PURPOSE: Systematic identification of all 4 parathyroid glands has been recommended during total thyroidectomy (TT); however, it is unclear whether this strategy necessarily translates into optimized functional parathyroid preservation. The lateral parathyroid gland of the rabbit was resected and autotransplanted under an ear skin flap. A skin flap at the opposite side was used as control. The success of autotransplantation was established by microangiography of the transplant and by measuring plasma concentrations of parathyroid hormone (PTH), calcium and phosphorus. The authors studied the parathyroid glands from 100 previously healthy subjects who died suddenly and were admitted to the Dade County Medical Examiner's (ME) morgue and from 66 inpatients who died at Jackson Memorial Hospital (JMH). A patient with an endocrine-active carcinoma of the parathyroid gland was observed. The typical signs of hyperthyroidism could be seen in the skeletal system. A myxoid lipoadenoma of the parathyroid gland causing primary hyperparathyroidism is reported. The very rare and histologically unusual tumor consists of an epithelial component which is combined with a stromal component in a characteristic way. Slender, irregularly branched trabeculae which mainly consist of chief cells as well as a few oxyphil cells are found as epithelial component. The predominantly myxomatous and only focally lipomatous stromal component is interpreted as an integrated tumor component of the lipoadenoma which is derived from the parathyroid adipose tissue.

Lipoadenoma is a rare tumor of the parathyroid gland composed of adult adipose tissue and irregular trabeculae of parathyroid cells. This variant of adenoma is poorly documented. Similar lesions that have been either functional or apparently nonfunctional have been reported as adenoma with unusual stromal development and as parathyroid hamartoma, respectively. In this report, the second case of a functional lipoadenoma of the parathyroid gland is described.
