Plasmablastic lymphoma through clonal expansion 17 years after EBV-positive DLBCL: case report and review.

Plasmablastic lymphoma (PBL) is a rare and aggressive subtype of B-cell lymphoma associated with immunodeficiency and Epstein-Barr virus (EBV) infection. Although PBL can develop from indolent lymphomas, its occurrence long after diffuse large B-cell lymphoma (DLBCL) is rare. A 67-year-old man was diagnosed with EBV-positive DLBCL 17 years ago and achieved a sustained remission following six cycles of R-CHOP therapy. Seventeen years after the initial DLBCL diagnosis, the patient developed PBL with EBV-positive tumor cells. After three cycles of EPOCH therapy, the patient achieved a partial res…

Plasmablastic lymphoma mimicking primary ovarian, tubal or peritoneal cancer: a case report and literature review.

INTRODUCTION: Plasmablastic lymphoma (PBL), an extremely aggressive B-cell non-Hodgkin lymphoma, is known to be associated with immunosuppression. PBL with primary peritoneal or ovarian involvement is extremely rare. Here, we report a case of PBL in an immunocompetent woman, which involved the right ovary, peritoneum, cervical lymph nodes, and bone marrow. CASE PRESENTATION: A 42 year-old immunocompetent premenopausal woman presented with abdominal dullness and distension. Imaging studies revealed the presence of ascites, ovarian mass, peritoneal seedings, and generalized lymphadenopathy. Lap…

Genomic characterization of HIV-associated plasmablastic lymphoma identifies pervasive mutations in the JAK-STAT pathway.

Plasmablastic lymphoma (PBL) is an aggressive B-cell non-Hodgkin lymphoma associated with immunodeficiency in the context of Human Immunodeficiency Virus (HIV) infection or iatrogenic immunosuppression. While a rare disease in general, the incidence is dramatically increased in regions of the world with high HIV prevalence. The molecular pathogenesis of this disease is poorly characterized. Here, we defined the genomic features of PBL in a cohort of 110 patients from South Africa (15 by whole exome sequencing and 95 by deep targeted sequencing). We identified recurrent mutations in genes of th…

Long-Term Survival of Primary Intracranial Plasmablastic Lymphoma: Case Report and Review of the Literature.

BACKGROUND: Primary intracranial plasmablastic lymphoma (PIPBL) is a rare malignant tumor. CASE DESCRIPTION: We present a case of PIPBL in a 32-year-old man who complained of a progressive growing, painful mass on the right parieto-occipital part of head. Computed tomography and magnetic resonance imaging revealed a homogeneously enhanced mass with partial bone destruction. The patient underwent total resection and cranioplasty in one stage. Histopathologic examination showed large tumor cells with immunoblast-like nuclei. Immunohistochemical staining displayed CD38(+), CD138(+), Mum-1(+), CD…

Plasmablastic haemato-lymphoid neoplasm with a complex genetic signature of Burkitt lymphoma responding to bortezomib.

Plasmablastic lymphoma shares many morphologic features with plasmablastic plasma cell myeloma. The activation of MYC oncogene in these lymphomas may be an important pathogenetic element associated with Epstein-Barr virus infection. We describe herein an elderly man with a plasmablastic lymphoid neoplasm displaying unique morphologic, cytogenetic and clinical features. This case might offer additional insights to the complex but fascinating topic of hybrid haemato-lymphoid neoplasms such as plasmablastic lymphoma-myeloma. In addition, the patient responded to the treatment with bortezomib. New…

Plasmablastic lymphoma involving breast: a case diagnosed by fine-needle aspiration and core needle biopsy.

Plasmablastic lymphoma (PBL) is a rare lymphoma originating from B-cells with terminal differentiation. Most common anatomic site involved by PBL is the oral cavity. Involvement of other body sites has only rarely been reported. Herein, we report a rare case of EBV-negative PBL involving the breast of an HIV positive 47-year-old woman. The patient presented with decreased vision and photophobia. During physical examination, she was found to have bilateral breast masses and multiple lymphadenopathy. Fine-needle aspiration of one of the breast masses showed large malignant cells with plasmacytoi…

Epstein-Barr Virus-Positive Plasmablastic Lymphoma Arising in the Anal Canal of an Immunocompetent Patient.

This case report describes a 57-year-old immunocompetent male diagnosed with Epstein-Barr virus (EBV)-positive plasmablastic lymphoma (PBL), a rare and aggressive subtype of diffuse large B-cell lymphoma (DLBCL), presenting in the anal canal. PBL is most commonly seen in immunocompromised patients, particularly those with a human immunodeficiency virus (HIV) infection; its presence in immunocompetent individuals is exceedingly rare. The patient presented with persistent anal pain, prompting a comprehensive diagnostic workup. Histopathological and immunohistochemical analysis confirmed PBL, wit…

Clinical and pathological aspects of human immunodeficiency virus-associated plasmablastic lymphoma: analysis of 24 cases.

