OBJECTIVE: To investigate whether a preoperative pessary test could be used to identify patients with occult stress incontinence and if simultaneous anti-incontinence surgery with a midurethral sling could prevent postoperative incontinence.
STUDY DESIGN: Eighty-two patients with a positive pessary test and severe genital prolapse participated in the study: 43 patients underwent prolapse surgery and transobturator tension-free vaginal tape (TVT-O) insertion and 39 patients underwent prolapse surgery only.
RESULTS: The objective cure rate for TVT-O and prolapse surgery was 90.7% at three mont

OBJECTIVE: The objective of the study was to evaluate the effectiveness of a new self-positioning women's incontinence pessary.
STUDY DESIGN: Thirty-two women were enrolled and followed up for 12 months. Evaluation included baseline questionnaires, physical examination including pelvic organ prolapse quantification (POP-Q) scores, cotton swab testing, and assessment of Kegel strength. A pad test and 7 day urolog were also performed before and after pessary fitting.
RESULTS: Incontinence questionnaire scores were all significantly reduced as percent of baseline: stress incontinence, 7 of 15 (

A silicone Gellhorn pessary, 3 inches in diameter, neglected for several years in an elderly woman, eroded through the anterior vaginal wall, ultimately to lie entirely within the bladder. General anesthesia and bilateral deep full-length Schuchardt's incisions were required to remove it. A Latzko procedure was done at a later date to close the large vesicovaginal fistula; similar Schuhardt's incisions were again used.

A case of neglected pessary is presented. After erosion through the anterior vaginal and bladder walls the device came to rest entirely within the bladder. Surgical management of removal of the pessary and repair of the resulting vesicovaginal fistula are described. Appropriate use of supportive pessaries as well as complications of misuse and neglect are discussed.

INTRODUCTION: We present a case of simultaneous ureterovaginal and rectovaginal fistulas caused by continuous placement of a cube pessary wi … ystostomy, transvaginal fistula closure, and anterior colporrhaphy was performed.
CONCLUSION: Cube pessaries require daily self-management, and continuous placement in elderly patients can lead to severe ischemic complications. Multidisciplinary surgical reconstruction is crucial to preserve renal function and restore quality of life.

OBJECTIVE: The objective was to determine patterns of pessary use in women prior to surgery for pelvic organ prolapse at one institution.
METHODS: Retrospective cohort study of women undergoing surgery for prolapse. Demographics, medical and surgical history, and treatment were collected from the medical record. Factors associated with pessary uptake and outcomes were explored using univariable and multivariable logistic regression. Kaplan-Meier plots were used to evaluate the relationship between key variables and time to surgery.
RESULTS: Among 1228 patients, the mean age was 60.8 (SD 29.2

BACKGROUND: Pessaries are commonly used to treat pelvic organ prolapse. While these devices generally pose negligible risk, serious morbidity can be associated with neglected use.
CASE PRESENTATION: A 91-year-old woman began to experience acute pain related to an impacted pessary. An Exam in the operating room under anesthesia revealed that the pessary was deeply embedded within the vaginal canal and was noted to have a hard metal wire under a rubber exterior. An intraoperative orthopedic consult was obtained to facilitate discussion and selection of tools outside the typical gynecological sc…

Vaginal pessaries have been used for millennia to alleviate symptoms of pelvic organ prolapse (POP). Despite their long-standing use, the success rate of pessary treatment is approximately 60%, and the underlying mechanisms of support are not well understood. This study aims to investigate three previously proposed hypotheses regarding the support mechanisms of pessaries, utilizing supine and upright magnetic resonance imaging (MRI): (1) support by bony structures, (2) support by levator ani muscles (LAM), and (3) the uterus keeping the pessary in place by acting as a lever. Thirty POP patient…

BACKGROUND: Pessaries are an effective treatment for pelvic organ prolapse, yet currently available pessaries can cause discomfort during removal and insertion. An early feasibility trial of an investigational, collapsible pessary previously demonstrated mechanical feasibility during a brief 15-minute office trial. Longer-term, patient-centered safety and efficacy data are needed.
OBJECTIVE: This study aimed to assess the effectiveness and safety of the investigational vaginal pessary for pelvic organ prolapse at 3 months.
STUDY DESIGN: This was a prospective, 7-center, open-label equivalenc…

Pelvic organ prolapse affects 40% of women over 40 years. Pessaries are often used as a first-line treatment and give high patient satisfaction. Complications of pessaries are rare, but vaginal erosions can lead to adhesions, haemorrhage, impaction and migration. This year we have seen an increase in pessary complications in our hospital after check-ups were postponed. In this article, we present a case of a complication of a vaginal pessary after the postponement of a follow-up visit in the COVID-19 era. An 85-year old woman had a pessary which had migrated into the bladder, 8 months after he…

INTRODUCTION: The primary objective of this study was to investigate the percentages of women choosing watchful waiting, pessary use or surgery as first-line treatment of pelvic organ prolapse (POP). Second, the rate and cause of discontinuation of pessary use were investigated.
METHODS: A retrospective chart review was conducted on 794 patients referred with POP at a Danish tertiary center for urogynecology at Aalborg University Hospital between 1 January 2014 and 31 December 2015. The following data were registered: age, BMI, previous use of a pessary, total number of births, vaginal births

BACKGROUND: Pelvic organ prolapse (POP) is a common chronic health issue. Pessary rings are used for conservative management. To date, there is little evidence on objective anatomical findings as predictors of successful ring pessary management.
AIM: To determine any association between history, clinical and four-dimensional translabial ultrasound (TLUS)/pelvic floor ultrasound examination and pessary success.
MATERIALS AND METHODS: From November 2013 to November 1015, all new patients presenting with symptomatic prolapse to a tertiary urogynaecological unit underwent an assessment including

AIMS: This study compared the effectiveness and safety of cervical cerclage combined with cervical pessary in the treatment of cervical incompetence and that of cervical cerclage alone and cervical pessary alone.
METHODS: We performed a prospective cohort study of women with cervical incontinence. Subjects were randomized into three groups: cerclage, pessary, and cerclage and pessary combined.
RESULTS: A total of 75 patients with cervical incompetence were enrolled: 37.3% patients had a cervical length <15 mm, and 62.5% had a cervical length of 15-25 mm. Patients with increased body mass ind

CASE: An 88-year-old woman visited us with a complaint of total urinary leak, and was diagnosed as having vesicovaginal fistula after pessary insertion. A part of pessary was seen in the bladder and it was removed by cutting pessary in the vagina. And then she was managed by antibiotics, local estrogen cream and Foley catheter indwelling.
OUTCOME: Vesicovaginal fistula was healed completely. This is the first successful case of conservative treatment of vesicovaginal fistula caused by vaginal pessary without surgical repair.
CONCLUSION: The conservative approach could be an option of treatme

Cervical incompetence is often complication in the II and III trimester of pregnancy and is a cause of 16-20% of premature deliveries. Treatment of incompetent cervix contains surgical methods ( cerclage and others) and conservative management as follow: tocolytics, hormones, bed rest and mechanical factors (pessary, rings, balloons etc). The aim of the study was evaluation of therapeutical efficacy Herbich cervical pessary in the cases of cervical incompetence I and II degree in II and III trimester of pregnancy.
