significant clinical feature in early-onset

EEG based oculographic analysis of epileptic nystagmus.

Epileptic nystagmus (EN) is a subtle seizure semiology, most commonly seen in seizures originating in the posterior cortical regions. EN is broadly categorized into type I and type II. Type I EN consists of contralateral repetitive saccadic eye movements alternating with post-saccadic slow drifts with an overall contralateral deviation. Type II EN is characterized by ipsilateral slow drift alternating with contralateral corrective saccades. In this article, we report a method to perform oculographic analysis of eye movements using EEG only. We used this method to classify the type of EN in thr

CLINICAL AND OBJECTIVE TEST CHARACTERISTICS OF VESTIBULAR MIGRAINE: IMPLICATIONS FOR DIAGNOSIS AND MANAGEMENT.

The research we provided look at a number of factors, such as age, unilateral testing, and squinting both during the ictal and interictal periods to define vestibular migraine. One hundred and ten adults with recurrent spontaneous and positional vertigo participated in the study, which the investigators did. Vestibular migraines (VM) or probable vestibular migraine constituted the two diagnoses given to the patients (n = 29 and n = 76, respectively). The findings revealed those surveyed frequently complained of headache (85.3%), spinning vertigo (76.2%), and Mal de Débarquement (60.2%), with m
