The Placenta: Identifying Normal.

PURPOSE: To provide information for the general pathologist about what is considered normal in the placenta and to differentiate findings that are abnormal or pathologic. BACKGROUND: The literature about the placenta is confusing. It can be opinionated, and terminology varies among authors. Even what is expected to be seen, which we refer to as normal, is not consistent among experts. RESULTS: This article focuses on what the general pathologist should know about normal macroscopic and histologic features of the placenta throughout gestation, as well as alterations that are controversial rel…

What is placenta accreta?

Accreta placentation is a clinicopathologic diagnosis at delivery when 1 or more placental lobules are abnormally attached into a myometrial scar or congenital defect requiring surgical removal. Over 90% of cases of placenta accreta spectrum are found in patients with a prior history of cesarean delivery presenting with a low-lying placenta or a placenta previa developing inside a lower uterine segment scar. Placenta previa accreta is a complex obstetric condition, and management strategies and clinical outcomes are directly linked to the quality of epidemiology data. We have recently question…

Treating the dysfunctional placenta.

Placental dysfunction underlies major obstetric diseases such as pre-eclampsia and fetal growth restriction (FGR). Whilst there has been a little progress in prophylaxis, there are still no treatments for placental dysfunction in normal obstetric practice. However, a combination of increasingly well-described in vitro systems for studying the human placenta, together with the availability of more appropriate animal models of pre-eclampsia and FGR, has facilitated a recent surge in work aimed at repurposing drugs and therapies, developed for other conditions, as treatments for placental dysfunc…

Placenta accreta: conservative approach.

Placenta accreta refers to any abnormally invasive placental implantation. Diagnosis is suspected postpartum with failed delivery of a retained placenta. Massive obstetrical hemorrhage is a known complication, often requiring peripartum hysterectomy. The authors report a case of placenta accreta in a primiparous patient with multinodular leiofibromyomatosis of the uterus following failed manual removals of a retained placenta. They describe a conservative management in a stable patient desiring future fertility with a unilateral prophylactic uterine artery embolization, a multidose regimen of …

[Placenta in gestational hypertension].

At an incidence of 3.2-4% world-wide, pregnancy-induced hypertension (PIH) is the most common disease of pregnancy. Since this holds a risk, not only for the mother, but also for the child, the placenta should undergo pathological-anatomical examination in every case. Pathomorphological findings can be described in the feto-maternal border zone as well as in the fetal placenta. These are not, however, specific, nor do they offer diagnostic proof. Pathomorphological findings in the feto-maternal border zone: defective invasion of the extravillous cytotrophoblast, hyperplastic arterio-/arteriolo…

Circumvallate placenta: sonographic diagnosis.

We prospectively identified 12 patients who had a shelf at the margin of the placenta consistent with the diagnosis of circumvallate placenta. Ten patients were followed to delivery and three placentas were sent for pathologic analysis. Circumvallate placenta was diagnosed by gross inspection in all three but was confirmed by microscopic analysis in only two. One of these patients had placental abruption and cesarean delivery for fetal distress at 27 weeks. Circumvallate placenta is another cause of uterine band, sheet, or shelf. The finding can be associated with a normal obstetric outcome bu…

[Pathology of the placenta. V. Circulatory disorders of the placenta. Fetal vascular system].

Discussed are circulatory disorders as well as pathologico-anatomic findings recordable from fetal vessels of the placenta. Thorough reference is first made to obliterative endoarteritis of the greater arteries. Its forms are described, with formal and causative pathogenesis being discussed. Conclusive coverage of causative pathogenesis has proved to be possible only for unambiguously determined inflammatory manifestations (rubella, lues). The second major complex covers alterations to greater and minor fetal vessels which are characterized by central parietal thrombosis (possibly associated w…

Abruptio placentae and placenta previa: frequency, perinatal mortality, and cigarette smoking.

Abruptio placentae and placenta previa are responsible for at least half the excessive fetal and neonatal death associated with maternal smoking during pregnancy. Data from a large prospective study were analyzed to determine if stopping smoking reduced the frequency of these disorders. Mothers who stopped smoking had a 23% lower frequency of abruptio placentae and a 33% lower frequency of placenta previa than women who continued to smoke during pregnancy. The mothers who stopped smoking had only about half as many fetal and neonatal deaths due to abruptio placentae and one third fewer losses …
