Precision psychiatry in clinical practice: What is clinically actionable, what is promising, and what remains experimental?

Precision psychiatry seeks to improve individual-level prediction, treatment selection, monitoring, and prevention by integrating clinical, biological, behavioural, digital, and contextual information. This structured narrative review assessed the clinical readiness of major approaches using a purpose-specific framework covering validity, external replication, incremental utility, actionability, patient benefit, feasibility, equity, and governance. Approaches considered clinically actionable in selected settings included structured longitudinal assessment and decision-linked measurement-based

[Exploring the risk factors of reoffending in patients under repeated compulsory treatment].

Introduction: Risk analysis, including the risk of recidivism, is of paramount importance in forensic psychiatry. Objective: The objective of this study is to examine which patterns may indicate an increased risk of recidivism in individuals with mental illness, and to identify factors that may have contributed to repeated court-ordered involuntary treatment. Method: In this retrospective study, authors analyzed the documentation of 301 inpatients, who were discharged from the National Institute of Forensic Psychiatry between 2000 and 2015. Sociodemographic, clinical, and crime-related informa

Palliative psychiatry for a patient with treatment-refractory schizophrenia and severe chronic malignant catatonia: case report.

BACKGROUND: Palliative psychiatry is an emerging field that suggests a role for palliative interventions in the management of severe and persistent mental illness (SPMI). Current literature describes using a palliative approach for patients with severe anorexia nervosa. To our knowledge, this is the first case report describing end-of-life care in a patient with treatment-refractory catatonic schizophrenia. CASE DESCRIPTION: We describe the case of a 49-year-old man with schizophrenia and severe chronic agitated/malignant catatonia who was hospitalized for ten months. Multiple treatment trial

Dexmedetomidine in Psychiatry: Repurposing of its Fast-Acting Anxiolytic, Analgesic and Sleep Modulating Properties.

Drug repurposing is a strategy to identify new indications for already approved drugs. A recent successful example in psychiatry is ketamine, an anesthetic drug developed in the 1960s, now approved and clinically used as a fast-acting antidepressant. Here, we describe the potential of dexmedetomidine as a psychopharmacological repurposing candidate. This α2-adrenoceptor agonist is approved in the US and Europe for procedural sedation in intensive care. It has shown fast-acting inhibitory effects on perioperative stress-related pathologies, including psychomotor agitation, hyperalgesia, and neu

Valproate-Induced Metabolic Syndrome.

Valproic acid (VPA) and its salts (sodium calcium magnesium and orotic) are psychotropic drugs that are widely used in neurology and psychiatry. The long-term use of VPA increases the risk of developing adverse drug reactions (ADRs), among which metabolic syndrome (MetS) plays a special role. MetS belongs to a cluster of metabolic conditions such as abdominal obesity, high blood pressure, high blood glucose, high serum triglycerides, and low serum high-density lipoprotein. Valproate-induced MetS (VPA-MetS) is a common ADR that needs an updated multidisciplinary approach to its prevention and d

A case report of suicidal behavior related to subclinical hyperthyroidism.

Abnormalities in thyroid function are associated with many psychiatric symptoms. We present a report of a 15-year-old girl who was admitted to the psychiatry inpatient unit with symptoms of suicidal behavior, irritability, and impulsivity. One year previously, she had become more short-tempered, and had started to cut her wrists impulsively. Laboratory tests revealed subclinical hyperthyroidism. She was treated with anxiolytic and antithyroid drugs, and her suicidal ideation and irritability resolved. This case demonstrates that subclinical hyperthyroidism can be associated with suicidal behav

Investigations into geriatric psychiatry challenges: AAGP Senior Investigator Award 2000.

The author summarizes the American Association for Geriatric Psychiatry Senior Investigator Award presentation delivered at the 13th Annual Meeting March 12-15, 2000, in Miami Beach, Florida, and reviews his research studies that define a trajectory leading to investigations of early detection and prevention of Alzheimer's disease (AD). Themes and critical transitions that fostered the research are emphasized, and effective collaboration, mentoring, and timing are discussed. These studies have combined neuroimaging and genetic methods to identify subjects before they develop AD, so that interv

Evaluation and management of the violent patient in emergency settings.

The evaluation and management of the violent patient in emergency settings is a complicated task for emergency psychiatric specialists. Etiologic considerations must be carefully weighed. The interplay of biologic, psychologic, and social factors has to be clearly recognized. Psychiatrists and physicians staffing emergency departments must be facile with the application of psychopharmacologic treatment. In addition, a familiarity and comfort with physical restraint must be developed. Interview styles and stances that diverge from traditional psychiatric "examining" should be employed. There is

Clozapine safety, 40 years later.

Clozapine is, and will remain in the coming years, an irreplaceable drug in psychiatry which has elective indication in treatment-resistant schizophrenia, suicide risk in schizophrenia spectrum disorders, aggressiveness or violence in psychiatric patients, psychosis in Parkinson's disease, prevention and treatment of tardive dyskinesia. Unfortunately, the drug is largely underused for many and serious side effects. Only a good knowledge of these side effects and of the main strategies to prevent their occurrence or minimize their impact can allow overcoming the underutilization of this valuabl

Premature mortality of people with severe mental illness: a renewed focus for a new era.

