Transverse testicular ectopia (TTE) is a rare congenital anomaly in which both testes descend through the same inguinal canal. The most frequent clinical presentation is undescended testis (UDT) with ipsilateral inguinal hernia and contralateral non-palpable testis. This condition is often diagnosed during surgery and is frequently associated with other anomalies. There is controversy in the surgical management of TTE. Thirty-four cryptorchid testis cancer cases were studied, of whom 9 patients had prior orchiopexy at the time of cancer diagnosis. Disease stage in this group was: stage I = 4, stage II = 1 and stages III and IV = 4 cases. Seventy-eight percent of these cases (n = 7) had non-seminomas; 4 of these patients died. We evaluated 77 male patients with true cryptorchidism ranging from 2 to 46 months old to determine whether orchiopexy could be performed safely and efficiently, and whether treatment was helpful in patients less than 1 year old. Our results indicate that orchiopexy in patients younger than 1 year is safe, effective and beneficial, and optimizes treatment of this condition.

In children with an impalpable testis, laparoscopy is a valuable procedure that can be used in conjunction with inguinal and abdominal exploration to localize the testis. Twenty-five gubernacula were removed for histological examination. All were routinely stained and some were additionally stained for elastic fibres and content of hyaluronic acid. The gubernaculum consisted of moderately vascular, fibrofatty tissue with a core of essentially longitudinally oriented collagenous fibres surrounded by mature fatty tissue. No contractile elements were seen in the gubernaculum. It did not differ from subcutaneous fascia concerning content of elastic fibres and hyaluronic acid.

PURPOSE: The optimal age at which to perform orchiopexy for cryptorchidism has long been debated. The aim of this study was to determine if age at orchiopexy affected testicular atrophy.
METHODS: A retrospective review of patients undergoing orchiopexy from 2000 to 2010 was conducted. An individual testis, rather than patient, was used as the dependent variable. Effective postoperative analgesia is important, especially in pediatric surgery. The efficacy of intraoperative surgical wound irrigation with bupivacaine for postoperative analgesia was investigated in 90 children undergoing elective inguinoscrotal surgery. We found that this method effectively reduced postoperative pain and narcotic drug requirement. Bupivacaine irrigation was simple and complication-free. We believe that the irrigation of surgical wounds with bupivacaine should be a routine procedure in elective inguinoscrotal surgery in children.
