INTRODUCTION: Pubic osteomyelitis has been described in three situations: children in whom Staphylococcus aureus is the preeminent pathogen; elderly patients who have undergone genitourinary procedures, and parenteral drug abusers. In contrast, pubic osteomyelitis in athletes has been described less often. We report three cases of acute staphylococcal pubic osteomyelitis in young athletic men and present a review of the literature.
EXEGESIS: The clinical presentation in each case was acute groin, hip, or perineal pain; fever; inability to bear weight; and pubic symphysis tenderness. The diagn…

Osteomyelitis can be subdivided into a bacterial exogenic (post-traumatic/postoperative), bacterial endogenic (hematogenous) and an abacterial type, including the rare group containing primary chronic sclerosing osteomyelitis, which is typically localized in the clavicle. In a review of the literature, the criteria for the various types of chronic sclerosing osteomyelitis are analyzed. Out of a group of 17 patients with osteomyelitis of the clavicle treated between 1978 and 1996, three cases of primary chronic sclerosing osteomyelitis are demonstrated. In the differential diagnosis, primary ch…

Osteomyelitis is an infection of the bone marrow caused by both non-specific and specific agents. Non-specific endogenic osteomyelitis represents the most frequent form, followed by the specific form caused by Mycobacterium tuberculosis infection. Typically, the non-specific form occurs in children and young adults, and the sites most frequently affected are the distal and the proximal ends of the tibia and femur. In tuberculosis, osteomyelitis is predominantly located in the vertebral bodies. In rare cases osteomyelitis can be caused by viruses, fungi or echinococcus. It may also occur as so-…

Osteomyelitis of the clavicle is an uncommon disease, but it should be considered in patients who present with pain, cellulitis, or drainage in the sternoclavicular area following head and neck surgery, irradiation, subclavian vein catheterization, or immunosuppression. An idiopathic presentation is possible. In contrast to primary osteomyelitis of the clavicle, which is occasionally seen in children, secondary osteomyelitis is quite rare. It is often mistaken for a fracture or a possible neoplasm on plain x-rays. Tomograms and CT scanning are confirmatory, and in early cases, technetium-99m b…

This article describes 12 patients (aged 1-28 days) suffering from neonatal osteomyelitis diagnosed between 1975 and 1987. Complications had occurred during 5 pregnancies, 9 patients had problems in the perinatal period (5 infections). In 9 cases (75%) only local symptoms existed, of which localized swelling, erythema, and loss of function were the most common. Deep soft-tissue swelling was the most common initial radiographic finding. Staphylococcus aureus (penicillin-resistant in all cases) was the most important aetiological organism (66%). Haemolytic streptococci occurred in 3 patients (gr…

The utility of bone imaging has remained significantly high despite the proven suboptimal specificity of the conventional three-phase bone scan. The quantitative four-phase study may play a role in maintaining its usefulness in the future. However, due to its extremely high sensitivity and excellent anatomical information, it remains as the first diagnostic study in most suspected cases of acute osteomyelitis. The future would see development of new imaging modalities aimed at enhancing the specificity for diagnosis of active osteomyelitis. The indium leucocyte study has been the single most i…

Pycnodysostosis is a rare sclerosing bone disorder in which osteomyelitis of the jaws is a frequent complication. Treatment of osteomyelitis of the jaws in pycnodysostosis is difficult and may lead to large resections. A review of the literature and 3 new cases in which osteomyelitis was successfully treated by a combined surgical and antibiotic approach, are reported.

Contrary to what used to be the case, actinomycosis is now a rare disease and only infrequently mentioned in otolaryngological textbooks (Ballantyne, J. and Groves, J. 1979). The disease is a chronic suppurative infection caused by micro-organisms from the actinomyces group, most often israelii. Actinomyces species are all oral commensals. They are gram-positive, non acid-fast, anaerobic or microaerophilic filamentous bacteria (Walker et al., 1981; Bennhoff, 1984). Associated flora of both aerobic and anaerobic bacteria are always present, most frequently anaerobic streptococci, fusiform or gr

The incidence of osteomyelitis has been greatly reduced since the advent of antibiotics. In many 3rd World countries, however, numerous cases of severe advanced osteomyelitis still present annually. A case of subacute osteomyelitis is reported and the pathology and clinical features of chronic osteomyelitis are reviewed.

We report a patient with systemic sporotrichosis and osteomyelitis without concomitant suppurative arthritis. To our knowledge this is the first such documented occurrence of hematogenous osteomyelitis in the systemic form of the disease. It was diagnosed by radionuclide bone scintigraphy and radiography and confirmed by biopsy.

The clinical, laboratory, and roentgenographic features of acetabular osteomyelitis were present in a 20-month-old boy and a five-year-old girl. The clinical and laboratory parameters were similar in extracapsular acetabular osteomyelitis and septic arthritis of the hip. A secondary effusion was evident in roentgenograms in both conditions. Acetabular osteomyelitis responded to selected intravenous antibiotic therapy. Septic arthritis of the hip required immediate surgical drainage as well as appropriate antibiotic coverage.

Fungal osteomyelitis is rare in children. Eumycetoma osteomyelitis occurs by direct external inoculation. Suspicion should be aroused when there are intermittent or persistent draining sinuses, even if bacterial cultures are positive. Treatment must be aggressive and should include radical local debridement. The quest for effective systemic chemotherapy should continue. Persistent drainage, ascending infection, and a functionless extremity are reasons for considering amputation.

Cortical fissuring, a well-recognized radiographic sign of osteomyelitis, represents spread of infection through the haversian system with osteoclastic bone resorption. In the proper clinical setting this finding may be recognized before there is other evidence of bone destruction or periosteal reaction. Three cases are presented that illustrate the sign at a relatively early stage in the progress of the osteomyelitis. While fissuring was previously considered evidence of sickle cell anemia, only one of our patients had sickle hemoglobin.

The management of seventy-six children admitted to Aler Hey Children's Hospital with osteomyelitis is discussed and the indications for surgical intervention are defined. The clinical and pathological course of the disease is examined and the term post-acute osteomyelitis is introduced. The importance of recognition of this stage of the disease and its relevance to the management is highlighted. The use of single antibiotic therapy (clindamycin) is shown to be at least as effective as combinational therapy. Complete resolution was achieved in 94.7% of children, surgical intervention was perfor…

A number of problems in the treatment of acute osteomyelitis have remained unresolved in recent years. The clinical experience of ninety-three patients with proven acute haematogenous osteomyelitis is presented to help resolve these problems. Analysis of the clinical features, the operative, bacteriological and haematological findings is made and discussed in detail. Eighteen patients had continuing bone infection and recommendations are made as to how diagnosis and management might have been improved. Surgery is considered to be an essential part of the diagnostic and therapeutic management o…