This perspective article applies public health principles to improve the physical health of selected populations with mental disorders. Two preventable adverse outcomes, poorer physical health and premature mortality, are described across mental disorders. Evidence of the lifetime effects of adverse childhood experiences and inequalities is presented: these are the 'causes of the causes'. Seven drivers of physical disorders are illustrated that drive preventable deaths and as doctors, psychiatrists must lead from the front to reverse rising mortality. Evidence supports universal and selective

Aggression in Teachers is Related to Role Conflict and Role Ambiguity as Occupational Stress.

BACKGROUND: Aggression in the workplace is increasingly recognized as a serious problem, but there are few studies about worker aggression toward outsiders in the workplace. We investigated the association between aggression and occupational stress among teachers. METHODS: This was a cross-sectional study of 1583 teachers, principals, and vice-principals. Aggression was measured using the Japanese version of the Buss-Perry Aggression Questionnaire (BAQ). The survey respondents were classified into tertiles according to the BAQ score. The high BAQ group was defined as the upper tertile for the

A randomized trial of integrated group therapy versus group drug counseling for patients with bipolar disorder and substance dependence.

OBJECTIVE: Although bipolar disorder and substance use disorder frequently co-occur, there is little information on the effectiveness of behavioral treatment for this population. Integrated group therapy, which addresses the two disorders simultaneously, was compared with group drug counseling, which focuses on substance use. The authors hypothesized that patients receiving integrated group therapy would have fewer days of substance use and fewer weeks ill with bipolar disorder. METHOD: A randomized controlled trial compared 20 weeks of integrated group therapy or group drug counseling with 3

Child and adolescent psychiatry: challenges, solutions, opportunities, and future directions.

It is estimated that, globally, the mean point prevalence of diagnosable mental disorders in children and adolescents is higher than 11%, and around half of cases of major mental disorders have their onset before the age of 18. Mental disorders with onset in childhood or adolescence have an enormous impact on the developing brain, body and personal identity, as well as on the short- and long-term social, educational and functional capacity of individuals. Child and adolescent psychiatry - as a discipline, profession, academic field, and network of clinical services - is still relatively young,

[Quality control in geriatric psychiatry: development and applicability of documented quality indicators].

The development and suitability testing of quality indicators for geriatric psychiatry. The indicators were developed by a group of experts and evaluated in terms of significance, scientific validity and practicability (according to QUALIFY). After a field test, qualitative interviews were conducted with the evaluators. All 24 indicators originally developed were classified as significant, 18 of them as scientific. Of these, 9 (by experts) and 8 (by evaluators) were rated as practicable. Indicators on delirium prevention, age-specific groups and liberty-depriving measures were rated as non-scien

Double successful buprenorphine/naloxone induction to facilitate cardiac transplantation in an iatrogenically opiate-dependent patient.

Buprenorphine/naloxone is used for the treatment of opioid dependence. In the following case, a potential use for the medication combination is explored in the arena of transplant surgery. Psychiatry was consulted for a 29-year-old woman with iatrogenic opioid dependence after bilateral ventricular assist device placement for congenital cardiomyopathy. Her ejection fraction was less than 15% and she was considered a poor candidate for transplant due to drug-seeking behaviors. We transitioned her onto buprenorphine/naloxone to prevent abuse and control symptoms, qualifying her for cardiac trans

[Changing Forensic Mental Health in France: A Review].

This article describes the background and recent changes in French forensic mental health. The literature suggests that three law reforms have been crucial to changes in the mental health system. First, the Penal Code of 1992 redefined the provisions of criminal responsibility and introduced the category of diminished responsibility. Second, a controversial law for preventive detention (rétention de sûretê) was enacted in 2008, according to which criminals with severe personality disorders are subject to incarceration even after the completion of their prison sentences if they are still consid

Precision medicine in mental health: applications, challenges, and recommendations.

Mental disorders represent a major and growing public health challenge in Europe and worldwide, characterised by marked clinical, biological, and functional heterogeneity, that limits the effectiveness of current diagnostic and therapeutic approaches. In recent years, advances in precision medicine have initiated a paradigm shift in psychiatry, offering new opportunities to improve prevention, prediction, diagnosis, treatment selection, and long-term management by integrating biological, psychological, social, and environmental information. This EPA Guidance Paper provides an overview of the cu

Headache psychiatry: A structured narrative review and integrative framework.

Headache disorders involve complex interactions between neurobiological, psychiatric, behavioral, and social factors. Despite robust evidence of psychiatric comorbidity and psychosocial influences, these dimensions remain inadequately integrated into clinical practice and conceptual models. This review synthesizes evidence linking psychiatric factors with headache disorders and proposes Headache Psychiatry as a unified, integrative framework, operationalized through the Psychocephalology model. We conducted a structured narrative review across PubMed, PsycINFO, and Web of Science (1990-2025),
