Role of laparoscopy in peritonitis.

OBJECTIVE: Laparoscopy has gained clinical acceptance in many subspecialties in the last decade. The conventional open surgery for peritonitis carries significant morbidity and mortality. The present study was done to extend and evaluate benefits of minimally invasive surgery in this subset of patients. METHODS: This was a prospective study spanning over a period of four years. All those patients diagnosed as having peritonitis on clinical assessment and preoperative investigations and those who were stable enough haemodynamically were included in this study. After initial resuscitation for f…

Granulomatous peritonitis after laparoscopic cholecystectomy.

BACKGROUND: Granulomatous peritonitis may indicate a number of infectious, malignant, and idiopathic inflammatory conditions. It is a very rare postoperative complication, which is thought to reflect a delayed cellmediated response to cornstarch from surgical glove powder in susceptible individuals. This mechanism, however, is much more likely to occur with open abdominal surgery when compared with the laparoscopic technique. METHODS: We report a case of sterile granulomatous peritonitis in an 80-y-old female after a laparoscopic cholecystectomy. Management was conservative, and no relapse wa…

[Hypothermal sanation in acute peritonitis].

Comparative analysis of treatment of 78 patients, suffering an acute peritonitis, was conducted for studying of practical significance of hypertonic peritoneal-enteric sanation (PES). In 40 patients (control group) the conventional methods of treatment were used and in 38 (the main group)--the treatment was conducted on the PES background. Such clinical indices, as the patient state, body temperature, respiration rate, pulse and intestinal peristalsis, while the PES application, have had normalized earlier. As well the raising of the survival capacity of an organism, suffering critical state, …

Tuberculous peritonitis during infliximab therapy.

Reactivation of tuberculosis is a severe side effect of anti-TNF treatment. Especially extrapulmonary forms of tuberculosis may occur, which are difficult to diagnose. The diagnosis may be obtained by a thorough search for Mycobacterium tuberculosis. We describe two patients who developed tuberculous peritonitis after infliximab therapy that was prescribed for treatment of rheumatoid arthritis. These cases illustrate that tuberculous peritonitis has a nonspecific clinical manifestation and that Mycobacteria can be difficult to find in ascites fluid. For this reason, tuberculostatic therapy has…

Granulomatous peritonitis in hydatid disease.

We report a rare case of persisting granulomatous inflammation within the peritoneal cavity due to previous leakage from a liver hydatid cyst, with the granulomata bearing hooklets originating from protosolices of Echinococcus granulosus. The patient was treated with a full three-month course of albendazole. We believe this represents a rare case of 'pseudotuberculous peritonitis'.

Tuberculous peritonitis in uremic patients.

AIMS: To identify clinical features and treatment response of tuberculous peritonitis (TBP) in a uremic population, a retrospective case-control study was performed. MATERIALS AND METHODS: Thirteen uremic patients with TBP (Group I: 62.7 +/- 6.8 years, male 54%) collected between January 1986 and January 2002 were compared with another two age- and sex-matched controls: 19 non-azotemic patients with TBP (Group II: 62.9 +/- 7.3 years, male 47%) and 30 uremic patients without TBP (Group III: 61.1 +/- 8.7 years, male 47%). Clinical information and data of ascites and blood examinations were eval…

[Peritonitis secondary to gastrointestinal perforations].

BACKGROUND: The study was designed to check the etiopathogenesis of gastrointestinal perforations and to indicate possible factors determining better results in terms of mortality and morbidity. METHODS: A retrospective study was carried out on 128 patients operated during the last 10 years for gastrointestinal perforation at our Department of Emergency Surgery, where about 600 abdominal surgical operations are performed every year. In this study patients with perforated acute appendicitis and patients with postoperative anastomotic leakage were excluded. Surgical techniques were essentially

Persistent diffuse peritonitis in children.

In adults, persistent diffuse peritonitis (PDP) having a special microbiologic spectrum is now defined as a distinct intraabdominal infection because of its aggressive clinical course. Considering also other types of peritonitis, this study was performed to determine characteristics of childhood PDP in regard to clinical picture, microbiologic features, treatment and outcome. Classification of 175 patients with peritonitis showed that nine patients had primary peritonitis and 121, 37 and eight patients had secondary peritonitis, PDP, and intraabdominal abscess, respectively. Rates of host defe…

[Primary pneumococcal peritonitis in children].

Since the advent of antibiotics primary pneumococcal peritonitis rarely occurs except in association with immunodepression and a detectable port of entry involving the lung, upper airways, or female genital tract. This clinical picture was observed in an 8-year-old girl who underwent surgery after an initial diagnosis of appendicitis-related peritonitis. Surgical exploration revealed a macroscopically and microscopically normal appendix and purulent peritoneal effusion. Culture of a peritoneal fluid specimen identified a pneumococcus sensitive to beta-lactam agents. Samples from the throat, ur…

Nocardial peritonitis in a cat.

OBJECTIVE: To illustrate a successfully managed case of nocardial peritonitis in a cat. DESIGN: Case report. ANIMAL: An 8 year old neutered male Burmese cat presented for nonspecific signs of depression, anorexia, pallor and mild dehydration. Pyrexia, loss of weight and abdominal distension developed despite treatment with amoxycillin-clavulanate and supportive therapy. PROCEDURE: Various medical and surgical procedures were used. RESULTS: Haematology revealed severe inflammatory left shift. Biochemistry showed mild prerenal azotemia. A "ground glass" appearance to the abdomen was seen on …

Tuberculous peritonitis: an overlooked diagnosis.

Contrary to widespread belief, abdominal tuberculosis is not rare, nor is it confined to the poor or patients with active pulmonary tuberculosis. The disease must be considered in the differential diagnosis of abdominal pain.

[Primary acute peritonitis in adults].

The authors report 12 cases of primary acute peritonitis-that were operated over a period of 10 years, representing 2.8% of the total number of acute cases of peritonitis, with the exclusion of cases of postoperative peritonitis. Since they are so rare it is understandable that primary acute peritonitis of the adult are less well known by the general practitioner in surgery. The particular background of these patients, frequently involving other forms of pathologic features, and the generally depressed immunological background explains the atypical clinical evolution, with attenuated local abd…

The microbiology of neonatal peritonitis.

To review the bacteriology of neonatal intra-abdominal sepsis, we reviewed peritoneal cultures from 86 newborns undergoing operation for nec … d in 10% of NEC cases (0% of appendicitis group). These results indicate the unique bacteriology of peritonitis in the critically ill newborn and probably reflect abnormal colonization in the neonatal intensive care unit.

Spontaneous bacterial peritonitis: variant syndromes.

Spontaneous bacterial peritonitis (SBP), a fascinating disease that had been reported perhaps 50 times in varying guises over the preceding century, suddenly burst forth in the 1960s and was recognized in clusters of cases almost simultaneously in Paris, London, and West Haven, Connecticut. The spectrum of the disease has broadened. Initially, it was associated almost exclusively with alcoholic cirrhosis, but it has now been found in association with posthepatitic cirrhosis, cryptogenic cirrhosis, chronic active liver disease, and, occasionally, in biliary cirrhosis and cardiac cirrhosis. Rece…
