Progress in successful surgical treatment of the obstructive sleep apnea/hypopnea syndrome (OSAHS) has been based on adjunctive treatment of the hypopharynx. Multiple myeloma is a systemic malignant disease and is associated with a poor prognosis. It is characterized by neoplastic proliferation of plasma cells involved in the production and secretion of monoclonal immunoglobulins (M proteins). A rare case of adenosquamous carcinoma in a 74 year-old man is reported. Presenting as a nodule on the soft palate, diagnosis was prolonged because of the benign macroscopic aspect. CT-scan and MR-tomography showed an encapsulated lesion but biopsy and histologic examination revealed the typical features of adenosquamous carcinoma. The tumour consisted of adenocarcinoma and squamous cell carcinoma in close proximity to minor salivary glands of which the tumour seemed to have its origin. Oral melanomas are extremely rare and occur most often on the palate and gingiva. Its prognosis is poor. Surgical management remains the preferred treatment, sometimes in combination with chemotherapy. We report the case of a 72 years old woman with a malignant melanoma of the hard palate who developed three years later a gingival nevi and 5 years later metastatic cervical lymph nodes. Our treatment has been only radical surgery all times.

Between 1989 and 1994 17 patients with carcinoma of the soft palate were treated in Department of Otolaryngology, Białystok. Majority of patients (15) had small tumors T1-T2. 1/3 of the patients developed regional metastases during the course of disease. 23.5% of the patients had second primary tumor in the upper aerodigestive tract, two previously, one synchronously and one metachronously. 10 patients were treated with surgery and 5 with radiotherapy as single treatment modalities. Palatal myoclonus (PM) is usually caused by lesions of the brainstem. We report a case of PM of focal cortical origin in a patient with epilepsia partialis continua. The PM sometimes occurred in isolation, and at other times was accompanied by unilateral face, neck, and arm twitching. This was documented by both EEG and SPECT.

Choristoma is the pathological term for a developmental tumor-like anomaly consisting of tissues foreign to the site at which it is located. A 3-month-old girl with a choristoma of the soft palate is presented. Its occurrence is extremely rare, as shown by a review of the literature.

Two cases with idiopathic palatal myoclonus without other neurological deficits were described. They did not have any other neurological deficits other than myoclonus of branchial muscles. In these cases, the myoclonus disappeared during natural or induced sleep. In Case 1, the myoclonus ceased transiently when the patient was calculating or receiving an injection. A 2-month-old girl presented with a history of intermittent cyanosis and dyspnea after feeding. This was found to be due to a 2 cm long polypoid lesion attached to her soft palate. This was removed and histologic examination showed it to be choristoma. Three years later she remains well with no evidence of recurrence.

Three articles on the influence of timing of palatal closure on jaw development, ENT condition and speech are subjected to an overall evaluation. A number of questions are formulated, which are still to be answered before a well-founded decision on the timing of palatal surgery can be taken. The evidence presently available made the author conclude that the whole palate should be closed before the age of 18 months, at least if the development of speech has priority over development of the maxilla.

A sample of 575 palatal casts was studied and found to possess varying degrees of papillary convolutions between and around the palatal rugae. No palatal appliances were worn and no irritative influence was present. A classification of 1-4 was established and the peak incidence was Class II at 48.4% (Class I, being an absence of papillarity, was 7.3%). It is postulated that the presence of a denture may well cause an inflammatory papillary hyperplasia to arise not by a progression through the various stages of denture stomatitis but directly, in an individual with a papillary palate.
