CT, MR, and SPECT findings in a general paresis.

CNS changes in a case with general paresis were investigated by X-ray computed tomography (CT), Magnetic resonance (MR), and single photon emission computed tomography (SPECT). CT and MR showed a mild degree of diffuse cortical atrophy and the dilatation of lateral ventricles with no signs of ischemic lesions or inflammations. On the other hand, SPECT using 123I-N-isopropyl-p-iodoamphetamine (123I-IMP demonstrated marked reduction of the cerebral blood flow especially in the bilateral frontal and temporal cortices. Moreover, the reduction of the blood flow was significantly improved after the …

[Treatment of vocal cord paresis with Teflon injections].

Twenty-five patients suffering from unilateral paresis of the vocal cord of differing durations caused by benign diseases and 25 patients with unilateral paresis caused by malignant tumours in the thorax/mediastinum were treated with Teflon injection of the paretic vocal cord. The results of treatment were evaluated by a speech therapist by voice recording pre-operatively, three weeks post-operatively and at a later control. This evaluation together with self-assessment demonstrated a satisfactory result even the long term result. In 2/3 of the patients good voice function was found at later c…

[Abducens paresis, a rare complication to spinal analgesia].

A case of paresis of the abducens nerve following spinal analgesia is described. Cranial nerve paresis is a rare complication of spinal analgesia although described for all cranial nerves except nos. 1, 9 and 10. It is very useful to know that cranial nerve paresis after spinal analgesia often occurs after a delay of several days, and that treatment is unnecessary because symptoms nearly always disappear spontaneously in about 2-4 months.

[Postoperatively developing serratus paresis as a legal problem].

It is demonstrated in five exemplary cases that a neuralgic amyotrophy with particular involvement of the long thoracic nerve was the cause of a postoperative serratus paresis. In any case liability claims, it is essential for an expert's opinion on postoperative serratus paresis to differentiate this pathology from intraoperative injuries caused by pressure. Above all the interval between operation and the first manifestation of symptoms, as well as the development of distinct pain count against an intraoperative plexus lesion.

Congenital neurosyphilis and juvenile paresis: a forgotten entity?

Juvenile neurosyphilis is rare and may be arrested or modified in the early stage by antibiotic treatment for intercurrent infections. Inadequate treatment may weaken the signs and symptoms of neurosyphilis so much that a high index of clinical suspicion is required for early and correct diagnosis. It should be remembered, however, that benzathine penicillin does not constitute adequate treatment for neurosyphilis.

Meningomyelocele and progressive hydromyelia. Progressive paresis in myelodysplasia.

Five cases of myelodysplasia with progressive paraparesis are presented. Three of the five patients developed spasticity, but dissociated sensory loss and loss of sphincter control was not a prominent feature. All were found to have compensated hydrocephalus and extensive communicating hydromyelia. The use of myelography and ventriculography in the diagnosis of hydromyelia is discussed. Ventricular drainage led to clinical improvement in two cases and radiological improvement in one. The relationship of compensated hydrocephalus, meningomyelocele, and progressive hydromyelia postnatally may su…

The effect of delayed disc surgery on muscular paresis.

A prospective study was carried out in 280 patients suffering from sciatica caused by myelographically verified disc prolapse. The patients … c voluntary contractions in all the patients 2 weeks after admission. Approximately 50 per cent had paresis. Control examinations of these patients 1 year later showed that operative treatment gave no better prognosis than conservative treatment with regard to the motor function, neither in the group chosen at random nor in the selected groups. The causat …
