AIMS: North America features some of the world's highest consumption levels for controlled psychoactive prescription drugs (PPDs; e.g. prescription opioids, benzodiazepines, stimulants), with non-medical use and related harms (e.g. morbidity, mortality) rising in key populations in recent years. While the determinants, characteristics and impacts of these 'use' problems are increasingly well documented, little is known about the 'supply' side of non-medical PPD use, much of which is facilitated by 'diversion' as a key sourcing route. This paper provides a select review of the phenomenon of PPD

INTRODUCTION: In recent years, there has been an increase in the non-medical use of psychoactive prescription drugs including pregabalin (PGB). Studies have shown that multiple drug users and patients in methadone treatment programs administered PGB at high dosages in order to achieve euphoria, reduce withdrawal symptoms, or potentiate the effects of methadone. For these reasons, accurate toxicological monitoring is required for these high-risk individuals.
MATERIALS AND METHODS: The present study investigated whether PGB could be detected in the hair samples of 250 patients with a history of

PURPOSE: Early detection of risky behaviors involving prescription opioids can assist prescribers in implementing safer prescribing. Patient-to-prescriber travel patterns may indicate potential opioid misuse. We introduce doctor hopping, patients bypassing nearby prescribers in favor of more distant ones, as a new spatial estimation of potentially risky behavior, and compare with traditional doctor shopping metrics.
METHODS: We examined all filled opioid prescriptions between 2015 and 2016 from the Arkansas Prescription Drug Monitoring Program. We calculated patient-to-prescriber travel times

BACKGROUND: The role of prescribed opioids and benzodiazepines as risk factors for opioid overdose are well established, however, their role as potential 'triggers' of opioid overdose has not been formally investigated.
OBJECTIVE: The objective of this study was to evaluate the temporal relationship between controlled substance acquisition and opioid overdose utilizing a case-crossover design.
METHODS: This study utilized Arkansas statewide data between 2014 and 2020. Prescription Drug Monitoring Program (PDMP) data were used to assess controlled substance acquisition and fatal and non-fatal

BACKGROUND: How state opioid policy environments with multiple concurrent policies affect opioid prescribing to individuals with acute pain is unknown.
OBJECTIVE: To examine how prescription drug monitoring programs (PDMPs), pain management clinic regulations, initial prescription duration limits, and mandatory continued medical education affected total and high-dose prescribing.
DESIGN: A county-level multiple-policy difference-in-difference event study framework.

BACKGROUND: As the nation works to improve the opioid epidemic, safer opioid prescribing is needed. Prescriber education is one method to assist with this aim. To gauge current surgical residents' opioid prescribing practices at a safety-net hospital, an evaluation was completed before a general surgery-specific opioid prescribing education (OPE) session. The effectiveness of this OPE was measured through a postparticipation evaluation.
METHODS: A voluntary, anonymous evaluation immediately before and after a one-hour OPE session was performed at an urban safety-net hospital. Descriptive stat

With the discovery of morphine in the early 1800s, substance abuse quickly followed. Next came the production of heroin and other synthetic opioids, along with increases in nonmedical use of prescription medications. In the 21st century, drug abuse and addiction continues to rise nationwide with the three most common drugs abused in adolescents being marijuana, synthetic marijuana, and hallucinogens. Among adolescents and adults nationwide, rates of alcohol, opioids, and amphetamine use have increased over the last decade. In South Dakota, the most prevalent drugs consist of alcohol, methamphe

More than 75 million Americans have chronic or recurring pain [1]. Such pain negatively impacts emotional and mental health, quality of life, and productivity and increases the use of health care resources [1-5]. Prescription opioids are widely used and may be effective analgesics for chronic, moderate-to-severe pain in appropriately selected patients, particularly when used as part of comprehensive pain management plans [4,6]. The increased use of prescription opioids over the last decade has been mirrored in an increase in the reported misuse, abuse, and diversion of these agents. Physician

