Therapy of ankylosing spondylitis--a review. Part I: Conventional medical treatment and surgical therapy.

The main outcome parameters in AS and other SpA are pain, disease activity, function, spinal mobility, and structural progression. There are several treatment options, which differ, depending on the localization and severity of the manifestation and the degree of structural changes already present. There is a basic role for physiotherapy to improve and maintain spinal mobility and function and prevent handicap. The basis for drug therapy in AS is NSAIDs, which may also have an influence on structural progression. DMARDs are mainly used in cases with peripheral arthritis. Corticosteroids have a

Management of Closed Incisions Using Negative-Pressure Wound Therapy in Orthopedic Surgery.

Negative-pressure wound therapy (NPWT) has been a major advance in the management of open wounds, especially in orthopedic trauma surgery. Due to the success of NPWT, surgeons began using NPWT on closed incisions as a way to help prevent surgical wound dehiscence, especially in at-risk patients for wound problems. It has been well established that obesity, diabetes mellitus, and smoking in addition to other comorbidities increase the risk of wound dehiscence and surgical site infections in orthopedic patients. This article reviews the current literature and provides an overview on the use of N

Secondary refinements of free perforator flaps for lower extremity reconstruction.

BACKGROUND: The aim of lower extremity reconstruction has focused on early wound coverage and functional recovery but rarely aesthetics. Free muscle flaps provide durable coverage; however, they require skin graft coverage and result in muscle atrophy limiting future revisions. Perforator-based flap reconstructions can be easily elevated to allow for both orthopedic and contouring procedures. The authors reviewed the role of secondary procedures in achieving improved functional and aesthetic results following perforator flap reconstruction of lower extremity defects. METHODS: A retrospective

Aspirin for Venous Thromboembolism Prophylaxis in Hip Fracture, Total Knee, and Hip Arthroplasty.

Venous thromboembolism is a complication of major orthopedic surgeries such as hip fracture repair, total knee arthroplasty, and total hip arthroplasty. Prophylaxis against venous thromboembolism reduces the risk of pulmonary embolism and deep vein thrombosis associated with these procedures. Aspirin is widely prescribed for venous thromboembolism prophylaxis in major orthopedic surgery. We review the epidemiology of hip fracture, total knee arthroplasty, and total hip arthroplasty, and describe aspirin's mechanisms of action, efficacy, and safety.
