Penile Priapism, Clitoral Priapism, and Persistent Genital Arousal Disorder: A Contemporary Review.

INTRODUCTION: Priapism is a persistent erection that arises from a dysfunction of the normal regulatory mechanisms of penile tumescence, rigidity, and flaccidity. It is defined as an erection lasting longer than 6 hours that is not related to sexual stimulation. There are three types of priapism: ischemic, non-ischemic, and stuttering. Similarly, clitoral priapism may occur in females manifested by symptoms such as engorgement with pain and swelling of the clitoris and surrounding tissue. Persistent genital arousal disorder (PGAD) is uncontrollable genital arousal in females, with or without o…

A Review of Antipsychotics and Priapism.

INTRODUCTION: Pharmacologically induced priapism is now the most common cause of priapism, with approximately 50% of drug-related priapism being attributed to antipsychotic usage. The majority of pharmacologic priapism is believed to result in ischemic priapism (low flow), which may lead to irreversible complications, such as erectile dysfunction. It is imperative that prescribing physicians be aware of potentially inciting medications. OBJECTIVES: To identify medications, specifically antipsychotics, associated with priapism and prolonged erections and understand the rates and treatment of t…

[Priapism in children and adolescents].

Priapism is a prolonged erection without sexual stimulation. In most cases it requires prompt management in order to avoid long-term consequences such as erectile dysfunction or penile deformity. Priapism is rare in children, but haematological diseases play an important role. The diagnosis is generally not difficult. Blood gas analysis and Doppler sonography are used to establish the form of priapism and determine its further management. The management of low-flow priapism consists of immediate blood aspiration and intracavernosal injection of sympathomimetic drugs as needed. In case of treat…

Advances in the understanding of priapism.

Priapism, a persistent penile erection lasting longer than 4 hours and unrelated to sexual activity, is one of the most common emergencies treated by urologists. Priapism can be categorized as ischemic, recurrent ischemic (stuttering), and non-ischemic. Advances in understanding the pathophysiology of various types of priapism have led to targeted management strategies. This review aims to provide an up-to-date picture of the pathophysiology and management of priapism. A search of Medline and PubMed for relevant publications using the term "priapism" was performed. In addition to the "classica…

[Diagnosis and management of priapism].

Priapism is defined as an erection for more than 4 h without sexual stimulation. The most common form with nearly 95% is the ischemic or low-flow form, which is very painful. The other 5% are comprised of nonischemic high-flow type usually caused by a blunt perineal trauma and the recurrent or intermittent so-called stuttering priapism. Anamnesis as well as physical and laboratory examination are important during the diagnostic workup. Patients who suffer from sickle cell anemia are predestined to develop priapism. Priapism constitutes a urological emergency because especially the low-flow typ…

A case of priapism with risperidone.

Priapism is a urologic emergency defined as a prolonged, possibly painful, penile erection. There are several known causes of priapism including psychotropic medications. One of the mechanisms by which antipsychotics are believed to induce priapism is through alpha-1 antagonism. This is case of a 50-year-old male with a history of schizophrenia with previous priapism related to trazodone, who presents with new onset priapism associated with risperidone. In this case, the treatment of priapism includes discontinuation of the offending agent and drainage of the corpus cavernosum twice along with…

Priapism as a manifestation of tetania.

In a 40-year old male admitted because of anginal chest pain, priapism developed after coronary angiography. Chest pain and priapism most probably were due to severe hypocalcaemia 6 weeks after thyroidectomy. Both chest pain and priapism disappeared after substitution of calcium.

Priapism in the oncology setting.

N.L. is a 20-year-old Caucasian man with no significant medical history. He presented to the hospital with a two-day history of feeling ill with intermittent nausea, vomiting, blurred vision, and headache. He also noted slurred speech and left-upper-quadrant fullness for almost a month.

[Priapism: diagnosis and treatment].

Priapism, prolonged and painful erection, is an exceptional cause of consultation in the pediatric population. High- and low-flow priapism and recurrent prolonged erection must be differentiated, as the prognosis and treatment of these entities differ. Assessment of patients with priapism begins with a detailed history, physical examination, and complete blood cell count. The definitive diagnosis is given by penile Doppler ultrasonography, corpora cavernosa blood gas analysis, and pelvic arteriography. We present two patients who attended our emergency service in the last year and propose an a…

Management of priapism in the newborn.

We report a case of priapism in a newborn and the results of a review of the publications on neonatal priapism. A preterm newborn developed idiopathic priapism on the first day after birth. The priapism disappeared after 4 days of observation. Periods with normal penile erection were reported at follow-up after 20 months. The results of the literature review also advocate management by observation alone.

Antiandrogens in the treatment of priapism.

Recurrent priapism in young men is a potentially devastating condition that may result in irreversible penile fibrosis. Hormonal manipulation using estrogens and gonadotropin-releasing hormone analogues has been successful in treating episodes of priapism refractory to other treatment forms, but it is associated with significant adverse effects, in particular the loss of libido and erectile function. We present 2 patients with sickle-cell disease and 1 patient with a spinal cord injury who had recurrent and refractory priapism. All 3 patients were successfully treated with low-dose antiandroge…
