Dorsal Midbrain Syndrome: Clinical and Imaging Features in 75 Cases.

BACKGROUND: Dorsal midbrain syndrome (DMS) consists of a constellation of clinical features, including reduced upgaze, pupillary light-near dissociation, lid retraction, convergence retraction, and eye misalignment. This syndrome results mostly from intrinsic or extrinsic mesodiencephalic tumors or strokes, obstructive hydrocephalus, failure of cerebrospinal fluid shunting to correct obstructive hydrocephalus, and head trauma. Published reports that include imaging corroboration are based on relatively small cohorts and have not included comprehensive patient self-reports on the impact of thes

Neuro-ophthalmologic disorders following concussion.

Visual symptoms, such as photophobia and blurred vision, are common in patients with concussion. Such symptoms may be accompanied by abnormalities of specific eye movements, such as saccades and convergence, or accommodation deficits. The high frequency of visual involvement in concussion is not surprising, since more than half of the brain's pathways are dedicated to vision and eye movement control. These areas include many that are most vulnerable to head trauma, including the frontal and temporal lobes. Vision and eye movement testing is important at the bedside and on the sidelines of athl

Oculo-Visual Dysfunction in Parkinson's Disease.

This review describes the oculo-visual problems likely to be encountered in Parkinson's disease (PD) with special reference to three questions: (1) are there visual symptoms characteristic of the prodromal phase of PD, (2) is PD dementia associated with specific visual changes, and (3) can visual symptoms help in the differential diagnosis of the parkinsonian syndromes, viz. PD, progressive supranuclear palsy (PSP), dementia with Lewy bodies (DLB), multiple system atrophy (MSA), and corticobasal degeneration (CBD)? Oculo-visual dysfunction in PD can involve visual acuity, dynamic contrast sens

Ocular motility of aging and dementia.

Visual complaints in patients with dementia are varied and attributable to both visual sensory (afferent) and ocular motor (efferent) dysfunction. This review focuses exclusively on the efferent visual dysfunction associated with dementia and aging. It provides a brief overview of the most common ocular motility disturbances associated with dementia, including Alzheimer's disease, Parkinson's disease, diffuse Lewy body disease, corticobasal syndrome, progressive supranuclear palsy, frontotemporal lobar degeneration, Creutzfeldt-Jakob disease, and others. An introduction to the six eye movement

Clinicopathological study of three cases of infantile fibromatosis of the orbit.

We report the clinical and pathological findings in three infants with infantile fibromatosis that involved several extraocular muscles which led to disorders of ocular motility. We also describe their clinical features before and after surgery. This was a retrospective interventional case study with clinicopathological correlations. We present three cases of infantile fibromatosis that were diagnosed by clinical features and histopathlogical examination of biopsy specimens. The three patients were all female children aged 1, 3, and 3 years at their initial visit. The orbital tumor was unilate

Ocular neuromyotonia treated by microvascular decompression: usefulness of preoperative 3D imaging: case report.

Ocular neuromyotonia is a rare ocular motility disorder characterized by involuntary contraction of extraocular muscles resulting in paroxysmal diplopia. Although ocular neuromyotonia is reported as a rare complication after radiation therapy, there are a few cases of ocular neuromyotonia in the absence of irradiation. In the reported cases the possibility of vascular compression has been suggested on radiological imaging. The authors report a case of ocular neuromyotonia treated by microvascular decompression of the third cranial nerve, supporting the hypothesis that neurovascular compression

Cyclotorsion as a structural and functional biomarker of ocular and neuroophthalmic diseases.

Cyclotorsion, defined as rotation of the eye around its anteroposterior axis, is an important but often underrecognized component of ocular motility. This review summarizes the physiological basis of cyclotorsion, methods of subjective and objective assessment, and the clinical relevance of torsional deviations in ocular and neuro-ophthalmic disorders. Particular emphasis is placed on objective measurement using the disc-foveal angle and its role in diagnosing and monitoring conditions such as trochlear nerve palsy, skew deviation, pulley disorders, inferior oblique overaction, thyroid eye dis

Neuro-ophthalmic genetics.