Plasmablastic lymphoma (PBL) is a rare AIDS-related malignancy with a poor prognosis. Little is known about this entity, and no standard treatment regimen has been defined. To establish an adequate treatment strategy, we investigated 24 cases of PBL arising in human immunodeficiency virus-positive individuals. Most of the patients were in the AIDS stage, with a median CD4 count of 67.5/µL. Lymph nodes (58 %), gastrointestinal tract (42 %), bone marrow (39 %), oral cavity (38 %), and CNS (18 %) were the most commonly involved sites. Histology findings for the following were positive at varying …

Primary cardiac plasmablastic (diffuse large B-cell) lymphoma mimicking left ventricular aneurysm with mural thrombus.

Primary cardiac lymphoma is rare and is usually of the non-Hodgkin type. By definition, it involves only the heart and the pericardium, with no evidence of extracardiac disease. Primary cardiac lymphoma accounts for 1% of primary cardiac tumors and 0.5% of extranodal lymphomas [Gowda RM, Kahn RA. Clinical perspectives of primary cardiac lymphoma. Angiology 2003;54(5):599-604]. On the other hand, secondary cardiac involvement can occur in approximately 20% of patients with disseminated extracardiac lymphoma [Gowda RM, Kahn RA. Clinical perspectives of primary cardiac lymphoma. Angiology 2003;54

[Unique case of caecum plasmablastic lymphoma CD138(+) in patient with late diagnosed colon neuroendocrine carcinoma].

Neuroendocrine tumors are frequently associated with other primary malignancies. Plasmablastic lymphoma is a rare, aggressive neoplasm, derived from large B-cell, associated with human immunodeficiency virus infection. Plasmablastic lymphoma cells share many cytomorphologic and immunophenotypic features with plasmablastic cells, causing some diagnostic problems. We present a unique case of coexisting two very uncommon neoplasms: plasmablastic lymphoma and neuroendocrine carcinoma in 54-years-old men. This is the first report of caecum localization of plasmablastic lymphoma. Presented case imag…

Plasmablastic lymphoma involving the penis: a previously unreported location of a case with aberrant CD3 expression.

Lymphomas of the penis are rare and can either arise at this site or be a manifestation of systemic disease. We report the case of an elderly man with a plasmablastic lymphoma (PBL) involving the uncircumcised penile prepuce. The neoplasm was composed of plasmablasts positive for monotypic immunoglobulin lambda light chain, CD3, CD79a, CD138 and Epstein-Barr virus encoded RNA (EBER), and was negative for CD2, CD5, CD7, CD20, and PAX5. This case is highly unusual for at least two reasons. The penile foreskin is a rare location for lymphoma and PBL at this site has not been reported. Secondly, t…

Haematuria As the Initial Manifestation of Bladder Plasmablastic Lymphoma: A Report of a Rare Case and Literature Review.

Plasmablastic lymphoma (PBL) is an aggressive and uncommon variant of diffuse large B-cell lymphoma, most frequently associated with immunocompromised states. Although extranodal disease is characteristic, involvement of the urinary bladder is exceptionally rare and may mimic primary urothelial malignancy both clinically and histologically. We report the case of a 62-year-old man with a previous history of high-grade diffuse large B-cell lymphoma in remission who presented with acute gross haematuria. Imaging demonstrated bladder wall thickening with associated hydronephrosis. Transurethral re…

Plasmablastic lymphoma with cutaneous manifestations in an immunocompetent man: A rare case report with literature review.

Plasmablastic lymphoma (PBL) is an aggressive lymphoma and is regarded as a subtype of diffuse large B-cell lymphoma (DLBCL). It is generally found in immunosuppressed and transplanted individuals, but very less often in immunocompetent individuals. It shows positivity for Epstein-Barr virus (EBV) infection in 60% of cases. Few cases have been seen in immunocompetent patients, but rarely as cutaneous manifestations. In the recent past, very few studies have discussed the cytological features of PBL. Due to its rarity, diagnosis and management of this unique entity pose a challenge. Moreover, i…

Extranodal Involvement of the Anorectal Region in an HIV-Positive Patient With Plasmablastic Lymphoma: A Case Report.

Plasmablastic lymphoma (PbL) is a rare type of aggressive B-cell malignancy that has an extremely poor prognosis without chemotherapeutic treatment, requiring a high degree of suspicion for an early and accurate diagnosis. It has been classically described in patients infected with the human immunodeficiency virus (HIV). However, it accounts for only 2.6% of acquired immunodeficiency syndrome (AIDS)-related lymphomas. Extranodal involvement is most commonly seen within the oral cavity (44%). Involvement of the gastrointestinal tract (14%) is rare and can often be confused with other malignanci…

Cytological Diagnosis of Plasmablastic Lymphoma Involving the Parotid Gland: A Case Report with Review of the Literature.

Plasmablastic lymphoma (PBL) is a rare subtype of non-Hodgkin's lymphoma that is associated with acquired immunodeficiency syndrome (AIDS), characterized by its association with Epstein-Barr virus (EBV), aggressive nature, and plasmacytic/plasmablastic differentiation. PBL remains a therapeutic and diagnostic challenge. Diagnosis of PBL by fine-needle aspiration cytology (FNAC) is reported infrequently. We herein describe the cytodiagnosis of a rare case of HIV-negative PBL in a 58-year-old man without EBV infection presented by parotid swelling. The current case study highlights the cytomorph…
