We report the case of an 8-years-old girl with recurrent pulmonary infections and wheezing since infancy, in whom asthma and immunoglobulin-G deficiency were diagnosed at the age of 7 months. Since then, the patient was treated for asthma without any satisfactory control of the disease. Cardiomegaly was finally diagnosed radiologically that led to cardiac assessment. Malignant mesothelioma of the pericardium is a rare and fatal condition that clinicians should be aware of due to its variability of clinical manifestation. The diagnosis may be delayed as a result of delayed treatment. Here, we report two cases of malignant pericardial mesothelioma with two different clinical aspects: cardiac tamponade and mimic tuberculous pericarditis. We report a case of two babies with absence of pericardium and patent ductus arteriosus (PDA). The absence of pericardium was found coincidentally during PDA ligation. The PDA was successfully ligated but the pericardium was not reconstructed. Postoperatively, the agenesis of the pericardium did not interfere with cardiac function.

We report a patient with congenital absence of the left pericardium with development of progressive annuloaortic ectasia and aortic insufficiency during a 12-year period. The patient was treated with a Bentall procedure. Pathologic examination of the aorta revealed cystic medial necrosis. The surgical management and a possible association between congenital absence of pericardium and Marfan syndrome are discussed.

We report on a case of a 60-year-old man with progressive heart failure, mitral and aortic valve insufficiency and bilateral asymmetrical skeletal upper-limb deformities. Central to the suspicion of Holt-Oram syndrome in this patient was the surgical finding of agenesis of the left pericardium. A 74-year-old white woman with diffuse myalgias, low grade fever, and pericardial effusion was found to have polyarteritis nodosa (PAN) in a pericardiectomy specimen. This diagnosis was confirmed at autopsy. Although pericardial involvement in PAN has been described, this is the first report of PAN diagnosed in a pericardiectomy specimen.

A peculiar vascular tumor of the pericardial cavity affecting a 21-year-old man is reported. The tumor was formed by spindle cells and narrow vascular channels. It was difficult to decide between primary pericardial Kaposi's sarcoma or angiosarcoma. The patient was HIV negative and no other tumors were revealed in the course of the disease. Immunohistochemical investigations showed positive endothelial markers-FVIII, CD 31, CD 34 in the tumor cells. Elderly patients often have unusual manifestations of common illnesses. We describe a geriatric patient having a pneumopericardium from pericardial penetration by a gastric ulcer. Benign gastric ulceration causing perforation of the pericardium or other cardiac structures was recognized as early as 1854; however, until 1964, the condition was invariably fatal. Our patient survived the episode after early pericardiocentesis and medical treatment. Aspects of this uncommon clinical entity and a brief review of the literature are described.

We describe a case of congenital absence of the pericardium of a 42 year-old patient with a variant of Marfan's Syndrome involving the skeletal and cardiovascular system. The diagnosis was formulated not only through an accurate clinical examination, but also through non-invasive tests like a standard radiography of the chest, an echocardiography M-mode and 2-D and a doppler-analysis.

Congenital defects of the pericardium are unusual. Patients may experience exertional chest pain, cardiac arrhythmias, syncope, sudden death, or incarceration of myocardium, or they may be entirely asymptomatic. We describe the case of a symptomatic pericardial herniation diagnosed by echocardiography and confirmed by cineangiography. Successful repair was accomplished using a polytetrafluoroethylene soft-tissue prosthesis.

Two multiple trauma patients with total rupture of the pericardium and luxation of the heart into the left pleural cavity after blunt trauma are presented. One of the patients also had rupture of the posterior wall of the left ventricle with abundant bleeding. Both patients had intra-abdominal organ injuries, and emergency laparotomy was the first surgical procedure.