Drug-drug interactions (DDI) are a major topic in programs for continuous medical education (CME). Many physicians are afraid of being trapped into charges of malpractice; however, DDI cannot be avoided in many cases. They belong to routine medical practice and it is often impossible to avoid them. Moreover, they do not just occur between drugs but between any kind of foreign substance (xenobiotica), such as food (e.g. grapefruit juice, broccoli, barbecue) as well as legal (e.g. tobacco smoke, caffeine and alcohol) and illegal drugs. Therefore, the medical challenge is not just to avoid any in

OBJECTIVES: Reducing the inappropriate benzodiazepine (BZD) prescriptions represents a challenge for health care systems worldwide. The 'Choosing Wisely' campaign recommends against the use of BZD in the elderly as the first choice for insomnia, agitation, or delirium. We aimed to determine whether a transparent monitoring-benchmarking together with educational interventions, on top of the internal publication of a targeted recommendation, could be effective in curbing BZD prescriptions.
METHODS: Multicenter before and after study in a network of five southern-Switzerland teaching hospitals.

OBJECTIVE: The South Carolina Department of Public Health (SCDPH) Prescription Drug Monitoring Program (PDMP) is used to monitor the prescribing patterns of controlled substances, including opioids and stimulants. Data collected by the National Center for Health Statistics show that the number of drug overdose deaths involving opioids and stimulants has been increasing since 1999. Although databases such as the PDMP monitor healthcare professionals' prescribing habits regarding opioids and stimulants individually, there has been little research regarding opioid-stimulant coprescribing patterns

This article proposes and develops novel components of community-oriented programs for creating and affording access to safe medication dispensing centers in existing retail pharmacies and in permanent or travelling pharmacy clinics that are guarded by assigned or off-duty police officers. Pharmacists at these centers would work with police, medical providers, social workers, hospital administrators, and other professionals in: planning and overseeing the safe storage of controlled substance medications in off-site community safe-deposit boxes; strengthening communication and cooperation with

BACKGROUND: Opioid use during pregnancy has been associated with adverse maternal and infant health outcomes. Prescription drug monitoring programs (PDMP) provide a population-based source of prescription data. We linked statewide PDMP and birth certificate data in Tennessee (TN) to determine patterns of prescription opioid and benzodiazepine use during pregnancy.
METHODS: We constructed a cohort of 311,217 live singleton births from 2013 to 2016 with prescription history from 90 days before pregnancy to birth. Descriptive statistics were used to describe opioid prescription patterns during p…

BACKGROUND: The opioid crisis continues to grow in the United States with 46,700 drug overdose deaths due to opioids in 2017 alone. Vet shopping, the practice of soliciting veterinarians for prescription medications, has been receiving national media attention in recent years. A 2014 review of Prescription Monitoring Drug Programs found less than 10 veterinary shoppers nationwide. Still much is unknown about the role of vet shopping and the opioid crisis. This study sought to understand the practice of vet shopping through the eyes of veterinarians practicing in Appalachian counties within the

Postmarketing surveillance (PMS) studies allow for the systematic and comprehensive monitoring of the beneficial and harmful effects of prescription drugs as well as their usage patterns. The objectives of PMS are to assess the uses and effects of new drugs under conditions of customary clinical use. The following methods are used to assess new drugs under these conditions: 1. the publishing of case reports describing a new adverse effect, drug-drug interaction, or an unexpected response to a drug; 2. voluntary adverse drug reaction reporting; 3. registries; 4. hospital-based intensive monitor

INTRODUCTION: The opioid crisis has increased morbidity and mortality in the U.S. In response, states have implemented policies targeting excessive opioid prescribing. Florida's House Bill 21, enacted in 2018, required prescribers to participate in prescription drug monitoring programs and limited acute opioid prescriptions to a 3-7-day supply.
METHODS: This study used a quasi-experimental interrupted time-series analysis of Florida Medicaid claims from 2016 to 2019 to evaluate whether House Bill 21 was associated with changes in opioid prescribing. The study included 2,813,775 Medicaid Manag…