Mutations in nuclear or mitochondrial DNA may cause disorders of neuro-ophthalmic significance. These include disorders of the optic nerve, such as Leber's hereditary optic neuropathy and Kjer-type optic atrophy, and disorders of ocular motility, such as congenital nystagmus, autosomal dominant progressive external ophthalmoplegia, and oculopharyngeal muscular dystrophy. In addition to more accurate disease classification and diagnosis, identification of genetic loci, genes, and their mutations has stimulated investigation into factors influencing disease expression and penetrance.

Congenital ocular motor apraxia. Case reports and literature review.

Two children with congenital ocular motor apraxia (C-OMA) associated with congenital malformations of the central nervous system (CNS) are presented, and the literature is reviewed. C-OMA is an abnormality of ocular motility characterized by defective or absent voluntary horizontal gaze, associated with a characteristic head thrust. Although not rare, it is infrequently recognized. C-OMA is not a specific disease entity, but a sign. It may be associated with other static congenital CNS disorders and must be distinguished from acquired ocular motor apraxias (A-OMA) seen in progressive brain dis

Frequency of Visual Deficits in Children With Developmental Dyslexia.

IMPORTANCE: Developmental dyslexia (DD) is a specific learning disability of neurobiological origin whose core cognitive deficit is widely believed to involve language (phonological) processing. Although reading is also a visual task, the potential role of vision in DD has been controversial, and little is known about the integrity of visual function in individuals with DD. OBJECTIVE: To assess the frequency of visual deficits (specifically vergence, accommodation, and ocular motor tracking) in children with DD compared with a control group of typically developing readers. [Internuclear bilateral pseudo-ophthalmoplegia and dermatomyositis].

A 60 year-old woman complaining of diplopia presents an ocular motility disturbance mimicking internuclear ophthalmoplegia. Idiopathic dermatomyositis is diagnosed by the help of clinical, biological, electrophysiological and histological data. The outcome is favorable under corticotherapy. Ocular muscle involvement is rare in dermatomyositis. An overlap syndrome with another auto-immune disorder like myasthenia should be excluded in this kind of manifestation.

Fixational Microsaccades in Patients With Parkinson Disease.

BACKGROUND: Fixational microsaccades (FM) encompass the involuntary, small-scale movements of the eye that occur as we attempt to fixate on a single point. These movements are crucial for maintaining a steady visual perception despite our gaze's constant, subtle changes. FM is significant because it can serve as an indicator of central nervous system disorders; for example, altered FM correlated with multiple sclerosis disability levels and disease progression. This study aims to characterize FM in patients with Parkinson disease (PD). METHODS: Fourteen older adults (age 71.0 ± 7.2 years, 6 f

The Influence of Age on Eye Movements during Reading in Early Elementary School Children.

PURPOSE: Eye movement disorders have been observed in many eye diseases, such as amblyopia and developmental dyslexia. The detection of pathological eye movement behaviour is difficult and requires more data for comparison. Therefore, the main purpose of this study was to evaluate the influence of age, school level, gender, and mother tongue on eye movements while reading. METHODS: One hundred and twenty-seven normally sighted children aged 7 - 12 were recruited from grades 2 - 5. The children were asked to read aloud two texts of The New International Reading Speed Text (IReST) of similar di

Binocular, Accommodative and Oculomotor Alterations In Multiple Sclerosis: A Review.

Multiple sclerosis (MS) is an acquired demyelinating and inflammatory neurodegenerative disease affecting the central nervous system (CNS). Clinical and subclinical ocular disturbances occur in almost all patients with MS. The objective of this narrative review was to collect and summarize the available scientific information on oculomotor, accommodative and binocular alterations that have been reported in MS. A systematic search strategy with the following descriptors was carried out: multiple sclerosis, ocular motility disorders, internuclear ophthalmoplegia, nystagmus, vergences, fixation,…

[Corrective saccades in patients with multiple sclerosis].

Basing on the data analysis of natural and erroneous fast eye movements, the quantitative estimation of static and dynamic features of corrective saccades in patients with multiple sclerosis has been carried out. The differences in latency, maximal velocity and other parameters of corrective movements are used in the discussion of brain strategies on the development of normal and corrective eye movements and neuronal pathways for their realization.
